OFFEROR’S QUESTIONS AND GOVERNMENT REPLY
(Questions that may be of general interest to all Offerors and/or the Government and that are not readily answered by the proceeding changes will appear below. These questions may have been paraphrased or altered to represent several questions regarding the same subject and/or simply revised to clarify and simplify the question(s). Questions and answers are issued to the Offerors “for information only ”and do not revise the solicitation, plans or specifications.)
Question #1: Due to the complexity of this project we respectfully request a 3 week extension to the project due date.
Question #2: Proposal Due Date - Box 13A of the SF1442, states the due date as Feb 4th by 12:00 AM. Box 10 states that proposals are due by 2:00 PM. Amendment 01 stated the due date as Feb 3rd by 2:00 PM. Please clarify the due date and time for proposals.
Answer #1-2: USACE grants a 2 week extension to the project due date. Proposals are due February 18 by 1400 hours.
Question #3: Please provide an additional site visit or unofficial site visit be made available to review in more detail the existing conditions with our subcontract team.
Answer #3: As stated in W912DS-15-R-0012 Amendment 0001, there will be only one site visit for all prospective offerors. This site visit occurred on Tuesday, January 19, 2016.
Question #4: Number of Hard Copies - Box 13A of the SF1442 states that one original and one copy is required. However, page 10, para. 2.5, states that an original and six copies are required (the 6 copies is also mentioned on page 16, para. 3.3). Please clarify the required number of hard copies of each volume to accompany the original.
Answer #4: Box 13A of the SF1442 has been revised. Please submit one original and six (6) copies of each volume.
Question #5: Volume 2, Price and Pro Forma Information - Page 16 of 126, para 3.3 states that the price proposal shall be submitted in a separate binder. It goes on to say that the Pro Forma information should be submitted in a separate envelope. Please clarify what goes into the Pro Forma information envelope (is it TAB C - Required Pre-Award Information?). Please also clarify how many copies of the Pro Forma information is required (para 3.3.4.2 indicates one copy, but we will have two Price Volumes with Tab C)
Answer #5: Submit one original and six copies of each volume (Technical Proposal and Price Proposal). Include all information listed under “Volume 2: Price Proposal and Pro Forma Information” (Tabs A through E) in one volume (Volume 2). Disregard the instruction to include the separate Pro Forma Information envelope.
Question #6: Factor 2, Contractor Past Performance - On page 11, under Factor 2 Past Performance, the instructions describe submitting past performance information on projects using the PPQ form included in the RFP. May we include in our proposal submission, previously completed PPQs and CCASS/CPARS evaluations for our projects submitted under Factor 1, in lieu of requesting new PPQs from clients?
Answer #6: Yes
Question #7: KEY PERSONNEL RESUME FORM- Please confirm if it is acceptable for offerors to submit resumes in their own format as long as they contain the same information required on the form provided with the RFP.
Answer #7: Offerors may submit resumes in their own format as long as they contain all information listed on the form and on page 13, paragraph 3.1.3 Factor 3 ii, and they do not exceed three (3) pages.
Question #8: Drawing CS110 Sheet Note 10 states to provide a concrete pad per detail 3 on sheet CS500. This detail only provides sectional dimension and not overall pad size. Please provide size of pad.
Answer #8: Sheet CS500, detail 3, states: "Exact utility pad dimensions and locations of openings and sleeves through pad to be coordinated with equipment suppliers. Pad to be a minimum of 1'-0" larger than equipment on all sides." For approximations, takeoff can be done from drawing CS110.
Question #9: Drawing SD110 Demolition Note 3 requires a survey of the existing piers and replacement if they are out of alignment. As the quantity of replacements cannot be made until a survey is performed, please confirm that the scope is to only perform the survey. After the survey, if any are misaligned or in bad condition the replacement will be treated as a change order.
Question #10: Drawing S-111 Construction Sheet Note 2, states “to reuse existing if possible”.
For bidding purpose please confirm that we are to assume all foundations can be reused and that after field survey if any are misaligned or in bad condition the replacement will be treated as a change order.
Question #11: Drawing SD112 Demolition Note 1 state to “remove all existing foundations” please confirm this note only applies to the 14 foundations shown on this drawing.
Answer #9-11: For Drawings SD110, S-111, and SD 112, contractor is to assume replacement of 90% of existing.
Question #12: Drawing EX003 Details 3&4 reference drawing EX112 which is missing from the set. Please provide.
Answer #12: Drawing EX112 is located on page 173/200 of the plans downloaded from FBO site.
Question #13: Reflected Ceiling Plan on sheet A-141 shows the GWB ceilings in rooms 107 and 108 as 10’-0” height. Finish Schedule on sheet A-610 shows the GWB ceilings in rooms 107 and 108 as 9’-0” height. Please clarify.
Answer #13: The GWB ceilings in rooms 107 and 108 should be 10’-0” in height.
Question #14: Restroom Elevations 2 and 4 on sheet A-400 appear to graphically show the wall tile all the way to the ceiling. The partition type details on sheet A-540 only show wall tile to a height of 5’-0” AFF, with the exception of the shower stalls which shows tile to the ceiling.
Please clarify the height of the wall tile in the restrooms.
Answer #14: Sheet A-400, Detail 1, Enlarged Restroom Plan, indicates which wall type to be used in each location in the restroom. These wall types are detailed on sheet A-540. Follow the wall type details given on sheet A-540 for all wall tile restroom elevations.
Question #15: There is a conflict on the bid bond requirement between Block 13 B of the Standard Form 1442 and page 53 of the solicitation. Please confirm a bid bond is not required.
Answer #15: Block 13B on the 1442 has been revised, Bid bonds is required.
Question #16: RFP solicitation, page 70, paragraph 1.22 Labor surplus area expenditure requirements, requires a percentage of labor to come from the labor surplus area. Please provide the Labor Surplus Areas listing.
Answer #16: The solicitation, page 70, paragraph 1.22 states: The Contracting Officer will furnish upon request a list of labor surplus areas. A copy is attached and available at the Department of Labor site https://www.doleta.gov/programs/lsa.cfm. A description of Labor Surplus Areas is available at https://www.doleta.gov/programs/laborsurplus02.cfm. Morris County, Jefferson Township, and Rockaway Township are not listed as Labor Surplus Areas for 2016, so this paragraph does not apply.
Question #17: There is conflicting information in the solicitation with regards to a bid bond. The SF1442 states a bond is not required; Page 16 of 126 of the solicitation states to place bid bond under Vol 2 Tab B; Page 17 of 126 states to provide evidence of bondability; and Page 53 of 126 states that a bond is required for 20% NTE $3.0m. Please advise which of these is correct.
Answer #17: Block 13B on the 1442 has been revised, Bid bonds is required.
Question #18: What is the deadline for all RFI's?
Answer #18 5 days before proposal due date.
