STATEMENT OF WORK

Repair Building 1010 Parking Lot, (JBLM-LEWIS/MCCHORD), BLDG. 1010
Date: 5/15/2017
1. DESCRIPTION OF WORK: Provide all plant, labor, tools, equipment, and materials (except as indicated otherwise in the specifications) and perform all work in strict accordance with the specifications and plans required for the Single Award Task Order Contract (SATOC). The Contractor shall accomplish all items of work needed for this task order in accordance with the applicable drawings, specifications, AWWA, NEC, NFPA, Washington State (WAC), ADA, and provisions of the SATOC contract in order to provide the government a complete and useable product.
1.1. AREA DESCRIPTION: This project removes the concrete pavement parking lot behind BLDG. 1010 and replaces it with asphalt pavement. The existing ADA parking will be relocated to the south of the building and a concrete ADA ramp will be installed to connect the new ADA parking and BLDG. 1010. A stormwater bio-swale will be installed to accommodate the stormwater of the new asphalt pavement. Drain rock will be installed parallel to the parking lot and adjacent to buildings 1010 & 1011. The drain rock is to intercept the stormwater from the roof drains from both buildings. A 6-inch water line will be relocated in order to accommodate the bio-swale.
2. PRINCIPLE ITEMS TO BE ACCOMPLISHED:
2.1. CLIN 0001AA: Contractor shall Repair BLDG. 1010 Parking Lot
2.1.1. Demolish concrete pavement.
2.1.2. Demolish asphalt pavement.
2.1.3. Demolish concrete curbs.
2.1.4. Demolish concrete stairs.
2.1.5. Demolish tree.
2.1.6. Clear & grub.
2.1.7. Remove irrigation system to accommodate excavation.
2.1.8. Excavate as required to install pavement, sidewalks, curb & gutter, ADA ramp, drain rock and bio-swale.
2.1.9. Demolish stormwater catch basins and associated pipe.
2.1.10. Demolish potable water lines.
2.1.11. Remove fire hydrant.
2.1.12. Relocate traffic & parking signs.
2.1.13. Remove ADA chair lift and return to the government.
2.1.14. Install Crushed Surfacing Top Course.
2.1.15. Install asphalt pavement.
2.1.16. Install curbs & gutters.
2.1.17. Install concrete sidewalks.
2.1.18. Install concrete sidewalk ramps (ADA).
2.1.19. Install ADA ramp to BLDG.1010. The ramp is to have a broom finish.
2.1.20. Handrail for ramp shall be aluminum or metal, with three horizontal rails, square vertical pickets, and either anodized or powder coated black color.
2.1.21. Install 12-inch diameter stormwater pipe.
2.1.22. Install Type 1 & 2 catch basins.
2.1.23. Install 6-inch potable waterline and valves.
2.1.24. Install fire hydrant.
2.1.25. Adjust manholes and utilities as required.
2.1.26. Install bio-swale soil.
2.1.27. Perform (1 ea.)Small-Scale Pilot Infiltration Test (PIT) and (3 ea.) Soil Grain Size Analysis with following the Stormwater Management Manual for Western Washington, Volume III, Hydrologic Analysis and Flow Control BMPs, December 2014
2.1.28. Install top soil.
2.1.29. Install sod.
2.1.30. Install irrigation to accommodate the new landscape adjacent to
BLDG. 1010 and connect to the existing irrigation system.
2.1.31. Install drain rock.
2.1.32. Relocate existing traffic signs and install on new metal posts.
2.1.33. Install new traffic & ADA Parking signs and metal posts.
2.1.34. Install parking lot paint markings.
2.1.35. Other work necessary to accomplish the above items:
a. The Contractor shall be responsible for providing a complete and useable product/system and shall be responsible for accomplishing all tasks incidental to the completion of the task order requirements. Requirements for a complete and useable product which are not explicitly called out in the SOW but reasonably discernable via any method other than destructive testing shall be considered a requirement of the Task Order.
2.2. Standards for finish and installation
2.2.1. All means, methods, and proposal submission requirements shall conform to specifications set forth in the SATOC contract and this statement of work. All construction and contractor provided products shall conform to the current JBLM Architectural, Structural, Electrical, Mechanical, Life Safety, and Environmental standards.
2.3. Proposal Support documents required for proposal:
2.3.1. Contractor shall supply the following items, at a minimum, to comply with the proposal support document requirements set forth in the SATOC contract:
CLIN 0001AA: The contractor’s proposal shall include an ELIN and bid schedule, work and traffic control plan, cutsheets and competitive quotes for Non Pre-Priced items over $2,500.
3. CONTRACT CONSTRUCTION PERIOD: The Contractor is required to commence work under this Task Order within 15 calendar days after the date of award, work diligently, and complete all work no later than 60 calendar days after the date of issuance of the Task Order Award. The time stated for completion includes final cleanup of the premises. The Legal Completion Date associated with this task order is the date when all construction work must be complete and a final inspection conducted and accepted; this is the date when Liquidated Damages will be assessed. Until all required close-out documentation has been submitted and accepted by the government, 5% of the task order price will be withheld.

3.1. The work site will not be in use during construction.

4.1. Dimensions and utility locations are approximate and must be verified in the field. All supplied As-Builts, shop drawings, and/or sketches are for conceptual use and the government will not be liable for any discrepancies.
5. CONSTRUCTION REQUIREMENTS: The contractor shall notify the Government at least 7 calendar days prior to starting the on-site phase of the project.
All work shall be scheduled and coordinated with the Government Project Management Team or their designee. The following phasing related issues shall affect the work.
5.1. Interface with Others: Government use of these facilities is anticipated while the work under this contract is being performed. The work shall be planned and accomplished so that there will be a minimum of interference and inconvenience to occupants in the building, daily operations of the facility, and the vicinity.
5.1.1. Any blockage of building exits, roads, or driveways must be coordinated in advance with the Building POC.
5.1.2. Building/Project POC – Project Manager
5.2. Work Hours: Normal work hours are from 0730 to 1630, Monday through
Friday, except Federal holidays. Contractor shall not be permitted to work other than normal work hours unless authorized by the Contracting Officer or Contracting Officer Representative. The contractor may request base access through the Contracting Office.
5.3. Liquidated Damages: Liquidated damages shall be in accordance with the base contract.
5.4. Permits:
5.4.1. The Contractor shall obtain and submit copies of ALL permits, unless otherwise specified. Each permit shall be obtained before starting the respective items of work. The includes, but is not limited to, permits necessary for decontaminating water and sewer systems, air permits, and digging permits.
5.4.2. Contractors must obtain a “Dig Permit” from Public Works prior to any ground disturbing activities.
5.4.3. Once utilities have been marked as part of the dig permit process, the contractor shall document (photographs and plan drawings) and maintain the markings for the duration of the project.
5.5. Environmental Project Restrictions: The Contractor shall comply with all Joint Base Lewis-McChord (JBLM) environmental requirements, including the most current installation regulation 200-1 (Environmental Protection and Enhancement) and SATOC specification section 01 57 20.00 10 (Environmental Protection). This includes the submission of an Environmental Protection Plan prior to commencement, as detailed in section 01 57 20.00 10. The JBLM Environmental Division will review the plan and provide any necessary corrections or additions to the Contracting Officer or their representative. In addition, the following shall be adhered to:
5.5.1. Environmental Management System (EMS): The JBLM EMS conforms to the criteria defined in the international standard, ISO 14001:2004 Environmental management systems – Requirements with guidance for use.
In accordance with EMS procedures, the Contractor shall be familiar with the JBLM environmental policy and shall ensure that this information is considered and incorporated into this project. Additional information regarding the JBLM EMS is available from the installation EMS Coordinator (253-966-6470).
5.5.2. Sustainable Acquisition. In order to comply with federally mandated sustainable acquisition requirements and Department of Defense (DOD) and Department of the Army (DA) Green Procurement Program (GPP) policies, Joint Base Lewis-McChord (JBLM) requires the purchase, supply, and use of environmentally preferable products and services. Program elements include: recovered material products, energy and water efficient products, alternative fuels and fuel efficiency, bio-based products, non-ozone depleting substances, and environmentally preferable products. Additionally, the JBLM Guide to Sustainable Acquisition for Construction is available in the “Reference Manuals” section of the JBLM Design Standards webpage (http://www.lewis-mcchord.army.mil/designstandards/index1.htm).
The Contractor shall comply with all applicable Federal, DOD, Army, and JBLM sustainable acquisition laws and policies. If compliant products are unavailable or determined to be impracticable based on cost or performance, the Contractor shall contact the Contracting Officer or their representative for further guidance. 5.5.3. Waste-Water: Any waste-water generated during this project is subject to JBLM EPA MS4 Storm Water Permit. To comply with the permit, all contaminated wastewater produced by this project must be collected, processed, characterized, and disposed of into the JBLM sanitary sewer system or another authorized disposal method. JBLM sanitary sewer discharge requires a JBLM Sanitary Sewer Discharge Permit which is used by DPW Waste Water Treatment Plant. Prior to discharge a laboratory analysis of the waste water may be required for review and approval by Wastewater Plant Personnel. Process water is not authorized to discharge to the ground or JBLM storm sewer. The Environmental Protection Plan for the project should identify any potential wastewater generation and the process to which that water will be properly disposed.
5.5.4. Hazardous Materials: For any hazardous materials, the Contractor shall submit an initial list of hazardous materials to be used (including unit of measure and Safety Data Sheet for each product).
A copy of these documents must be submitted to the Joint Base Lewis- McChord (JBLM) Pollution Prevention Program (usarmy.jblm.imcom.list.dpw-hm-inventory@mail.mil prior to commencement. These items must be reviewed by Pollution Prevention and authorized for use if they are not already on the JBLM installation Authorized Use List. The Contractor shall not use any hazardous materials not authorized by Pollution Prevention.
For any in-use hazardous materials, the Contractor shall submit a hazardous material inventory on HJB Form 953 noting the hazardous materials used, the units of measure, quantities used, storage locations, and other information indicated on the form. This inventory shall be submitted at the end of the project. [If the project goes into a new calendar year, the Contractor shall also submit a copy of this inventory (accounting for all hazardous materials) by 15 January.] For any hazardous materials stored on JBLM, this inventory shall be submitted by the 15th day following the calendar year quarter in which the hazardous materials were used or stored. All HJB 953 inventories shall be submitted electronically and addressed to the Contracting Officer or their representative with usarmy.jblm.imcom.list.dpw-hm-inventory@mail.mil included in the carbon copy (“cc”) line of the email. Questions regarding the form may be directed to the JBLM Pollution Prevention Program (966-6469).
5.6. Utility Outages: Work shall be scheduled so that such utility outages are minimized. Any outages shall be coordinated with building’s users, through the JBLM PW Project Manager and/or JBLM PW Quality Assurance.
A minimum of 14 calendar days is required for notification of utility outages.
5.6.1. Availability of Utilities: Subject to available supply, reasonable amounts of potable water and electricity will be furnished by the Government, without charge, to the contractor from existing outlets and supplies. Contractor shall reasonably conserve these resources. 6. SUBMITTALS: All submittals shall be submitted in accordance with the specification section titled SUBMITTAL PROCEDURES. Required submittals are identified in their applicable specification section.
All sketches, modifications, adaptations, quality assurance requirements, and contractual provided products should be submitted to the government for review and consideration. Any submittals determined to be necessary during the task order will become a requirement of the contract. Accurate submission of submittals remains the responsibility of the contractor and verification remains the requirement of the government.
7. ASBESTOS AND LEAD PAINT:
7.1. Notification of “Does Not Exist” from Public Works or other Federal organization concerning all Hazardous Materials does not alleviate the contractor’s responsibilities of a “good faith” survey required by the current contract to protect their workers if suspect material is discovered. This good faith survey is to be conducted, if necessary, at the contractor’s expense. All surveys conducted by the contractor, whether showing lack of ACM or confirmation, shall be submitted to the Government.
7.2. ASBESTOS SURVEY:
7.2.1. An AHERA survey is not required as part of this project.
7.3. LEAD PAINT SURVEY:
7.3.1. A lead paint survey is not required as part of this project.
8. Anti Terrorism (AT) and Operational Security (OPSEC)
8.1. AT Level I Training: All contractor and their subcontractors required to work on JBLM shall complete AT Level 1 awareness training prior to commencing work on JBLM. The contractor shall submit a signed company letter stating the annual AT Level 1 training has been or will be completed by all employees and subcontractors prior to commencing any work on JBLM. Training certificates shall be maintained and be available for inspection. Compliance may be verified by government QA.

8.2. Access and General Protection/Security Policy and Procedures: All contractor and all associated sub-contractors employees shall comply with applicable installation, facility and area commander https://jkodirect.jten.mil/ https://jkodirect.jten.mil/ installation/facility access and local security policies and procedures (provided by government representative). The contractor shall comply with Joint Base Lewis-McChord security regulations: JBLM Regulation 210 1, Fort Lewis Post Regulations, Chapter 7, section XVII; JBLM Regulation 190-16, Access to and Conduct While upon the Fort Lewis Military Reservation; and JBLM Regulation 190-5, Installation Traffic Code. Contractor employees shall display either contractor provided identification badges or the identification badges issued under the RAPIDGate Program. If the contractor does not participate in the RAPIDGate program, they shall develop a contractor-provided identification badge made of nonmetallic material, easily readable and include the employee's name, the contractor's name, and a recent color photograph of the employee. Specific requirements for identification badges issued under the RAPIDGate Program are Appendix B, Local Instructions, paragraph 4.13, Identification of Contractor's Employees RAPIDGate Program. Contractor personnel shall wear the identification badge at all times when performing work under this contract at a Government site, including while attending Government meetings that may take place outside the Government facility. Each contractor employee shall wear the identification badge in a conspicuous place on the front of exterior clothing and above the waist except when safety or health reasons prohibit such placement. The contractor shall also provide all information required for background checks to meet installation access requirements to be accomplished by installation Provost Marshal Office, Director of Emergency Services or Security Office. Contractor workforce must comply with all personal identity verification requirements as directed by DOD, HQDA and/or local policy. In addition to the changes otherwise authorized by the changes clause of this contract, should the Force Protection Condition (FPCON) at any individual facility or installation change, the Government may require changes in contractor security matters or processes.
8.3. iWATCH: All contractor employees and their subcontractors required to work on JBLM shall complete iWatch training prior to commencing work on JBLM. The contractor shall submit a signed company letter stating the one-time iWatch training has been or will be completed by all employees and subcontractors prior to commencing any work on JBLM. Compliance may be verified by government QA. iWatch training is available at the following website:
https://www.bliss.army.mil/iWATCH/. This training will be used to inform employees of the types of behavior to watch for and instruct employees to report suspicious activity. Report the suspicious activity to the JBLM Fusion Center (CRIMINT) 253-966-7303/7320, 911 or usarmy.jblm.imcom.list.des-provost-marshal@mail.mil. The “Report Suspicious Activity” flyer will be posted at the job site.
8.4. OPSEC
8.4.1. Operations security (OPSEC) denies adversaries sensitive information, critical information or essential elements of friendly information. This information requires special protection from disclosure that could cause compromise or threat to our National Security, an Army organization, activity, family member, DA civilian, or DOD contractor. OPSEC is everyone’s responsibility that comes in contact with the information. Information that is described below must be controlled, and if the information is in digital or hardcopy format, it needs to be destroyed at contract completion. Control mailto:usarmy.jblm.imcom.list.des-provost-marshal@mail.mil includes not disclosing the information to personnel who do not have the need to have this information; if digital do not post in a manner open to the public (on cloud storage or publicly available site) and if hard copy secure in a locked area (office, trailer, vehicle, etc…) Examples which may be deemed sensitive include but are not limited to information related to:
• Unit structuring
• manning
• equipment
• Training and readiness
• vulnerabilities
• administration and personnel
• Military Operations Planning
• intelligence, counterintelligence
• security
• logistics
• deploying
• acquisition plans
• capabilities
• morale
• financial data and funding
• Drawings, blue prints, as-builts and designs
• utility systems information (electrical, water, waste water, storm water, gas, communications)
8.4.2. Handle any attempt by unauthorized personnel to solicit sensitive information (for example computer breach or questioning employees on or off JBLM) as a Threat Awareness and Reporting. Program (TARP) incident. Report all facts immediately to the JBLM Fusion center (iWatch para 10.3) and inform the COR.
8.4.3. OPSEC Level I Training. All contractor employees and their subcontractors required to work on JBLM or have access to contract documents shall complete Level I OPSEC training prior to commencing work on JBLM. The contractor shall submit a signed company letter stating the annual Level I OPSEC training has been or will be completed by all employees prior to commencing any work on JBLM.
Training certificates shall be maintained and be available for inspection. Compliance may be verified by government QA. Level I OPSEC training is available at the following website:

8.5. The Contractor and subcontractors shall pre-screen all employees required to work on JBLM using the E-Verify website (http://www.dhs.gov/E-Verify) to meet the established employment eligibility requirements. The Contractor shall ensure that the employee has two valid forms of Government issued identification to ensure the correct information is entered into the E-verify system. The contractor shall submit a signed company letter stating all employees have or will be screened in E-Verify prior to commencing any work on JBLM.
