STATEMENT OF WORK

REPAIR MAIN GATE PAVEMENT

(JBLM-LEWIS/MCCHORD)

Date: 3/14/2018

1. DESCRIPTION OF WORK: Provide all plant, labor, tools, equipment, and materials (except as indicated otherwise in the specifications) and perform all work in strict accordance with the specifications and plans required for the Single Award Task Order Contract (SATOC). The Contractor shall accomplish all items of work needed for this task order in accordance with the applicable drawings, specifications, AWWA, NEC, NFPA, Washington State (WAC), ADA, and provisions of the SATOC contract in order to provide the government a complete and useable product.

1.1. AREA DESCRIPTION: This project removes and replaces the asphalt pavement at JBLM’s Main Gate entrance between the perimeter gate and Entry Control Point vehicle barriers with asphalt pavement. The asphalt pavement parking lot for the Visitor Center, BLDG. 05901, will also be removed and replaced with asphalt pavement.

2. PRINCIPLE ITEMS TO BE ACCOMPLISHED:

2.1. CLIN 0002AA: Main Gate Pavement Repair

2.1.1. Demolish asphalt pavement.

2.1.2. Demolish concrete curbs.

2.1.3. Demolish concrete sidewalks.

2.1.4. Remove street & parking lot lights and return the poles, mast arms, and lights to the government.

2.1.5. Relocate CCTV & antennas attached to existing parking lot light poles to the new light poles.

2.1.6. Demolish vehicle traffic induction loops at ECP.

2.1.7. Clear & grub.

2.1.8. Demolish trees.

2.1.9. Remove irrigation system to accommodate excavation.

2.1.10. Excavate as required to install pavement, sidewalks, curb & gutter, ADA ramp, drain rock and bio-swale.

2.1.11. Demolish stormwater catch basins and associated pipe.

2.1.12. Demolish vehicle traffic bollards.

2.1.13. Demolish ground mounted flood lights to BLDG. 05903.

2.1.14. Demolish utility pole and cable from BLDG. 5902 to power meter.

2.1.15. Install Crushed Surfacing Top Course.

2.1.16. Install asphalt pavement.

2.1.17. Install curbs & gutters.

2.1.18. Install raised traffic islands with brick pattern stamped concrete.

2.1.19. Install concrete sidewalks.

2.1.20. Install concrete sidewalk ramps (ADA).

2.1.21. Install 12-inch diameter stormwater pipe.

2.1.22. Install Type 1 & 2 catch basins.

2.1.23. Adjust manholes and utilities as required.

2.1.24. Install bio-swale and bio-swale soil.

2.1.25. Install stormwater dispersion system and topsoil.

2.1.26. Perform (2 ea.)Small-Scale Pilot Infiltration Test (PIT) and (3 ea.) Soil Grain Size Analysis with following the Stormwater Management Manual for Western Washington, Volume III, Hydrologic Analysis and Flow Control BMPs, December 2014

2.1.27. Install top soil.

2.1.28. Install grass seed.

2.1.29. Install irrigation to accommodate the new landscape.

2.1.30. Install drain rock.

2.1.31. Install traffic signs and new metal posts.

2.1.32. Install new ADA Parking signs and new metal posts.

2.1.33. Install parking lot paint markings.

2.1.34. Install decorative vehicle traffic bollards.

2.1.35. Install roadway and entry control point paint markings.

2.1.36. Install vehicle induction loops at ECP & Barriers.

2.1.37. Install street and parking lot lights.

2.1.38. Install electrical hand holds, conduit, and cable.

2.1.39. Install electrical power service panel in BLDG. 05903.

2.1.40. Install LED in-ground lights to BLDG. 05903.

2.1.41. Re-lamp exterior lights under BLDG. 05903 arch.

2.1.42. Tee tap electrical power from BLDG. 05902 to existing underground power.

2.1.43. Other work necessary to accomplish the above items:

a. The Contractor shall be responsible for providing a complete and useable product/system and shall be responsible for accomplishing all tasks incidental to the completion of the task order requirements. Requirements for a complete and useable product which are not explicitly called out in the SOW but reasonably discernable via any method other than destructive testing shall be considered a requirement of the Task Order.

2.2. CLIN 0002AB: Install Sidewalk

2.2.1. Clear & grub, excavate, & install sidewalk that is to connect to the existing sidewalk in the vicinity of the front gate monument (BLDG. 5903), parallel to 41st Division towards the intersection of 41st Division & Ohio Street.

2.2.2. Other work necessary to accomplish the above items:

The Contractor shall be responsible for providing a complete and useable product/system and shall be responsible for accomplishing all tasks incidental to the completion of the task order requirements. 2.3. CLIN 0002AC: Install Decorative Fence

2.3.1. Install fence, 4-ft height, black aluminum, commercial grade, vertical pickets between Visitors Center Parking Lot and Visitors Center BLDG. P05901.

2.3.2. Install fence, 4-ft height, black aluminum, commercial grade, vertical pickets between Guard Shack, BLDG.P05905 and cantonment perimeter fence.

2.3.3. Characteristics of the Fence:

-Horizontal Rails: 1.25" square x .10" aluminum (min.)

-Vertical Pickets: 3/4” square x .050 aluminum (min) -Picket Tops: 6” Sand Cast Finials -Panel Width: 8-Feet -Finish: Black fade-resistant powder coat finish

2.4.1. Install irrigation system in bio-swales between the Visitors Center Parking Lot and 41st Division.

2.4.2. Install irrigation system in the bio-swale between the monument, BLDG. P05903, and BLDG. P05905.

2.2. Standards for finish and installation

2.2.1. All means, methods, and proposal submission requirements shall conform to specifications set forth in the SATOC contract and this statement of work. All construction and contractor provided products shall conform to the current JBLM Architectural, Structural, Electrical, Mechanical, Life Safety, and Environmental standards.

2.3. Proposal Support documents required for proposal:

2.3.1. Contractor shall supply the following items, at a minimum, to comply with the proposal support document requirements set forth in the SATOC contract:

The contractor’s proposal shall include an ELIN and bid schedule, work, construction phase and traffic control plans, cutsheets and competitive quotes for Non Pre-Priced items over $2,500.

3. CONTRACT CONSTRUCTION PERIOD: The Contractor is required to commence work under this Task Order within 15 calendar days after the date of award, work diligently, and complete all work no later than 164 calendar days after the date of issuance of the Task Order Award. The time stated for completion includes final cleanup of the premises. The Legal Completion Date associated with this task order is the date when all construction work must be complete and a final inspection conducted and accepted; this is the date when Liquidated Damages will be assessed. Until all required close-out documentation has been submitted and accepted by the government, 5% of the task order price will be withheld.

Construction Duration: 120 Closeout Submittal Duration: 30

3.1. The work site will be in use during construction.

3.2. This is a short duration for this project and the contractor will be allowed extended hours operations as addressed in para 5.2 to complete the project.

4.1. Dimensions and utility locations are approximate and must be verified in the field. All supplied As-Builts, shop drawings, and/or sketches are for conceptual use and the government will not be liable for any discrepancies.

5. CONSTRUCTION REQUIREMENTS: The contractor shall notify the Government at least 7 calendar days prior to starting the on-site phase of the project.

All work shall be scheduled and coordinated with the Government Project Management Team or their designee. The following phasing related issues shall affect the work.

5.1. Interface with Others: Government use of these facilities is anticipated while the work under this contract is being performed. The work shall be planned and accomplished so that there will be a minimum of interference and inconvenience to occupants in the building, daily operations of the facility, and the vicinity.

5.1.1. Any blockage of building exits, roads, or driveways must be coordinated in advance with the Building POC.

5.1.2. Building/Project POC – Project Manager.

5.1.3. A minimum of two inbound lanes and one refusal lane is required at all times at the Access Control Point (ACP).

5.1.4. Outbound through traffic will not be permitted through the ACP during construction.

5.1.5. A weekend closure of both inbound and outbound vehicle traffic lanes and visitor center parking lot may be permitted in order to place the final asphalt pavement wearing course.

5.2. Work Hours: Normal work hours are from 0630 to 1930, Monday through

Saturday. Contractor shall not be permitted to work other than normal work hours unless authorized by the Contracting Officer or Contracting Officer Representative. The contractor may request base access through the Contracting Office.

5.3. Liquidated Damages: Liquidated damages shall be in accordance with the base contract.

5.4. Permits:

5.4.1. The Contractor shall obtain and submit copies of ALL permits, unless otherwise specified. Each permit shall be obtained before starting the respective items of work. The includes, but is not limited to, permits necessary for decontaminating water and sewer systems, air permits, and digging permits.

5.4.2. Contractors must obtain a “Dig Permit” from Public Works prior to any ground disturbing activities.

5.4.3. Once utilities have been marked as part of the dig permit process, the contractor shall document (photographs and plan drawings) and maintain the markings for the duration of the project.

5.5. Environmental Project Restrictions: The Contractor shall comply with all Joint Base Lewis-McChord (JBLM) environmental requirements, including the most current installation regulation 200-1 (Environmental Protection and Enhancement) and SATOC specification section 01 57 20.00 10 (Environmental Protection). This includes the submission of an Environmental Protection Plan prior to commencement, as detailed in section 01 57 20.00 10. The JBLM Environmental Division will review the plan and provide any necessary corrections or additions to the Contracting Officer or their representative. In addition, the following shall be adhered to:

5.5.1. Environmental Management System (EMS): The JBLM EMS conforms to the criteria defined in the international standard, ISO 14001:2004 Environmental management systems – Requirements with guidance for use.

In accordance with EMS procedures, the Contractor shall be familiar with the JBLM environmental policy and shall ensure that this information is considered and incorporated into this project. Additional information regarding the JBLM EMS is available from the installation EMS Coordinator (253-966-6470).

5.5.2. Sustainable Acquisition. In order to comply with federally mandated sustainable acquisition requirements and Department of Defense (DOD) and Department of the Army (DA) Green Procurement Program (GPP) policies, Joint Base Lewis-McChord (JBLM) requires the purchase, supply, and use of environmentally preferable products and services. Program elements include: recovered material products, energy and water efficient products, alternative fuels and fuel efficiency, bio-based products, non-ozone depleting substances, and environmentally preferable products. Additionally, the JBLM Guide to Sustainable Acquisition for Construction is available in the “Reference Manuals” section of the JBLM Design Standards webpage (http://www.lewis-mcchord.army.mil/designstandards/index1.htm).

The Contractor shall comply with all applicable Federal, DOD, Army, and JBLM sustainable acquisition laws and policies. If compliant products are unavailable or determined to be impracticable based on cost or performance, the Contractor shall contact the Contracting Officer or their representative for further guidance. 5.5.3. Waste-Water: Any waste-water generated during this project is subject to JBLM EPA MS4 Storm Water Permit. To comply with the permit, all contaminated wastewater produced by this project must be collected, processed, characterized, and disposed of into the JBLM sanitary sewer system or another authorized disposal method. JBLM sanitary sewer discharge requires a JBLM Sanitary Sewer Discharge Permit which is used by DPW Waste Water Treatment Plant. Prior to discharge a laboratory analysis of the waste water may be required for review and approval by Wastewater Plant Personnel. Process water is not authorized to discharge to the ground or JBLM storm sewer. The Environmental Protection Plan for the project should identify any potential wastewater generation and the process to which that water will be properly disposed.

5.5.4. Hazardous Materials: For any hazardous materials, the Contractor shall submit an initial list of hazardous materials to be used (including unit of measure and Safety Data Sheet for each product).

McChord (JBLM) Pollution Prevention Program (usarmy.jblm.imcom.list.dpw-hm-inventory@mail.mil prior to commencement. These items must be reviewed by Pollution Prevention and authorized for use if they are not already on the JBLM installation Authorized Use List. The Contractor shall not use any hazardous materials not authorized by Pollution Prevention.

For any in-use hazardous materials, the Contractor shall submit a hazardous material inventory on HJB Form 953 noting the hazardous materials used, the units of measure, quantities used, storage locations, and other information indicated on the form. This inventory shall be submitted at the end of the project. [If the project goes into a new calendar year, the Contractor shall also submit a copy of this inventory (accounting for all hazardous materials) by 15 January.] For any hazardous materials stored on JBLM, this inventory shall be submitted by the 15th day following the calendar year quarter in which the hazardous materials were used or stored. All HJB 953 inventories shall be submitted electronically and addressed to the Contracting Officer or their representative with usarmy.jblm.imcom.list.dpw-hm-inventory@mail.mil included in the carbon copy (“cc”) line of the email. Questions regarding the form may be directed to the JBLM Pollution Prevention Program (966-6469).

5.6. Utility Outages: Work shall be scheduled so that such utility outages are minimized. Any outages shall be coordinated with building’s users, through the JBLM PW Project Manager and/or JBLM PW Quality Assurance.

A minimum of 14 calendar days is required for notification of utility outages.

5.6.1. Availability of Utilities: Subject to available supply, reasonable amounts of potable water and electricity will be furnished by the Government, without charge, to the contractor from existing outlets and supplies. Contractor shall reasonably conserve these resources. Contractor shall provide, at its expense, sanitary accommodations for the use of employees and shall comply with the requirements and regulations of the State Health Department, County Sanitarian, or other authorities having jurisdiction.

6. SUBMITTALS: All submittals shall be submitted in accordance with the specification section titled SUBMITTAL PROCEDURES. Required submittals are identified in their applicable specification section.

All sketches, modifications, adaptations, quality assurance requirements, and contractual provided products should be submitted to the government for review and consideration. Any submittals determined to be necessary during the task order will become a requirement of the contract. Accurate submission of submittals remains the responsibility of the contractor and verification remains the requirement of the government.

7. ASBESTOS AND LEAD PAINT:

7.1. Notification of “Does Not Exist” from Public Works or other Federal organization concerning all Hazardous Materials does not alleviate the contractor’s responsibilities of a “good faith” survey required by the mailto:usarmy.jblm.imcom.list.dpw-hm-inventory@mail.mil mailto:usarmy.jblm.imcom.list.dpw-hm-inventory@mail.mil current contract to protect their workers if suspect material is discovered. This good faith survey is to be conducted, if necessary, at the contractor’s expense. All surveys conducted by the contractor, whether showing lack of ACM or confirmation, shall be submitted to the Government.

7.2. ASBESTOS CONTAINING MATERIAL:

7.2.1. An AHERA survey is not required as part of this project. suspect materials involved with all disturbance work are assumed to be Asbestos-Containing Material (ACM). It is the contractor’s responsibility to either test for, or to assume the suspect material is ACM, and that it is abated /encapsulated in accordance with AHERA, Federal, and Local regulations. No Asbestos Hazard Emergency Response Act (AHERA) inspection is required when the suspect materials are assumed to be ACM. The government reserves the right to accept assumptions of ACM-positive, or to require such AHERA surveys to be performed.

7.2.2. When performed, inspections shall be accomplished in accordance with the Asbestos Hazard Emergency Response Act (AHERA), Federal and Local regulations. A Certified AHERA Asbestos Building Inspector shall perform all inspections. The inspection documentation shall comply with AHERA, Federal and Local regulations and shall include assessment of each Asbestos-Containing Material (ACM), its location, quantity, type, friability and percentage of asbestos.

7.2.3. If the tested material is positive for asbestos, or assumed to be

ACM and must be disturbed during project execution, contractor shall have a certified Asbestos Contractor perform all asbestos disturbance work necessary to complete the project in accordance with 29 CFR 1926.1101, WAC 296-62-077, 296-65, and PSCAA Regulation 3.4. A certified abatement plan and all other applicable requirements of ‘UFGS 02 82 13.00 10 Asbestos Abatement’ are also required to be submitted and accepted before work is started. If this effort is believed to constitute differing site condition(s), then submit an RFI identifying the situation.

7.3. LEAD CONTAINING PAINT:

7.3.1. Existing suspect painted surface materials involved in disturbance work will be considered to contain detectable lead paint.

It is the contractor’s responsibility to either test for, or to assume the suspect material is either Lead Based Paint (LBP) or Lead Containing Paint (LCP), and that it is abated/encapsulated in accordance with OSHA, HUD, Federal and Local regulations. The government reserves the right to accept assumptions of LCP/LBP-positive, or to require such LCP/LBP surveys to be performed.

7.3.2. When performed, testing shall be accomplished in accordance with OSHA and/or HUD guidelines. A Certified Lead Paint Inspector shall perform lead paint inspections. The inspection documentation shall comply with Federal and Local regulations and shall include identification of all lead paint, its location, quantity, percentage by weight, and TCLP characteristics IAW ASTM E1908-16, “Standard Guide for Sample Selection of Debris Waste from a Building Renovation or Lead Abatement Project for Toxicity Characteristic Leaching Procedure (TCLP) Testing for Leachable Lead (Pb)”

7.3.3. If the suspect lead containing material is tested and proves positive for lead, or when assumed to be a lead containing painted surface, any disturbance work concerning the painted surfaces shall be in accordance with:

A contractor work plan is required to be submitted and accepted before work is started, in accordance with applicable sections of ‘UFGS 02 83

13.00 20 Lead in Construction’. If this effort is believed to constitute differing site condition(s), then submit and RFI identifying the situation.

8. Anti Terrorism (AT) and Operational Security (OPSEC)

8.1. AT Level I Training: All contractor and their subcontractors required to work on JBLM shall complete AT Level 1 awareness training prior to commencing work on JBLM. The contractor shall submit a signed company letter stating the annual AT Level 1 training has been or will be completed by all employees and subcontractors prior to commencing any work on JBLM. Training certificates shall be maintained and be available for inspection. Compliance may be verified by government QA.

8.2. Access and General Protection/Security Policy and Procedures: All contractor and all associated sub-contractors employees shall comply with applicable installation, facility and area commander installation/facility access and local security policies and procedures (provided by government representative). The contractor shall comply with Joint Base Lewis-McChord security regulations: JBLM Regulation 210 1, Fort Lewis Post Regulations, Chapter 7, section XVII; JBLM Regulation 190-16, Access to and Conduct While upon the Fort Lewis Military Reservation; and JBLM Regulation 190-5, Installation Traffic Code. Contractor employees shall display either contractor provided identification badges or the identification badges issued under the RAPIDGate Program. If the contractor does not participate in the RAPIDGate program, they shall develop a contractor-provided identification badge made of nonmetallic material, easily readable and include the employee's name, the contractor's name, and a recent color photograph of the employee. Contractor personnel shall wear the identification badge at all times when performing work under this contract at a Government site, including while attending Government meetings that may take place outside the Government facility. Each contractor employee shall wear the identification badge in a conspicuous place on the front of exterior clothing and above the waist except when safety or health reasons prohibit such placement. The contractor shall also provide all information required for background checks to meet installation access requirements to be accomplished by installation Provost Marshal Office, Director of Emergency Services or Security Office. Contractor workforce must comply with all personal identity verification requirements as directed by DOD, HQDA and/or local policy. In addition to the changes otherwise authorized by the changes clause of this contract, should the Force Protection Condition (FPCON) at any individual facility or installation change, the Government may require changes in contractor security matters or processes.

8.3. iWATCH: All contractor employees and their subcontractors required to work on JBLM shall complete iWatch training prior to commencing work on JBLM. The contractor shall submit a signed company letter stating the one-time iWatch training has been or will be completed by all employees and subcontractors prior to commencing any work on JBLM. Compliance may be verified by government QA. iWatch training is available at the following website:

https://www.bliss.army.mil/iWATCH/. This training will be used to inform employees of the types of behavior to watch for and instruct employees to report suspicious activity. Report the suspicious activity to the JBLM Fusion Center (CRIMINT) 253-966-7303/7320, 911 or usarmy.jblm.imcom.list.des-provost-marshal@mail.mil. The “Report Suspicious Activity” flyer will be posted at the job site.

8.4. OPSEC

8.4.1. Operations security (OPSEC) denies adversaries sensitive information, critical information or essential elements of friendly information. This information requires special protection from disclosure that could cause compromise or threat to our National Security, an Army organization, activity, family member, DA civilian, or DOD contractor. OPSEC is everyone’s responsibility that comes in contact with the information. Information that is described below must be controlled, and if the information is in digital or hardcopy format, it needs to be destroyed at contract completion. Control includes not disclosing the information to personnel who do not have the need to have this information; if digital do not post in a manner open to the public (on cloud storage or publicly available site) and if hard copy secure in a locked area (office, trailer, vehicle, etc…) Examples which may be deemed sensitive include but are not limited to information related to:

8.4.2. Handle any attempt by unauthorized personnel to solicit sensitive information (for example computer breach or questioning employees on or off JBLM) as a Threat Awareness and Reporting. Program (TARP) incident. Report all facts immediately to the JBLM Fusion center and inform the COR.

8.4.3. OPSEC Level I Training. All contractor employees and their subcontractors required to work on JBLM or have access to contract documents shall complete Level I OPSEC training prior to commencing work on JBLM. The contractor shall submit a signed company letter stating the annual Level I OPSEC training has been or will be completed by all employees prior to commencing any work on JBLM.

Training certificates shall be maintained and be available for inspection. Compliance may be verified by government QA. Level I OPSEC training is available at the following website:

8.5. The Contractor and subcontractors shall pre-screen all employees required to work on JBLM using the E-Verify website (http://www.dhs.gov/E-Verify) to meet the established employment eligibility requirements. The Contractor shall ensure that the employee has two valid forms of Government issued identification to ensure the correct information is entered into the E-verify system. The contractor shall submit a signed company letter stating all employees have or will be screened in E-Verify prior to commencing any work on JBLM.
