SECTION 1
Introduction This report summarizes the efforts of the 2019 Annual Stormwater Site Compliance Evaluation for the U.S. Environmental Protection Agency (EPA) as per the Multi-Sector General Permit (MSGP) for Stormwater Discharge Associated with Industrial Activities at Mountain Home Air Force Base (MHAFB) under the National Pollutant Discharge Elimination System (NPDES). In addition, this evaluation meets the general requirements outlined in the Basewide 2016 Stormwater Pollution Prevention Plan (SWPPP).
The following report summarizes the activities performed and the observed findings.
1.1 Notice of Intent
In compliance with the provisions of the Clean Water Act (CWA) as amended (33 U.S.C. 1251 et seq.), operators of stormwater discharges associated with industrial activity located in Idaho are authorized to discharge to waters of the United States in accordance with the eligibility and Notice of Intent (NOI) requirements, effluent limitations, inspection requirements, and other conditions set forth in the June 4, 2015 MSGP. MHAFB’s MSGP tracking number is IDR050002. The discharge authorization date for MHAFB is 30 days after the EPA posts the dischargers’ NOI. The NOI deadline for Idaho was November 9, 2015; MHAFB’s effective date was December 12, 2015.
1.2 Re-Certification
The re-certification for the 2019 site inspections is provided in Appendix A. A copy of the signed re-certification (to be signed by Paula Jo Brown, Deputy Director, A6/7 Directorate) will be placed after the original certification in the original hardcopy SWPPP located at the 366th Fighter Wing, Installation Management Division, Environmental Compliance (366 FW/A7IE) office. Copies of this signed re-certification wil l be distributed (by email or by hand delivery) to the individual facilities and inserted into the specific facility Stormwater Pollution Prevention Handbook (SWPPH).
1.3 Scope of Compliance Evaluations
Site evaluations and reporting efforts were completed by representatives from CH2M including Mike Wirtz, P.E. and Rick McCormick, P.E. The Base vicinity is depicted in Figure 1 and the MHAFB industrial sector sites are identified in Figure 2. Mike Wirtz, P.E. physically inspected and reviewed quarterly assessment forms for 29 industrial facilities between the dates of October 20 through December 12, 2018 and evaluated activities designated in the original SWPPP that present evidence of, or potential for, pollutant releases to the MHAFB surface water discharge system.
The inspections evaluated measures to reduce pollutants and were used to determine whether the measures are adequate and properly implemented in accordance with terms of the permit or whether additional control measures are needed. The inspections also evaluated measures to document and record spills [see Appendix B Hazardous Material Spill Logs for 2016-2018 as maintained and provided by 366 FW/A7IE Installation Management Division (Environmental Compliance)]. An evaluation of each industrial facility was documented on an evaluation form (see Comprehensive Site Compliance Evaluation and Photo Log portions of Appendix C). The site-specific evaluations focused on the following items:
• Checking structural stormwater management measures, sediment and erosion control measures, and other structural pollution prevention measures identified in the SWPPP to make sure that these measures are operating correctly and have been documented on each facility’s site drainage maps;
SECTION 1 – INTRODUCTION
1-2 AX1228181110BOI
• Visually inspecting equipment needed to implement spill response to ensure adequate spill containment materials are available, accessible to staff, and that Safety Data Sheets (SDS) identify chemicals that may be handled at that facility are readily available;
• Examining and documenting spills (recent spills are recorded on the facility site drainage maps) that have occurred over the last 3 years to the date of inspection from industrial equipment, drums, barrels, tanks, vehicles, or other items that may leak (a list of spills are identified in Appendix B);
• Checking for evidence of, or the potential for, pollutants entering the drainage system.
A description of potential pollutant sources and pollution prevention measures and controls that require corrective action have been reviewed and identified in Section 2.0 of this report. Identification of corrective action issues are based on the field investigation of this year’s site compliance inspection and are summarized in Table 2. As required, revisions to the SWPPP for corrective actions and recommendations have been included in the EPA Annual Reporting Form in Appendix E. Updates of the individual facility SWPPHs (revisions to Section 3 of the Basewide SWPPP) are included in Appendix C.
AX1228181110BOI 2-1
SECTION 2
Inspection Findings This section summarizes the primary findings of the annual site compliance evaluation efforts, detailed information can be found in the various appendices attached.
2.1 Basewide Discharge Location(s)
Outfall #001, also known as McCalley Dam, is the main stormwater outfall for MHAFB and is located on the west perimeter of the Base just south of the Wastewater Treatment Plant, See Figure 1. The last three benchmark sampling events at Outfall #001 occurred on May 22, 2010, January 21, 2012 and February 8, 2017. The 2010 and 2012 sampling events were inspected by the United States Geological Survey (USGS). CH2M performed benchmark sampling during the February 8, 2017 discharge at the outfall. Discharge and sampling records documented by USGS and CH2M are included as Appendix D.
Records for this time period include gage height readings at the McCalley Dam (Outfall #001), pictures, quarterly visual assessment reports, and water sampling results for benchmark sampling. In the event of significant precipitation, Outfall #001 is the primary stormwater discharge point for MHAFB.
2.2 Best Management Practices (BMPs)
Best Management Practices (BMPs) are implemented Basewide and enforced on a facility-by-facility basis. Facility-specific BMPs are listed in the individual Facility SWPPH (Section 3 of the MHAFB SWPPP).
No significant deficiencies were noted during the inspection process (October 2018 through December 2018). Military personnel are trained for the storage, handling, and emergency spill response of hazardous materials; worker safety and hazard communication; and additional facility-specific training before they are assigned work tasks. Training for military personnel is documented on AF-55 forms or logged on a SWPPP Review Log provided in the Facility SWPPP. Training is completed annually;
records are maintained on site at each facility and are available for review upon request.
Some of the facilities at MHAFB have had military personnel changes, additions, and augmentation to their staff within the last 12 months. As part of this comprehensive site compliance evaluation, CH2M personnel instructed each Facility Stormwater Pollution Prevention (SWPP) Manager of his or her responsibilities for documenting and reporting spills to 366 FW/A7IE, maintaining spill response kits, and making sure that military/non-military staff working in their respective facilities are updated and trained on BMPs. As required under the Facility SWPPH, the Facility SWPP Manager is responsible for training facility staff and recording the training under the SWPPH review log (or on the AF-55 forms with a note specifying SWPP training). Suggested training videos related to SWPP and the CWA are available through 366 FW/A7IE. Each video is approximately 20 minutes long and discusses typical BMPs used in day-to-day industrial operations. The close-out meeting PowerPoint presentation is also available for the Facility SWPP Manager’s use in training personnel. A summary of monitoring, inspection, reporting, and training outlined in the MHAFB SWPPP is shown in Table 1.
2.3 Current Construction
Construction activities within the general industrial sectors of MHAFB were also reviewed during the period of the 2018 comprehensive site inspections. The following is a list of construction projects which have occurred (or are currently underway) since January 2018:
• Bldg 278, Repair, Major Renovation & Addition, 389 FS, Fac 278 – 0.25 acres
• WO 37186 Renovate HAWC for Airmen's Center Fac 2371 – 100 square feet

• Construct Foot Path Extensions QYZH 15-2057 - 2.78 acres
• Repair Gunfighter Street Lights WO 36170 - 5,000 square feet
Contractors performing earthwork construction must meet the requirements of the NPDES Construction General Permit (CGP). As required under the site-specific stormwater permit, the construction contractor is responsible for their activities in minimizing impact of sediment into the storm drains to Outfall #001. 366 FW/A7IE is not directly responsible for activities related to stormwater discharges from active construction sites.
2.4 Facility Changes
Quarterly site evaluations were conducted at (29) facilities this year. Several minor facility and site modifications have occurred since the 2016 Annual Site Compliance Evaluation (new or relocated bowsers, repaved asphalt areas, combination and re-organization of facilities, etc.). Changes may also include the types of industrial activities conducted and institutional controls implemented at a given facility. Significant facility changes and findings are described in Table 2. Individual Facility SWPPHs (Section 3 of the MHAFB SWPPP) and the associated facility drainage maps have been updated to reflect these changes, see Appendix C.
The Exposure Checklist included in the Annual Comprehensive Site Compliance Evaluation Form is used to verify and document “No Exposure” exclusions at facilities deemed as such, see Appendix C. A number of facilities have been evaluated and tracked over a period of time with results consistently verifying their “No Exposure” status. As a result, the following facilities have effectively been removed from the SWPPP tracking list given the justifications provided below:
• Part of Facility 1 - Bldg 201. AMU moved from Hangar 201 in 2011 (now EMS Strike 21).
Deemed “No Exposure” in 2011 and re-evaluations over the following 3-year period verified the exemption.
• Facility 5 - Bldg 211. Changed from 390th AMU to Weapons Load Training in 2011 at which point it was deemed “No Exposure”. Re-evaluations over the following 3-year period verified the exemption.
• Facility 9 - Bldg 2800. Yard used to store recreational and flatbed trailers. Deemed “No Exposure” in 2011 and re-evaluations over the following 3-year period verified the exemption.
• Facility 12 - Bldgs 1225 East & 1229. All activities are performed inside with minimal petroleum, oil, lubricants, and bearing grease use. Deemed “No Exposure” in 2011 and re-evaluations over the following 3-year period verified the exemption. Drive-by evaluation performed this year to confirm.
• Facility 13 - Bldg 1225 West. Bldg 1224 was demolished early 2014 and Bldg 1225 users operate as one group (see Fac 12 above).
• Facility 15 – Bldg 1296. Bldg 1296 by definition is not a Treatment, Storage, Disposal Facility (TSDF) facility. Therefore, it is not required to be part of the stormwater program.
• Facility 16 – Bldg 1356. Operations at Building 1351 moved to Building 1356 in 2016. Also, as a result of the move, sources to impact stormwater have been removed including the oil water separator.
Facility 16 has been deemed “No Exposure”.
SECTION 2 – INSPECTION FINDINGS
AX1228181110BOI 2-3
• Facility 17 – Bldg 1322. DRMO moved from facility in 2013/2014, facility is currently used for material and equipment storage. Former DRMO Yard no longer has potential stormwater impacts and obtained “No Exposure” status in 2014.
• Facility 26 - Bldg 1353. Demolished in 2010.
• Facility 35 – Landfill. Permanently closed and capped in 2009.
• Facility 38 - Bldg 196. Use changed to full administrative function. Deemed “No Exposure” in 2011 and re-evaluations over the following 3-year period verified the exemption.
• Facility 39 – Bldg 920. In 2016, the entire facility was combined into one operation with regards to stormwater due to their shared space and resources. A previously tenant of the building, the 366th Component Maintenance Squadron (366th CMS) Hydroshop (Facility 39 – Bldg 920 North) is no longer located at MHAFB and removed all their resources from the building. As a result, Facility 39 was omitted from the stormwater program.
• Facility 41 - Bldg 920. In 2017 facilities 39 and 41 were combined into one facility (Facility 41) due to the hydroshop leaving (Facility 39). Following the move, oil storage equipment found within Bldg 920 was removed including the 500-gallon used oil tank located outside the facility. No new oil containment machinery or storage has been added in 2017. Therefore, Facility 41 has been deemed “No Exposure”.
These facility identification numbers will be tracked as “omitted” and will not be completely removed from the SWPPP until a new MSGP is issued and a new edition of the MHAFB SWPPP is authored.
2.5 Stormwater Management Issues
Some stormwater management issues were noted during the 2018 quarterly site inspection process.
These issues are identified in Table 2 and have been updated on the EPA Annual Reporting Form, which is included in Appendix E of this report. Issues were reported to 366 FW/A7IE at the time of the facility evaluation.

3.0 GENERAL FACILITY SPECIFIC GUIDANCE
This Stormwater Pollution Prevention Handbook (SWPPH) has been prepared for this facility as part of MHAFB’s compliance with requirements of the federal Clean Water Act. Under the authority of this act, MHAFB is required to obtain coverage under the Multi-Sector General Permit (MSGP) to discharge stormwater associated with industrial activities.
A key requirement of the MSGP is the preparation of a Stormwater Pollution Prevention Plan (SWPPP) which describes potential sources of stormwater pollution at the base and measures, or Best Management Practices (BMPs), to be used to control stormwater pollution. The base 366 FW/A7IE office has prepared a SWPPP intended to fulfill the requirements of the MSGP. This facility specific SWPPH constitutes the portion of the SWPPP developed specifically for implementation at this facility. The entire SWPPP, including BMP plans for the other regulated facilities on base, is maintained at the 366 FW/A7IE office.
This facility was assigned a SWPPH so that responsibilities for day-to-day implementation of the BMP plan would be assigned to personnel at the facility level. However, responsibility for the MHAFB permit compliance will remain with the Installation Management Division, 366 FW/A7IE.
This facility SWPPH contains a description of activities at the facility, including potential stormwater pollution prevention sources and site drainage. It identifies BMPs to control stormwater pollution. BMPs specified in this facility SWPPH are enforceable conditions of the base’s stormwater discharge permit, and are required to be implemented to maintain compliance with the permit.
The Facility stormwater pollution prevention (SWPP) Manager is responsible for implementing this handbook. The Facility SWPP Manager must be identified and appointed in writing by the unit commander.
Questions regarding this SWPPP should be addressed to:
Water Quality Compliance Program Manager – Danicza Lopez Danicza.Lopez@us.af.mil

3.0.1 Best Management Practices (BMPs)
Stormwater BMPs are measures implemented to reduce the potential for pollution associated with stormwater runoff. General BMPs are discussed in this section and facility specific BMPs are identified in the individual sections of this SWPPP.
January 2019 3.0-2 Mountain HoMe aFB
The MSGP requires that the following categories of stormwater BMPs be addressed in the SWPPP:
• Operational BMPs o Good Housekeeping o Quarterly Inspections and Preventative Maintenance o Spill Prevention and Response Procedures o Erosion and Sediment Control o Pollution Prevention Training o Non-Stormwater Discharges
• Activity Specific BMPs o Minimize Exposure o Salt Storage Piles o Sector Specific Effluent Limits o Waste, Garbage, and Floatable Debris o Dust Generation and Vehicle Tracking
• Management of Runoff
Additional Activity Specific BMPs are provided based on the facility usage and sector classification.
Minimize Exposure
Facility users should minimize the exposure of material storage areas (including loading and unloading, storage, disposal, cleaning, maintenance, and fueling operations) to rain, snow, snowmelt, and stormwater runoff by either locating these materials and activities inside or protecting them with storm resistant coverings. Some alternatives may include:
• Use of grades, berms, or curbs to prevent runoff of contaminated flows and divert run-on away from these areas
• Locate materials, equipment, and activities such that leaks are contained of diverted
• Clean up spills and leaks promptly using dry methods (absorbents)
• Use drip pans and absorbents under or around leaky vehicles and equipment or store indoors
• Use spill/overflow protection equipment
• Drain fluids from equipment and vehicles prior to on-site storage or disposal
• Perform all cleaning operations indoors, under cover, or in designated containment areas
• Ensure that all washwater drains to a proper wastewater collection system (NOT the stormdrain system)
January 2019 3.0-3 Mountain HoMe aFB
Good Housekeeping
Good housekeeping BMPs reduce the risk of stormwater pollution by preventing contact of significant materials with stormwater. Users should keep clean all exposed areas that are potential sources of pollutants; efforts should include routine sweeping, keeping materials orderly and labeled, and storing materials in appropriate containers. Facility specific housekeeping BMPs are outlined within each facility’s section of this SWPPP.
The base wide Foreign Object Debris (FOD) management program implemented by MHAFB performs routine inspections and vacuum-sweeping of the runway, taxiways and aprons to prevent aircraft damage. This practice prevents the accumulation of sediments and other pollutants on the paved surfaces of the airfield.
Quarterly Inspections and Preventive Maintenance
Quarterly inspections and preventive maintenance are intended to identify and correct conditions which may lead to exposure of pollutants to stormwater. Vehicles and equipment should be inspected periodically to ensure they are in good operating condition. Any leaking equipment should be repaired immediately, drained, or relocated indoors.
Routine facility inspections are required in accordance with Section 4 of the MSGP. Inspections should cover all areas of the facility where industrial materials or activities are exposed to stormwater, stormwater control measures, BMPs, and of the storm drainage collection system.
Issues or corrective actions identified should be reported to the 366 FW/A7IE office, including control measures needing maintenance or repairs, failed control measures which require replacement, and any incidents of noncompliance observed. See Form 3.0-1 Quarterly Visual Examination (QVE) and Routine Facility Inspection Report and Section 3.0.2 Record Keeping and Internal Reporting Procedures below.
In addition, per Section 8.S.5.1 of the MSGP, monthly inspections are required during the deicing season typically (October to April) for all air transportation facilities (Sector S).
The Facility SWPP Manager is responsible for performing and documenting these inspections on Form 3.0-1 Quarterly Visual Examination (QVE) and Routine Facility Inspection Report or an equivalent version of the EPA Additional MSGP Documentation Template and implementing follow-up measures as required. Any issues should be reported to the 366 FW/A7IE office.
Containment Structures
Facilities which utilize containment area BMPs should ensure that these features are properly maintained and inspected.
