STATEMENT OF WORK

Repair 2nd Division Drive, Between Owen and Combat Vehicle Trail, JBLM
Date: 6/4/2018
1. DESCRIPTION OF WORK: Provide all plant, labor, tools, equipment, and materials (except as indicated otherwise in the specifications) and perform all work in strict accordance with the specifications and plans required for this single award task order contract (SATOC). This task order provides full depth repairs to the failing asphalt cement concrete (ACC) pavement on a segment of 2nd Division Drive, between Owen and the combat vehicle trail shown on the plans. The scope of the project also includes incidental utility works associated with the repairs.
The contractor shall accomplish all items of work needed for this task order in accordance with the applicable drawings, specifications, AWWA, NEC, NFPA, Washington Administrative Code (WAC), ADA, and provisions of the SATOC contract in order to provide the Government a complete and useable product.
1.1. AREA DESCRIPTION: Subject segment of 2nd Division Drive is located on JBLM, Lewis Main – at the southern perimeter of the Gray Army Airfield
(GAAF). The Segment of roadway between Owen and the combat vehicle trail is dilapidated with a critically low overall pavement condition index (PCI) of 27 (Recorded in 2015).
2. PRINCIPLE ITEMS TO BE ACCOMPLISHED
2.1. (Base Item: CLIN 0001AA)
Demolition
2.1.1. Demo existing asphalt sidewalks, Portland cement concrete (PCC) apron, segment of airfield fencing, obsolete power poles, and other existing structures as shown on attached demolition sheets of the drawings package.
Note: Demolition plans and physical security plans (temporary fencing, etc.)
shall be furnished by the contractor and approved by the COR/GAAF security personnel prior to demolition of perimeter fencing.
2.1.2. Pulverize existing pavement. Pulverized pavement meeting gradation requirements may be used as subbase material. Haul excess pulverized material to JBLM Earthworks or other authorized locations.
2.1.3. Demo existing curb and gutter at the intersection of 2nd Division. and
Owen, as shown on attached plans. Concrete shall be broken into acceptable sized pieces and delivered to JBLM earthworks or other authorized location.

2.1.4. Clear and grub and excavate /fill areas as required to install new pavement, PCC apron, asphalt cement sidewalks, street lighting, curbs and gutters, ADA ramps, topsoil and hydroseeding.

Repair Works
2.1.5. Grade roadway based on attached conceptual grading plan shown on the details sheet. The recommended minimum grade for the roadway is 2% from the crown sloped towards the full natural dispersion areas.
2.1.6. Provide 6-inch extruded slotted concrete curbing along the sides of the roadway as shown on details sheet for stations 17+00 to 18+00. All shall be vertical curb unless otherwise stated. Provide quarry spalls behind openings of slotted curbing for bank stability and erosion control.
2.1.7. Provide natural dispersion (8-INCH topsoil and hydroseeding) as shown on attached plans. Topsoil/compost is available at the JBLM Earthworks but contractor shall be responsible for loading and transportation, if required.
Note: [Landscaping materials used shall meet all applicable recycled and/or biobased content requirements under Section 6002 of the Resource Conservation and Recovery Act and Section 9002 of the Farm Security and Rural Investment
Act unless qualifying for a legally valid exception under those laws. The contractor shall submit documentation demonstrating compliance to the COR or government’s project manager or justification for legal exception using HJB
Form 225 (submitted per instructions on the form).

2.1.8. Provide restorative chain link fencing for segment of fencing that was demoed to install the full natural dispersion. Note that new fencing shall include critter guard IAW with fencing specifications for the airfield.
2.1.9. Provide asphalt cement concrete (ACC) sidewalks, driveway entrances and PCC ADA compliant ramps where indicated. New sections of sidewalks shall match existing and provide smooth transition with 6-inches of CSTC and 4-inches of HMA and widths of 6 feet.
2.1.10 Provide new PCC 12-INCH PCC apron at the intersection of the combat vehicle trail and 2nd Division Drive, as shown on attached plans. Replace/Re-install all signage disturbed or damaged during construction.
Note : [Concrete is a designated item under Section 6002 of the Resource
Conservation and Recovery Act and form release agents are designated under section 9002 of the Farm Security and Rural Investment Act. Supplied materials shall meet the recycled and biobased content requirements therein or qualify for a legally valid exception under RCRA 6002/FSRIA 9002 (note that ground granulated blast furnace slag additive to concrete is likely more conformant with EPA guidance cited at Part 23.7 of the Federal Acquisition
Regulation than are coal derivatives such as fly ash, cenospheres, or silica fume). The contractor shall submit documentation demonstrating compliance to the COR or government’s project manager or justification for legal exception using HJB Form 225 (submitted per instructions on the form).

2.1.11. Compact subgrade and provide new 6” of crushed surfacing top course material as shown on attached details sheet with a minimum of 2% slope.
Pavement thickness shall be a minimum of 6” and shall be placed in 2 lifts.
Tack coats shall be provided between lifts and at all vertical paved surfaces.
Note: Tackifiers, if used, shall be submitted for hazardous material authorization and reporting per paragraph 7.4.below.
2.1.12. Adjust utilities (Hydrants, survey monuments, manholes, duct banks, etc.) as necessary to complete required works.
2.1.13. Provide new street lighting and all necessary conduit, handholes and wiring, as shown on attached plans. Street lighting poles, mast arms and lights shall be aluminum satin finish with LED 400W. In addition, provided street lighting poles shall incorporate warning beacons as they are in the glide path of the airfield.
2.1.14. Provide restoration. All areas of pavement demo shall be restored with 8-inches of topsoil and hydoseeded according to contract specification unless otherwise indicated. Contractor shall ensure that seed takes either by providing temporary irrigation or planting during appropriate times.
2.1.15. Provide fog lines, centerline striping and raised pavement markers.
Ensure paint meets fed standard TT-P-1952F.
Note: Paint shall also be submitted for hazardous material authorization and reporting per paragraph 7.4 below.
2.1.16. Perform (2 ea.)small-scale pilot Infiltration Test (PIT) and (2 ea.)
soil grain size analysis with following the stormwater management manual for western Washington, Volume III, Hydrologic Analysis and Flow Control BMPs, December 2014
2.1.17. Provide as-builts plans in both PDF and CAD to the contracting officer’s representative (COR) showing the final layout of the installed parking lot and all features at the end of the project.
2.1.18. Other work necessary to accomplish the above items: The contractor shall be responsible for providing a complete and useable product/system and shall be responsible for accomplishing all tasks incidental to the completion of the task order requirements. Requirements for a complete and useable product which are not explicitly called out in the SOW but reasonably discernable via any method other than destructive testing shall be considered a requirement of the Task Order.
2.2. Standards for finish and installation
2.2.1. All means, methods, and proposal submission requirements shall conform to specifications set forth in the SATOC contract and this
Statement of Work. All construction and contractor-provided products shall conform to the current JBLM architectural, structural, electrical, mechanical, life safety, and environmental standards.

The contractor’s proposal shall include ELINs and bid schedule, work and traffic control plan, cutsheets and competitive quotes for Non
Pre-Priced items over $2,500.
3. CONTRACT CONSTRUCTION PERIOD: The Contractor is required to commence work under this task Order within 15 calendar days after the date of award, work diligently, and complete all work no later than 180 calendar days after the date of issuance of the task order award and notice to proceed
(NTP). The time stated for completion includes final cleanup of the premises. This legal completion date associated with this task order is the date when all construction work must be completed and a final inspection conducted and accepted; this is the date when liquidated damages, if any, will be assessed. Until all required close-out documentation has been submitted and accepted by the Government, 5% of the task order price will be withheld as retainage.
3.1. Construction shall be phased such that access through 2nd Division
Drive is maintained at all times. Contractor shall provide a proposed construction phasing plan for Government’s approval prior to starting any constructing activities on site.

Dimensions and utility locations are approximate and must be verified in the field. All supplied As-Builts, shop drawings, and/or sketches are for conceptual use and the Government shall not be liable for any discrepancies.

5.2 Specifications
5.2.1 The contractor shall comply with all relevant specification sections as set forth in the base contract.
6 SUBMITTALS
6.2 Project Submittal Requirements All submittals shall be submitted in accordance with the specification section titled SUBMITTAL PROCEDURES.
Required submittals are identified in their applicable specification sections.
6.2.1 In addition to the requirements as outlined in the specification section titled SUBMITTAL PROCEDURES, an electronic copy of each submittal shall be provided to the Government. Delivery method shall be decided during the CQC coordination meeting.
6.2.2 All sketches, modifications, adaptations, quality assurance requirements, and contractually provided products should be submitted to the Government for review and consideration. Any submittals determined to be necessary during the task order will become a requirement of the contract. Accurate submission of submittals remains the responsibility of the contractor and verification remains the requirement of the Government.
6.3 Close-out Submittal Requirements
6.3.1 The Contractor shall provide close-out submittals as required by the contract specifications.
6.3.2 The Contractor shall provide a Letter of non-retention for all contract documents, plans, drawings, and specifications after the destruction of all copies that are not required by the contractor to meet legal requirements.
7. CONSTRUCTION REQUIREMENTS: The contractor shall notify the Government at least 7 calendar days prior to starting the on-site phase of the project.
All work shall be scheduled and coordinated with the Government’s project management team or designee. The following phasing related issues shall affect the work.
7.1. Interface with Others: Government use of these facilities is anticipated while the work under this contract is being performed. The work shall be planned and accomplished so that there will be minimum interference and inconvenience to occupants in the building, daily operations of the facility, and the vicinity.

7.1.1. Any blockage of building exits, roads, or driveways must be coordinated in advance with the Building POC.
7.1.2. Building/Project POC – Project Manager.
7.2. Work Hours: Normal work hours are from 0730 to 1630, Monday through Friday, except Federal holidays. Contractor shall not be permitted to work other than normal work hours unless authorized by the contracting officer or contracting officer’s representative. The contractor may request base access through the Contracting Office.
7.3. Liquidated Damages: Liquidated damages shall be in accordance with the base contract.
7.4. Environmental Project Restrictions: The Contractor shall comply with all Joint Base Lewis-McChord (JBLM) environmental requirements, including the most current installation regulation 200-1 (Environmental
Protection and Enhancement) and SATOC specification section 01 57 20.00
10 (Environmental Protection). This includes the submission of an
Environmental Protection Plan prior to commencement, as detailed in section 01 57 20.00 10. The JBLM environmental division shall review the plan and provide any necessary corrections or additions to the
Contracting Officer or the COR. In addition, the following shall be adhered to:
7.4.1. Environmental Management System (EMS): The JBLM EMS conforms to the criteria defined in the international standard, ISO 14001:2004
Environmental management systems – Requirements with guidance for use.
In accordance with EMS procedures, the contractor shall be familiar with the JBLM environmental policy and shall ensure that this information is considered and incorporated into this project. 7.4.2. Sustainable Acquisition. In order to comply with federally mandated sustainable acquisition requirements and Department of
Defense (DOD) and Department of the Army (DA) green procurement program (GPP) policies, Joint Base Lewis-McChord (JBLM) requires the purchase, supply, and use of environmentally preferable products and services. Program elements include: recovered material products, energy and water efficient products, alternative fuels and fuel efficiency, bio-based products, non-ozone depleting substances, and environmentally preferable products. The Contractor shall comply with all applicable Federal, DOD, Army, and JBLM sustainable acquisition laws and policies. If compliant products are unavailable or determined to be impracticable based on cost or performance, the Contractor shall contact the Contracting
Officer or the COR for further guidance. Additional information is available from the JBLM sustainable acquisition program at (253) 966-

7.4.3. Waste-Water: Any waste-water generated during this project is subject to JBLM EPA MS4 storm water permit. To comply with the permit, all contaminated wastewater produced by this project must be collected, processed, characterized, and disposed of into the JBLM sanitary sewer system or another authorized disposal method. JBLM sanitary sewer discharge requires a JBLM sanitary sewer discharge permit which is used by DPW waste water treatment plant. Prior to discharge a laboratory analysis of the waste water may be required for review and approval by wastewater plant personnel. Processed water is not authorized to discharge to the ground or JBLM storm sewer. The environmental protection plan for the project should identify any potential wastewater generation and the process to which that water will be properly disposed.
7.4.4. Hazardous Materials: For any hazardous materials, the Contractor shall submit an initial list of hazardous materials to be used
(including unit of measure and Safety Data Sheet for each product).
A copy of these documents must be submitted to the Joint Base Lewis-
McChord (JBLM) pollution prevention program
(usarmy.jblm.imcom.list.dpw-hm-inventory@mail.mil prior to commencement. These items must be reviewed by pollution prevention and authorized for use if they are not already on the JBLM installation Authorized Use List. The Contractor shall not use any hazardous materials not authorized by pollution prevention.
For any in-use hazardous materials, the contractor shall submit a hazardous material inventory on HJB Form 953 noting the hazardous materials used, the units of measure, quantities used, storage locations, and other information indicated on the form. This inventory shall be submitted at the end of the project. [If the project goes into a new calendar year, the Contractor shall also submit a copy of this inventory (accounting for all hazardous materials) by 15 January.] For any hazardous materials stored on
JBLM, this inventory shall be submitted by the 15th day following the calendar year quarter in which the hazardous materials were used or stored. All HJB 953 inventories shall be submitted electronically and addressed to the contracting Officer or the COR with usarmy.jblm.imcom.list.dpw-hm-inventory@mail.mil included in the carbon copy (“cc”) line of the email. Questions regarding the form may be directed to the JBLM Pollution Prevention Program (966-6469).
7.5 JBLM Log Decking Requirements
7.5.1 If timber is to be harvested, coordination with DPW forestry is necessary in advance of cutting.

7.5.2 As per AR-200-1, 4-3, “Assure that agricultural and forest products are not given away, abandoned, carelessly destroyed, used to offset contract costs or traded for services, supplies, or products or otherwise improperly removed.”
7.5.3 Salable Timber Requirements
7.5.3.1 Merchantable trees shall be piled in a neat, limb-free deck for subsequent disposal by the Government.
7.5.3.2 A merchantable tree is defined as a tree with a small end diameter of at least 4 inches and 16 feet in length. Shorter lengths are still salable as firewood and shall be decked accordingly.
7.5.3.3 Log decks shall be located AS DIRECTED by the Joint Base Lewis-
McChord Public Works Forestry Branch. DECKING MAY REQUIRE LOG TRANSPORT
OFF-SITE TO AN APPROPRIATE LOCATION ON THE INSTALLATION.
7.5.3.4 Trees shall be cut from the stump and limbed (flush to the trunk) out to a 4 inch stem diameter.
7.5.3.5 Trees shall be topped at 4 inches diameter and from this point to the top of the tree shall be considered unmerchantable (slash).
7.5.3.6 Trees shall not be cut into log lengths; if trees are too large to be handled tree length, cut 41- foot logs from the butt end until the top log is less than 16 feet with a small end diameter of 4 inches.
7.5.3.7 Tree length logs shall be piled separately from all shorter material (cut and broken logs).
7.5.3.8 Logs shall not be cut in lengths shorter than 41 feet simply to facilitate haul by dump truck or flatbed unless authorized by the
Government.
7.5.3.9 Log decks shall be located so as not to interfere with construction work and so they will be easily accessible for disposal action by the Government.
7.5.3.10 Log decks shall be stable and should not exceed 8 feet in height
7.5.3.11 Log decks should be separated by species to the greatest extent possible.
7.5.3.12 Black cottonwood is considered unmerchantable and shall NOT be placed in log decks. Treat as slash.
7.5.3.13 Logs with rot showing in both ends are considered unmerchantable and shall be disposed of as slash.
7.5.3.14 Unauthorized removal of logs from JBLM is considered timber theft and subject to prosecution.]
7.5.4 [Oregon White Oak replacement
7.5.4.1 Development, construction or timber harvest operations on Joint
Base Lewis-McChord that results in the loss of Oregon white oak trees will be subject to Fish and Wildlife Program mitigation measures.
7.5.4.2 Oregon white oak mitigation will require the project proponent to replant six Oregon white oak seedlings to every one tree (> 4” diameter at breast height (dbh)) that is removed through the direction and coordination with Fish & Wildlife Program staff.]
7.6 Archaeological Requirements
7.6.1 Joint Base Lewis-McChord contains many unmarked archaeological sites that may be eligible for listing in the National Register of Historic
Places.
7.6.2 In the event that human remains, artifacts, or features of archaeological interest are inadvertently discovered, the contractor shall do the following:

7.6.2.1 Immediately cease activity in the vicinity of the discovery.
7.6.2.2 Stabilize and protect such discoveries from further disturbance or public disclosure.
7.6.2.3 Provide immediate notice (no longer than 24 hours following discovery) by telephone and email to the Installation Cultural Resource
Manager and Contracting Officer's Representative.
7.6.2.4 Work may not proceed in the vicinity of the discovery until authorized to proceed by the Installation Cultural Resource Manager and the Contracting Officer's Representative.
7.7 Construction and Demolition (C&D) Waste Planning and Reporting
The contract is required to achieve a minimum of 60% waste diversion.

7.8 Excess soils and clearing and grubbing debris
7.8.1 JBLM may, as capacity exists, accept excess or unsuitable soil from JBLM construction and demolition projects for placement in designated borrow source areas on JBLM. Borrow source area use is controlled and authorized by JBLM Directorate of Public Works Environmental Division
(DPW/ED). No land clearing debris or sod is accepted. If soil contamination (based on the project description, location or prior use of the project site) is suspected, soil analysis may be required as determined by DPW staff. A Borrow Source Area Use Authorization Form
HJB 224 (http://www.lewis-mcchord.army.mil/designstandards/pdf/HJB_224.pdf) must be completed and approved by DPW/ED staff prior to removal or delivery of soil to any borrow source area. All borrow source users must abide by all operational requirements and restrictions for earth-moving, stockpiling and removing of aggregate/soil and other operations in JBLM borrow source areas.
7.8.2 Logs, stumps, roots, brush, limbs and other organic based debris from the clearing and grubbing of land, except for salable timber, shall be diverted from landfill disposal to the maximum extent possible.
Landfill disposal shall be the last option. Diversion and disposal shall be at the contractor's expense. On a case by case pre-coordinated basis logs, stumps, roots, brush, limbs and other organic debris may be shredded and left on site, taken to the JBLM Composting Facility or an approved off-site recycling facility at the contractor's expense. 8. ASBESTOS AND LEAD PAINT:

9. Anti Terrorism (AT) and Operational Security (OPSEC)
9.1. AT Level I Training: All contractor employees and their
subcontractor employees required to work on JBLM shall complete AT
Level 1 awareness training prior to commencing work on JBLM. The contractor shall submit a signed company letter stating the annual AT
Level 1 training has been or will be completed by all employees and subcontractor employees prior to commencing any work on JBLM. Training certificates shall be maintained and be available for inspection.

9.2. Access and General Protection/Security Policy and Procedures: All contractor and all associated sub-contractors employees shall comply with applicable installation, facility and area commander installation/facility access and local security policies and procedures
(provided by government representative). The contractor shall comply with Joint Base Lewis-McChord security regulations: JBLM Regulation
210-1, JBLM Post Regulations, Chapter 7, section XVII; JBLM Regulation
190-13, Access to and Conduct While upon Joint Base Lewis-McChord; and
JBLM Regulation 190-5, Installation Traffic Code. Contractor employees shall display either contractor-provided identification badges or the identification badges issued under the RAPIDGate Program. If the contractor does not participate in the RAPIDGate program, the contractor shall develop a contractor-provided identification badge made of nonmetallic material, easily readable and include the employee's name, the contractor's name, and a recent color photograph of the employee. Specific requirements for identification badges issued under the RAPIDGate Program are Appendix B, Local Instructions, paragraph 4.13, Identification of Contractor's Employees RAPIDGate
Program. Contractor personnel shall wear the identification badge at all times when performing work under this contract at a government site, including while attending government meetings that may take place outside the government’s facility. Each contractor employee shall wear the identification badge in a conspicuous place on the front of exterior clothing and above the waist except when safety or health reasons prohibit such placement. Force Protection Condition (FPCON) at any individual facility or installation change, the Government may require changes in contractor security matters or processes.
9.3. iWATCH: All contractor employees and their subcontractor employees required to work on JBLM shall complete iWatch training prior to commencing work on JBLM. The contractor shall submit a signed company letter stating the one-time iWatch training has been or will be completed by all employees and subcontractor employees prior to commencing any work on JBLM. Compliance may be verified by government
QA. iWatch training is available at the following website:
https://www.bliss.army.mil/iWATCH/. This training will be used to inform employees of the types of behavior to watch for and instruct employees to report suspicious activity. Report the suspicious activity to the JBLM Fusion Center (CRIMINT) 253-966-7303/7320, 911 or usarmy.jblm.imcom.list.des-provost-marshal@mail.mil. The “Report
Suspicious Activity” flyer will be posted at the job site.
9.4. OPSEC
9.4.1. Operations security (OPSEC) denies adversaries sensitive information, critical information or essential elements of friendly information. This information requires special protection from disclosure that could compromise or cause a threat to our National
Security, an Army organization, activity, family member, DA civilian, or DOD contractor. OPSEC is everyone’s responsibility that comes in contact with the information. Information that is described below must be controlled, and if the information is in digital or hardcopy format, it needs to be destroyed at contract completion. Control includes not disclosing the information to personnel who do not have the need to have this information; if digital, do not post in a manner open to the public (on cloud storage or publicly available site) and if hard copy, secure in a locked area (office, trailer, vehicle, etc…) Examples which may be deemed sensitive include but are not limited to information related to:
Unit structuring manning equipment
Training and readiness vulnerabilities administration and personnel
Military Operations Planning intelligence, counterintelligence security logistics deploying financial data and funding

9.4.2. Handle any attempt by unauthorized personnel to solicit sensitive information (for example computer breach or questioning employees on or off JBLM) as a Threat Awareness and Reporting. Program (TARP) incident. Report all facts immediately to the JBLM Fusion Center

9.4.3. OPSEC Level I Training. 9.5. The Contractor and subcontractors shall pre-screen all employees required to work on JBLM using the E-Verify website
(http://www.dhs.gov/E-Verify) to meet the established employment eligibility requirements. The Contractor shall ensure that the employee has two valid forms of government-issued identification to ensure the correct information is entered into the E-verify system. The contractor shall submit a signed company letter stating all employees have or will be screened in E-Verify prior to commencing any work on JBLM.

11 OTHER CONDITIONS AFFECTING WORK
11.1 Work Coordination
11.1.1 Interfacing With Others
11.1.1.1 The Contractor shall plan its work activities in a manner such that there will be minimal interference and inconvenience to ongoing traffic and daily operations in the facility and local areas.
11.1.1.2 All work will be scheduled and coordinated with the government’s project management team or designee at all times.
11.1.1.3 The contractor may not block traffic, roads, access routes, or exits without prior coordination with the project manager or other authorized government representative.

11.2 Pre-Construction Conference
11.2.1 The contractor shall attend a pre-work meeting presided over by the project manager prior to commencing construction activities. The

11.2.2 Pre-Construction CQC Coordination Meeting
11.2.2.1 The Contractor shall attend a CQC Coordination meeting held in accordance with specification section titled QUALITY CONTROL.
11.3 Contractor Personnel and Sub-Contractor Qualifications
11.3.1 Contractor personnel shall meet the qualification requirements as outlined in the Base Contract.
11.4 Site Safety and Health Officer
11.4.1 The Contractor shall provide a Site Safety and Health Officer
(SSHO) as required in the specification section titled GOVERNMENTAL
SAFETY REQUIREMENTS.
11.5 Construction Quality Control System Manager
11.5.1 The Contractor shall provide a Construction Quality Control (CQC)
System Manager as required in the specification section titled QUALITY
CONTROL.
11.6 Compliance with Rules, Regulations, and Statutes
11.6.1 All contractor employees shall observe and comply with all applicable local, State, and Federal rules, regulations, and statutes including those concerning fire, safety, sanitation, security, vehicle safety, and hazardous material handling while performing this Task
Order. All work shall meet or exceed all applicable industry standards.
11.7 Firearms
11.7.1 Contractor personnel shall carry no firearms while performing work under this Contract.
11.8 Site Utilities
11.8.1 Outages
11.8.1.1 Any utility outages, including but not limited to, electrical, mechanical, and plumbing systems, shall be coordinated at least ten
(10) working days prior to proposed outages. Outages shall be scheduled to minimize interruptions to normal using agency operations.
11.8.2 Potable Water
11.8.2.1 The Government shall make available to the Contractor, from existing outlets and supplies, reasonable amounts of potable water without charge. The Contractor shall conserve potable water furnished.
The Contractor at its own expense shall install and maintain necessary temporary connections and distribution lines and shall remove the connections and lines prior to final acceptance of construction. All connections must be coordinated through the Directorate of Public
Works.
11.8.3 Electricity
11.8.3.1 Subject to available supply, the Government, without charge, shall make reasonable amounts of electric current available to the
Contractor for performing work at the work area. The Contractor shall conserve electric current furnished. The Contractor at its own expense shall install and maintain necessary temporary connections and distribution lines and shall remove the connections and lines prior to final acceptance of construction.
11.9 Permits

11.9.1 The Contractor shall obtain and submit copies of ALL permits, unless otherwise specified. Each permit shall be obtained before starting the respective items of work. This includes, but is not limited to, permits necessary for decontaminating water and sewer systems, and digging permits.
11.9.2 Contractors must obtain a "Dig Permit" from Public Works prior to any ground disturbing activities.
11.9.3 Once utilities have been marked as part of the dig permit process, the contractor shall document (photographs and plan drawings) and maintain the markings for the duration of the project.
11.10 Safety
11.10.1 USACE Safety and Health Requirements Manual (EM 385-1-1), available online at http://www.publications.usace.army.mil/USACEPublications/EngineerManual s.aspx, shall be considered to be a part of this Task Order and will be enforced as such. The Contractor shall prepare an accident prevention plan in accordance with the most current version of EM 385-1-1.
11.10.2 The Traffic Control Plan, as required in section 08 of the EM
385-1-1, shall be prepared in accordance with the Manual on Uniform
Traffic Control Devices.
11.11 Operation and Maintenance Requirements
11.11.1 Operation and maintenance manuals as required by the UFGS
Technical Specifications (Divisions 2 thru 48), shall be provided by the Contractor, and edited as required. The intent of the O&M Manuals is to promote and maximize the efficiency, economy, safety, and effectiveness of the life cycle operation, maintenance, and repair of the facility.
11.12 Construction Facilities And Temporary Controls
11.12.1 Staging areas are available for use at the project site.
11.12.2 The Contractor is responsible for security of its own property and security of government property when construction activities affect existing security measures.
11.12.3 The contractor shall provide and maintain portable toilet facilities at the site for the use of contractor and subcontractor personnel, as required by the EM 385-1-1. The units shall be emptied and cleaned at least once a week, or more often if required by the contracting officer. The doors to the unit shall be self-closing.
Additionally, the toilet units shall be located out of public view as the site allows.
