ATTACHMENT 11

COST PROPOSAL FORMAT

1. The purpose of this attachment is to provide the format to show a breakdown of your total costs shown in Section B and Attachment 12.

2. Utilizing an Activity Base Costing approach provide the total cost, inclusive of all direct cost and all indirect cost, by training program: Initial Entry Rotary Wing (IERW) Flight School XXI (FSXXI), Graduate, and Air Force.

3. Provide your costing information to support your proposal on the following worksheets. You should use as many worksheets as you feel necessary to adequately show your costing scheme. For instance, if your costs are not spread evenly throughout the fiscal year, use a separate worksheet for each month in that fiscal year. Do the same for the other worksheets, if necessary, to show your monthly cost variations. Label each worksheet with the time periods. You may provide other cost information and/or format to support your proposal, in addition to the minimum required.

4. For the purposes simplifying the cost of training as it relates to training days utilize a Standard Training Year. A Standard Training Year is defined as a standard 365 day calendar year, minus weekends, Federal Holidays, and Holiday Exodus and consists of 242 training days.

a. Example: 365-105(weekend days)=260, 260-10(Federal Holidays)=250, 250-8(Exodus)=242.

5. Show associated Direct Cost breakdown by flight training program (IERW FSXXI, Graduate, and Air Force) for:

a. Positions that directly support academic and flight training.

b. Payment of course related night differential premium. (See Attachment 5)

c. Payment of additional skill and qualification premiums, if applicable. (See Section C-1)

d. Payment of authorized weekend/holiday flight training and academic training overtime. Compute the undergraduate flight training overtime requirement based on 3400 (600 for Air Force, 2800 for Army) overtime hours per year, undergraduate academic training overtime requirement based on 1380 (80 for Air Force, 1300 for Army) overtime hours per year, and graduate academic training overtime requirement based on 40 (0 for Air Force, 40 for Army) overtime hours per year. (See Section C5.4.1.8.1 and G3.3.2.1) On the Overtime Worksheet of the Pricing Proposal, show your computations for authorized overtime with applicable benefits.

e. Payment of authorized optional overtime. Compute the overtime requirement based on 50 (40 for undergraduate, 5 for Air Force, 5 for graduate) hours per month.. (See Section C5.4.1.8.2 and G3.3.2.2) On the Overtime Worksheet of the Pricing Proposal, show your computations for authorized optional overtime with applicable benefits.

6. Show associated Indirect Cost breakdown by flight training program (IERW FSXXI, Graduate, and Air Force) for:

a. Positions that share duties, responsibilities, and/or oversight between Initial Entry Rotary Wing (IERW) Flight School XXI (FSXXI), Graduate, and Air Force academic and flight training programs. (See Attachment 5 Master Course List for breakdown of flight and academic training). Consider a distribution factor to determine indirect costs for Initial Entry Rotary Wing (IERW) Flight School XXI (FSXXI), Graduate, and Air Force by dividing the employee’s salary by the total number of direct employees to flight training program percentage ratio. (Total number of direct flight and academic employees required to support the flight and academic training program divided by the total number of all employees required to support the flight and academic training programs.)

b. Positions that do not have direct duties, responsibilities, and/or oversight of the Initial Entry Rotary Wing (IERW) Flight School XXI (FSXXI), Graduate, and Air Force academic and flight training, but add value to the contracted service. Consider a distribution factor to determine the value-added indirect costs to Initial Entry Rotary Wing (IERW) Flight School XXI (FSXXI), Graduate, and Air Force flight and academic training by dividing the employee’s salary by the total number of direct employees to flight training program percentage ratio. (Total number of direct flight and academic employees required to support the flight and academic training program divided by the total number of all employees required to support the flight and academic training programs.)

c. Breakdown of your organization’s support costs and, if applicable, corporate support costs.

7. Show method for calculating night differential.

8. Show definitions for and method for determining additional skill and qualification premiums.

9. Show method for calculating authorized overtime.

10. Show Student Increase/Decrease prices.

a. Compute the Student Increase price for the entire course length at a cost per Training Day price rate. (See Attachment 5 and applicable FTG/POI/CMP for contractor training requirements). Example: 100 contractor training days x 2 students x $50.00 (training day rate) = $10,000.00

b. Compute the On Time Student Decrease price for the entire course length at a cost at a cost per training day price rate. (See Attachment 5 applicable FTG/POI/CMP for contractor training requirements). Example: 100 contractor training days x 2 students x $50.00 (training day rate) = $10,000.00

c. Compute the Late Student Decrease price for 85 days at a cost per training day price rate. (See Attachment 5 applicable FTG/POI/CMP for contractor training requirements). Example: 85 days x 2 students x $50.00 (training day rate) = $8,500.00

d. Consider the direct cost distribution between Flight Training Instructor Pilot and/or Training Device Instructor Pilot and Academic Instructor as one half the TD each. (½ TD for Flight Training Instructor Pilot and/or Training Device Instructor Pilot and ½ TD for Academic Instructor to equal the total instructor related direct cost.

e. Consider all of the associated in-direct costs required to support each specific course within the specific flight training program. (IERW FSXXI, Graduate, and Air Force).

f. Show your method for calculating direct and in-direct costs to derive at the Student Increase/Decrease price.

11. Show Student Defect Decrease prices.

a. Compute the per Student Defect Decrease price at the per Training Day (TD) price rate. (242 TD per fiscal year)

b. Consider the direct cost distribution for a Flight Training Instructor Pilot, Training Device Instructor Pilot, or Academic Instructor as applicable.

c. Show your method for calculating costs to derive at the Student Defect Decrease price.

12. Show your method for calculating Contingency Work price cost.

a. Show your method for calculating direct and in-direct costs to derive the Student Increase/Decrease price.

b. Show risk factors considered and method for determining applicable risk factors.

13. Show your method for calculating direct and in-direct costs associated with providing new work for this contract.

a. Show your method for calculating direct and in-direct costs.

b. Show risk factors considered in providing New Work and method for determining applicable risk factor costs.
