About this file
This document is a provider guidance document for the REACH VET (Recovery Engagement And Coordination for Health - Veterans Enhanced Treatment) program, updated July 6, 2021. The document outlines a six-step process that healthcare providers must follow when notified of high-risk Veterans they serve.
Providers receive notification from a REACH VET Coordinator about identified high-risk Veterans and are instructed to complete all tasks by the fourth Wednesday of each month, with outreach to Veterans occurring within one week of notification. The process requires providers to: (1) receive notification; (2) optionally access a clinical dashboard for patient-level data; (3) comprehensively re-evaluate the Veteran's care by reviewing screening evaluations, diagnoses, current problems, and treatment plans to ensure access to all needed evidence-based services; (4) consider treatment enhancement strategies including enhanced communication, safety planning, increased monitoring of stressful life events, and interventions to enhance coping strategies; (5) contact the Veteran using REACH VET Talking Points to provide program information, inform them of enhanced care identification, assess current symptoms and stressors, and collaboratively discuss treatment plan modifications; and (6) document all activities in the Computerized Patient Record System (CPRS) using the national REACH VET PROVIDER NOTE template. Dashboard access is automatically granted to clinicians with existing CPRS access, with the REACH VET SharePoint serving as a reference for access issues.
