Summary of RFP 36C26126R0014

This is a Request for Proposal (RFP) for an Indefinite Delivery Indefinite Quantity (IDIQ) fixed-price contract to establish and operate a Community Based Outpatient Clinic (CBOC) in Yreka, California, serving beneficiaries of the VA Northern California Health Care System. The contract period spans ten years from September 1, 2026, through August 31, 2036, with a guaranteed minimum of $1,000 and a ceiling of $18,000,000. The solicitation employs per member per month (PMPM) capitation-based billing for primary care services, with estimated patient panels declining from 946 members in year one to 882 in year ten. The proposal due date is April 24, 2026, at 10:00 AM PDT, with contract award anticipated shortly thereafter. This is a total set-aside for certified Service-Disabled Veteran-Owned Small Businesses (SDVOSB), requiring offerors to be registered in the SBA certification database and comply with 50% limitations on subcontracting for service contracts.

The contractor must provide comprehensive primary care services including preventive care, chronic disease management, mental health integration, pharmacy services, telehealth capabilities, and specialty care coordination. Mandatory staffing includes primary care providers (physicians, nurse practitioners, or physician assistants), registered nurse care managers, clinical associates, administrative staff, clinical pharmacist practitioners, social workers, dietitians, and specialty providers (podiatrists, physical therapists). The facility must be located within Yreka city limits, operate minimum business hours of 7:30 AM to 5:30 PM Monday through Friday, achieve VA performance standards for access and quality of care, and comply with all VHA policies, Joint Commission accreditation standards, and VA security and privacy requirements. Proposals are evaluated on technical capabilities (quality standards, staffing qualifications, continuity of care, IT security), past performance, and price, with non-price factors considered significantly more important than pricing.
