PERFORMANCE REQUIREMENTS SUMMARY (PRS)
1 General: This Performance Requirement Summary (PRS) is applicable to all contractor provided services. Services shall be inspected under the FAR 46.407 – Nonconforming Supplies or Services. The Contractors primary consideration when considering quality control as it relates to refuel and defuel in support of flight training is to ensure that aircraft are available for training or maintenance with the required fuel on board, and aircraft that are unable to meet the standards are identified early and accurately for administration by the Government.
1.1 Required Services: The contract service requirements subject to this PRS listed in Table 1, Column 1.
1.2 Performance Measure and Standard: Contract service performance requirements are listed in Table 1, (Column 2 and 3.
1.4 Minimum Acceptable Standard (MAS): Table 1, Column 4 represents the MAS rate expressed as a percentage of aircraft that are rejected for nonconformance. A defect is defined as one nonconforming aircraft. When computing MAS all fractions will be rounded up to the next whole number.
1.4.1 Major Nonconformance:
a. The services listed in the PRS will be considered a major nonconformance when the number of aircraft at any Basefield or Stagefield rejected for noncompliance with the inspection criteria established for cold, warm or hot refueling or defueling exceeds the established MAS.
b. The services listed in the PRS will be considered a major nonconformance when either safety or environmental requirements are not being adhered to.
1.4.2 Critical Nonconformance:
a. The services listed in the PRS will be considered a critical nonconformance when the number of aircraft at any Basefield or Stagefield rejected for noncompliance 30 minutes following identification of a Major Nonconformance (re-evaluation) with the inspection criteria established for cold, warm or hot refueling or defueling exceeds the established MAS.
b. The services listing in the PRS will be considered a critical nonconformance when either safety or environmental requirements have been identified previously as a major nonconformance and during re-inspection the same issues still exist.
c. The services listed in the PRS will be considered a critical nonconformance when safety or environmental requirements have been willfully ignored and place aircraft, personnel or the environment at risk.
1.5 Method of Surveillance: Table 1, Column 5 Outlines the evaluation tools and techniques utilized to assess the Contractor performance of the Work Requirements to establish and measure standards.
2 Determining Nonconformance:
2.1 General: Determining Nonconformance can broken down into two separate groups, one for aircraft not refueled or defueled to meet the established criteria, and another for those who fail to meet the established criteria for subsequent reevaluation.
2.2 Major Nonconformance: In the event the number of aircraft not receiving refuel/defuel which are rejected for noncompliance with the inspection criteria established for the Basefields/Stagefields or concerning safety and/or environmental criterion exceeds the established MAS.
2.3 Critical Nonconformance: Critical Nonconformance deductions apply only to major nonconformance areas being reevaluated which were previously identified due to nonconformance, or when safety of personnel, equipment or the environment is willfully put at risk.
2.3.1 Unperformable Task: For the purposes of Nonconformance deductions an Unperformable Task is defined as a refueling/defueling mission that the contractor was never informed of or if there is an equipment malfunction on the equipment being serviced if it is not owned or under control of the contractor. Missions that are considered to be an Unperformable Task will not be considered as part of MAS computations for nonconformance.
2.3.2 For purposes of establishing acceptance and rejection criteria under 100% Lot size = Sample size.
Example 1 (100% Inspection) Given
· A flight training period (see PWS 1.5.1) at a Base-Field has 90 aircraft issued including spares in support of USAACE flight operations.
· The MAS for major nonconformance for this requirement is 1%
· 4 of the 90 issued aircraft including spars do not meet the requirements of 5.3 of the PWS.
Then
· See Attachment 1, Acceptance and Rejection Table (Normal Inspection) under 1% Colum.
· Find the sample size. If the sample size falls between two values use the next greater value. In this case 125.
· 4 nonconforming aircraft meets the rejection criteria and is considered a major nonconformance.
Given
· Of the 4 nonconforming aircraft, 1 aircraft does not meet the requirements of 5.3 PWS.
· The MAS for critical nonconformance is 0%.
Then
· The MAS for re-evaluation is 0%. 0 is the maximum number of defects
· 1 nonconforming aircraft exceeds the MAS and is considered a critical nonconformance.
Example 2 (Planned Sampling Inspection Stagefields Hot Refuel)
· There is planned sampling of 5 Stagefields this week for Hot Refuel the QAEs may go to any Stagefield listed in Table A in the PWS. Upon arrival at the Toth Stagefield. The Stagefield is operational and has been for 30 minutes. The Refuel personnel show up 40 minutes later this is one hour and 10 minutes and is a Major Nonconformance.
· When the Refuel personnel show up it takes them one hour making it 2 hours and 10 minutes to be prepared to pump fuel and there have been aircraft ready for refuel for 30 minutes. This would be a Critical Nonconformance.
· If the Government Quality Assurance Evaluator shows up at High Bluff Stagefield and the refuel personnel are present and ready to hot refuel aircraft within one hour of the tower becoming operational there is no nonconformance.
· If a QAE shows up at Hunt Stagefield and the refuel personnel are there but when aircraft land they are not immediately refueled then this in not in accordance with the PWS para 5.7 and an observation will be taken and passed through the COR to management.
Example 3 (100% Inspection for Fuel Dispensing Equipment Maintenance and Inspections)
· Every major item in use at the Basefields or Stagefields will be inspected by the QAEs at least monthly.
· If during any given Calendar Week Sunday to Saturday the QAEs find 2 or more items not properly maintained or without valid inspections this is Major Nonconformance.
· If the Items found are not corrected or taken out of service within 72 hours it will become a Critical Nonconformance. This does not alleviate any responsibility to provide refuel/defuel at any locations.
Example 4 (100% Inspection for Spill Prevention and Clean-up and Safety)
· This means that every time anything is inspected the QAE may ask to see the Spill Kit and required safety items. This will be part of every inspection but may be reduced if compliance becomes common place.
· If the QAEs identify 3 safety issues but no Spill Prevention and Clean Up issues this would be a Major Nonconformance for safety.
· If these issues are not corrected within 48 hours it becomes a Critical Nonconformance.
