ATTACHMENT 1 – PERFORMANCE REQUIREMENTS SUMMARY (PRS)

1 General: This Performance Requirement Summary (PRS) is applicable to all contractor provided services. Services shall be inspected under the FAR 46.407 – Nonconforming Supplies or Services. The Contractors primary consideration when considering quality control as it relates to flight and academic training is to ensure that students whom are unable to meet the course standards are identified early and accurately for administration by the Government.

1.1 Required Services: The contract service requirements subject to this PRS listed in Table 1, Column 1.

1.2 Performance Measure and Standard: Contract service performance requirements are listed in Table 1, Column 2 and 3.

1.4 Minimum Acceptable Standard (MAS): Table 1, Column 4 represents the MAS rate expressed as a percentage of students in a class which were rejected for nonconformance. A defect is defined as one nonconforming student. When computing MAS all fractions will be rounded up to the next whole number.

1.4.1 Major Nonconformance: The services listed in the PRS will be considered a major nonconformance when the number of students in a class rejected for noncompliance with the inspection criteria established for the first time End of Stage (EOS) Flight Evaluation or academic first time Criterion Test (CT) exceeds the established MAS.

1.4.2 Critical Nonconformance: The services listed in the PRS will be considered a critical nonconformance when the number of students in a class rejected for noncompliance during any subsequent reevaluation of an EOS Flight Evaluation or Criterion Test exceeds the established MAS.

1.5 Method of Surveillance: Table 1, Column 5 Outlines the evaluation tools and techniques utilized to assess the Contractor performance of the Work Requirements to establish and measure standards.

2 Determining Nonconformance:

2.1 General: Determining Nonconformance can broken down into two separate groups, one for students who fail to meet the established criteria for first time EOS flight evaluation or academic CT, and another for those who fail to meet the established criteria for subsequent reevaluation.

2.2 Major Nonconformance: In the event the number of students in a given class which are rejected for noncompliance with the inspection criteria established for the first time EOS Flight Evaluation or academic first time Criterion Test (CT) exceeds the established MAS.

2.3 Critical Nonconformance: Critical Nonconformance deductions apply only to students being reevaluated on an EOS Flight Evaluation or CT which they have been previously rejected due to nonconformance. In the event the number of students in a given class being reevaluated which are rejected for nonconformance with the inspection criteria established for the first time EOS Flight Evaluation or academic first time CT exceeds the established MAS.

2.3.1 For the purposes of computing nonconformance for flight training under the TH-67 POI the government considers there to be three “stages of training”, Primary, Instruments and BWS. For the purposes of computing deducts from contract price for flight training under the UH-72 POI the government considers there to be four “stages of training”, Basic Rotary Wing Skills, Instruments, Advanced RW/Instrument Skills, and Warfighter Skills. For computing deducts for academic training the government considers the “stage of training” to be defined as the academic days associated with each CT.

2.3.2 Unperformable Task: For the purposes of Nonconformance deductions an Unperformable Task is defined as a student who despite being properly administered by the Contractor does not possess the ability or aptitude to perform to EOS Flight Evaluation or CT standards. Students whom are considered to be an Unperformable Task will not be considered as part of MAS computations for nonconformance.

2.3.2.1 Proper Student Administration: Proper student administration is defined as a student whom is administered by the Contractor IAW all applicable regulations with specific emphasis on Fort Rucker Regulation 350-16, the applicable FTG / POI, and the Instructor Pilots Handbook. Each student record in question will be subjectively evaluated by the government to establish if the student has been properly administered.

Example 1 (Planned Sampling) Given

· A class which contains 50 students under normal surveillance has completed training and are ready for EOS evaluations.

· Based on the sample size chart 8 students will be selected to complete EOS evaluations for quality assurance purposes.

· The MAS for Major Nonconformance during this stage of training is 10% and based on the acceptance and rejection table 2 is the maximum acceptable number of defects.

Then

· After completion of first time EOS evaluations 3 students are found to be nonconforming after failing the EOS flight evaluation and none are found to be an unperformable task.

· The class has exceeded the MAS for Major Nonconformance.

Given

· Of the 3 students who received additional training 1 fails their reevaluation and is not found to be an unperformable task.

· The MAS for Critical Nonconformance during this stage of training is 0%. 0 is the maximum number of defects

Then

· The class has exceeded the MAS for critical nonconformance.

Example 2 (100% Inspection) Given

· A class which contains 50 students has 7 students which are found to be nonconforming after failing a first time Criterion Test (CT).

· The MAS for major nonconformance during this stage of training is 10%.

Then

· The number of students assigned to the class (50) is multiplied by the MAS (10%): 50 x10% = 5.0.

· All fractions are rounded up to the next whole number 5.0 = 5. 5 Becomes the maximum number of defects.

· This class has exceeded the MAS for major nonconformance.

Given

· Of the 7 students who received additional training 2 fail their reevaluations.

· Based on a review of the students training records by the Government one student was found to be an unperformable task.

· The MAS for critical nonconformance during this stage of training is 0%.

Then

· The MAS for reevaluation 0%. 0 is the maximum number of defects.

· This class has exceeded the MAS for critical nonconformance.
