Factor 1, Experience

Adjectival Rating Description

Unacceptable The proposal fails to demonstrate experience on at least two Relevant Contracts (per

Paragraph 4.1.2).

5.0 Volume 1, Tab 3: Past Performance

5.1 Factor 2 – Past Performance

5.1.1 Submission Requirements

a. For the contracts listed under Factor 1 – Experience, the Government will query the Offeror’s Contract Performance Assessment Reporting System (CPARS) to validate past performance ratings on Department of Defense contracts and any other past performance information the Government deems necessary to evaluate an Offeror’s past performance.

b. For each contract that does not have a CPARS evaluation, the Offeror shall send a copy of the Past Performance Questionnaire (PPQ) which is included in Attachment B to the points of contact responsible for monitoring performance under such contracts. The Past Performance Questionnaire included in the solicitation is provided for the Offeror or its team members to submit to the client for each contract the Offeror includes in its proposal for Factor 1, Experience. Ensure the name, phone numbers, and e-mail addresses are provided for the client’s point of contact. Completed Past Performance Questionnaires should be submitted with the Offeror’s proposal. Offerors may submit PPQs from previous responses to USACE solicitations. If the Offeror is unable to obtain a completed PPQ from a client for a contract(s) before the proposal closing date, the Offeror shall complete and submit with the proposal the first page of the PPQ (Attachment B) which will provide contact and client information for the respective contract(s). Offerors should follow-up with clients/references to ensure timely submittal of questionnaires. The Offeror should explain problems encountered and actions taken for any submitted contract. If the client requests, questionnaires may be submitted directly to the Government's point of contact, via email at W9128A22R0020@usace.army.mil prior to proposal closing date. Offerors shall not incorporate by reference into its proposal PPQs previously submitted for other solicitations. However, this does not preclude the Government from utilizing previously submitted PPQ information in the past performance evaluation.

c. The Information contained in the questionnaires will be used to evaluate the Offeror’s past performance.

If any negative past performance information is received, the Offeror will be given an opportunity to provide a rebuttal if it has not already provided such a rebuttal. The Offeror is responsible for ensuring it submits a copy of past performance assessments (CPARS or PPQs) or notifies the Government and ensures that an assessment is already in CPARS. An Offeror that has no past performance ratings shall indicate such in its proposal, i.e., that it has no previous clients and that the questionnaires cannot be provided. It is the Offeror’s responsibility to ensure the referenced documentation is provided, as the Government shall not be obligated to make an additional request for past performance from the references. The evaluation form shall be provided to the Government's point of contact, via email at W9128A22R0020@usace.army.mil. Offerors are not to request past performance questionnaires on contracts that have final CPARS evaluation. If a past performance questionnaire is received on a contract where a final CPARS evaluation exists, the CPARS evaluation will serve as the official rating of record for evaluation purposes.

d. The Government may (but shall not be obligated to) contact references provided as part of Factor 1 –

Experience, for information regarding the Offeror’s past performance on the contract, and for the purposes of assessing and verifying the scope of work performed. Offerors should provide accurate, mailto:W9128A22R0020@usace.army.mil mailto:%20W9128A22R0020@usace.army.mil.

The assembly shall have a bronze, ductile iron, cast-iron or stainless steel body with flanged ends. The assembly shall include pressure gauge test ports and OS&Y shutoff valves on the inlet and outlet, 2-positive-seating check valves for continuous pressure application, and four test cocks. Assemblies shall be rated for working pressure of 175 psi. A test port for a pressure gauge shall be provided both upstream and downstream of the backflow prevention assembly valves.

2.5.1 Backflow Preventer Test Connection

Test connection must consist of a series of listed hose valves with 2 1/2-inch National Standard male hose threads with cap and chain.

2.6 FIRE DEPARTMENT CONNECTION

Fire department connection must be projecting type with cast-brass body, matching wall escutcheon lettered "Auto Spkr" with a hard coated aluminum finish. The connection must have individual self-closing clappers, caps with drip drains and chains. Female inlets must have 2 1/2-inch diameter American National Fire Hose Connection Screw Threads (NH) per NFPA 1963 .

Comply with UL 405 .

2.7 SPRINKLERS

Sprinklers must comply with UL 199 and NFPA 13 . Sprinklers with internal O-rings are not acceptable. Section 00 22 16 – Supplementary Instructions to Proposers, 7.0 Volume 2: Price Proposal,

7.1.10 Volume 2, TAB 7: Subcontracting Plan (only required from Large Business Concerns (page 30 of 128), states that a subcontracting plan must be submitted in accordance with FAR 52.219-9 no later than five (5) calendar days after a notification request from the Contracting Officer or Contract Specialist. The table included on page 29 of 128 of the RPF also lists the contents of the price proposal (volume 2) and make it seem like the subcontracting plan would be due at the same time as the price proposal (for large business concerns). Please verify if/when a subcontracting plan is due. Please also provide a template of the subcontracting plan because although the RFP references to the template being Appendix A, a template was not included.

See Section 00 22 16, para 7.1.10 for when the subcontracting plan will be due.

Section 00 22 16, para 4.1.2. The proposal states that "evidence provided by the owner/contracting agency or CPARS documentation must be included in the proposal to demonstrate the date the contract was substantially complete." In the case of contracts for which a CPARS is not available, what type of documentation is approved to fulfill this requirement? Please advise

Section 00 22 16, para 7.1.8. Per the RFP, "All Offerors must have electronically completed the annual Representations and Certifications […] OR respond with the completed Representations / Certifications found in the solicitation. […] Additionally, the Offeror must also complete and return the 'Representations, Certifications, and Other Statements of Offerors' included in the solicitation." Are Offerors who have already completed and updated their Representations and Certifications on SAM.gov required to also complete the Representations and Certifications form in the solicitation? The first statement seems to indicate that either is acceptable, while the latter seems to mandate both. Please clarify.

See Am-0002, Section 00 22 16, para 7.1.9.

Section 00 22 16, Section 5.1.1.b states “Offerors shall not incorporate by reference into its proposal PPQs previously submitted for other solicitations. However, this does not preclude the Government from utilizing previously submitted PPQ information in the past performance evaluation.” Please reconsider this language since there are two CDCs on the bidding process, it will benefit everyone to be able to use the PPQs for both projects.
