L.4 PROPOSAL FILES
1. Format.
The submission needs to be clearly indexed and logically assembled. Each volume should be clearly identified and begin at the top of a page. All pages of each volume need to be appropriately numbered and identified by Offeror name, date and solicitation number in the header and/or footer. Create a Master Table of Contents and a Table of Contents for each volume. Use the following page setup parameters with MS Word (doc) files:
Table Three: Page Setup Parameters
Margins Top, Bottom, Left, Right - 1 Page Size, Width - 8.5 Gutter – 0 Page Size, Height - 11 From Edge Header, Footer - 0.5
Both hard and electronic submissions are to follow the above format. Additional direction for electronic packaging is in the paragraph below.
2. Electronic File Packaging.
(a) Complete electronic volumes are to be submitted in Microsoft Office format; readable by Microsoft (MS) Word, MS Excel, MS Power Point, and/or Adobe Acrobat (.pdf). Do not password protect electronic copies. All MS Excel formulas and hidden columns shall be viewable by evaluators. All electronic copies must be submitted on non-re-writable digital versatile disks (DVD-R) in a protective sleeve. Proposals submitted via e-mail will not be evaluated.
(b) Submit each volume on a separate DVD-R. Clearly mark the DVD-R and protective sleeve with volume number, Offeror name, and solicitation number. Each directory is to contain a Master Table of Contents for that Volume. Do not use "Pivot Tables" in Offeror spreadsheets. Offerors must ensure electronic copies (DVD-R) are virus free.
Electronic versions are to be an exact duplicate of the original hardcopy proposal. The Government is not responsible for identifying inconsistencies between the two and may rely on either version at its discretion. Page format restrictions and limitations (as identified in Table Two and Three) apply. Sound or video (e.g. MPEG) files are not to be embedded into the proposal files.
(c) Cost/Price DVD-R requirements: Files contained on the Volume IV Cost DVD-R may not be password protected. If the Cost/Price Volume contains electronic links, the links must be intact and maintained through all revisions.
(d) No cost information shall be presented in any part of the proposal except the UNSANITIZED cost proposal.
Proposal volumes shall not contain classified data. NO ZIPPED FILES are permitted.
3. Content Requirement.
(a) Confine all information to the appropriate file. If information is not located in the appropriate file, that information may not be evaluated and may be considered omitted. The Offeror should limit information to essential matters, sufficient to define the proposal in a clear and concise manner, to permit a complete and accurate evaluation of each proposal.
(b) Number all pages in each volume sequentially with Arabic numerals (1, 2, 3, etc.). Information that can be construed as correctly belonging in another volume of the proposal may be so construed and counted against the appropriate volume’s page limitation (the Government will not double count pages).
(c) Pages submitted in excess of the respective volume page limit will be removed and will not be evaluated by the Government.
(d) Title pages, tables of contents, cross-reference matrices, glossaries, acronym lists, page tabs, and section dividers that do not contain material that can be construed as proposal information will not be counted as part of the page limitations. However, these pages shall be numbered with lower case Roman numerals (i, ii, iii, etc.) or marked as tabs as appropriate to provide ease of reference for the Government.
(e) At a minimum, each volume of the proposal is to consist of a Table of Contents and a narrative discussion. Any proprietary information shall be clearly marked as such. Content for each proposal volume narrative discussion is prescribed in the following volume instructions.
L.5 VOLUME 1 - GENERAL
1. Proposal Transmittal Letter. A letter formally transmitting the proposal shall include the following:
(a) Statement of Compliance. Each Offeror shall include a statement indicating complete compliance with the solicitation, or detailed analysis of any objections, exceptions, contingencies, or additions. Any objections, exceptions, contingencies, or additions shall include an appropriate rationale and must be cross referenced to the particular paragraph(s) in the solicitation document to which they apply.
(1) Offerors are cautioned that they take exception to any requirements (including objectives and constraints) or Terms and Conditions of the solicitation at their own risk. The Government may, at its option, reject an offer which contains any such exceptions. Nonetheless, any exception taken by the Offeror, at its own risk, shall be identified in the Statement of Compliance.
(2) If no exceptions are taken, the Offeror shall include the following statement in the Statement of Compliance:
(3) “[Name of Offeror] takes no exception to any requirement or Terms and Conditions of Solicitation No.

Each Offeror shall indicate the cost to the Government for acquisition of each and all proprietary information identified in the proposal. Each Offeror shall carefully complete the appropriate representations and certifications, keeping in mind the above requirements.
(c) Format and Content. Each Offeror shall describe any deviations of their proposal from the specified format. If the content of the Offeror’s proposal differs significantly from these guidelines, Offerors shall state the differences and explain the reason for the differences.
(d) Joint Venture/Teaming Arrangements. Each Offeror shall provide, if applicable, a summary describing the Joint Venture/Teaming Arrangement established for this RFP and documentation evidencing the legal relationship among the joint venture/teaming parties. Documentation is to be included as an attachment to the proposal transmittal letter.

Include a statement certifying that all personnel employed to perform under the resultant contract meet the qualifications (to include the requisite security clearances, and National Agency Checks) in the DD Form 254, Contract Security Classification Specification, at Section J, as of the performance start date. Along with this statement please provide the following for both the Prime and Subcontractor(s):

(f) DCMA and DCAA Points of Contact. Each Offeror shall provide their cognizant field office information to include the point of contact’s name, address, phone number, and e-mail address. Offerors shall include this information for all subcontractors. Each Offeror shall provide their most recent DCMA Forward Pricing Rate Agreement (FPRA), Forward Pricing Rate Recommendation and/or DCAA Provisional Rate Agreement, as well as any DCAA audit reports relative to your direct labor, indirect expense, and any other rates or factors. If you are an Offeror who does not have an assigned DCAA representing office, please contact your regional DCAA office for the contact information to supply in response to this RFP.
1. If a current FPRA, FPRR or DCAA/DCMA report/memorandum providing recommendations regarding your forward pricing rates does not exist, then Offeror and its subcontractor(s) shall provide the following:
a. Forward Pricing Rate proposal (FPRP) to include documentation to validate the proposed direct and indirect rates.
Examples of documentation which may be submitted for proposed direct labor rates could be recent actual direct labor rates supported by payroll records, and/or paystubs, Letters of Intent (LOIs), or salary survey data. If providing LOIs, you and/or your subcontractors shall include the direct hourly labor rate for the proposed employee, identify the position, submit the LOI on your subcontractor’s letterhead (as applicable) with both the prospective employee’s and the employer signatures, and state that the LOI is for the subject procurement. You and/or subcontractors shall provide the range of rates for each labor category based on its salary structure.

Documentation for the FPRP indirect expense rates shall include the forecasted expense pool (by expense account) and allocation base detail, along with the last three years’ actual pool (by expense account) and allocation base information. This information shall include any incurred cost submissions along with your and/or your subcontractors’ certifications.
2. The rates reflected in the FPRA, FPRR, FPRP, or DCAA/DCMA audit or review should directly match the proposed rates as detailed in the proposal. If the proposed rates do not match the FPRA, FPRR, FPRP, or DCAA/DCMA audit or review, you and/or your subcontractors shall provide sufficient detail explaining how the proposed rates are realistic.
(g) Direct Billing Authorization. Contractors approved by DCAA for direct billing authorization shall provide, as part of their proposal, a copy of their DCAA direct billing authorization memorandum. This memorandum will be signed by the cognizant DCAA office manager and state that the contractor has adequate internal controls over its billing system and submits incurred cost proposals and final vouchers per FAR 52.216-7; therefore, provisional DCAA approval of interim vouchers is not needed.
(h) Disclosure of Potential Organizational Conflicts of Interest. The Offeror shall disclose complete information of any work performed by their company that is in any way associated with the contemplated acquisition or which could result in a potential organizational conflict of interest. Should a conflict of interest, or appearance of a conflict of interest, exist, the Contracting Officer shall determine the impact of the matter on the acquisition and the appropriate actions to be taken as a result of the determination. Should a potential conflict of interest exist, the Offeror is encouraged to contact the Contracting Officer prior to proposal submission for a determination prior to expending resources in proposal preparation.
(i) Solicitation Documents. Each Offeror shall complete (fill-ins and signatures) for the RFP/solicitation sections listed below. An authorized official of the firm shall sign the SF 33. Documents with original signatures shall be submitted as part of the hard copy document submission. PDF versions of the signed documents shall be submitted as part of the electronic DVD submissions. An Offeror's accounting system shall be deemed adequate prior to award.
Offerors shall provide the date the Government deemed its accounting system adequate and name the Federal Government agency that provided the audit. Offerors shall also submit a copy of the audit report with their proposal.
Offerors who have an adequate accounting system, but do not have a DCMA/DCAA audit report to submit as verification of adequacy, shall complete the “Pre-award Survey of Prospective Contractor Accounting System Checklist”, attached to this solicitation at Section J, to document how the accounting system is designed to meet the SF 1408 criteria. The WBS shall be at a minimum of the third level. All hours shall be sub-totaled at each WBS element level and then totaled at each of the next highest levels. Many times, the WBS is a compilation of tasks from various functional areas, resulting in input from several people who are considered experts. If this is the case, then one person responsible for compiling the WBS should ensure there is no duplication and that all work for the requirement is included. Adjustments to the format below should be made only as required by the Offeror’s specific technical approach. No cost information shall be provided within this WBS.
