Supplemental Bidder Attestation for DOR Department of Motor Vehicles Credentialing System
This is a Supplemental Bidder Attestation form issued by the State of Rhode Island, Division of Purchases, pursuant to R.I. Gen. Laws 37-2-13.1(b), for the Department of Revenue, Division of Motor Vehicles Credentialing System replacement project (RFP26006341). The form requires authorized signatories from bidding entities to certify that their organization and all parent corporations, subsidiaries, affiliates, and subcontractors have no conflicts of interest with any state official, officer, or agency involved in the RFP's formation and promulgation. The attestation must confirm that the bidder did not materially participate in or receive consultation regarding the RFP's requirements, technical aspects, or development. The vendor question and answer submission period opens February 5, 2026 at 10:15 AM EST and closes February 18, 2026 at 2:00 PM EST, with all proposals due electronically through Ocean State Procures no later than March 5, 2026 at 11:00 AM EST. The initial contract term is four years beginning approximately July 1, 2026, with optional renewal periods of up to four additional 12-month terms contingent upon performance and fund availability.
The attestation form explicitly notes that current or past vendor status with the State does not automatically constitute a conflict of interest, and formal responses to prior Requests for Information do not disqualify vendors from future RFP participation. Bidders must complete the form by providing their entity name, authorized signatory information, solicitation number/title, and the execution date. The State will retain a 0.33% contract administrative fee deducted from all invoices. All awarded vendors must maintain comprehensive insurance coverage including general liability ($1,000,000), workers' compensation ($100,000), automobile liability ($1,000,000), professional liability ($2,000,000), technology errors and omissions ($10,000,000), and IT cyber/privacy insurance ($5,000,000-$20,000,000). ISBE (Minority, Women, and Disability Business Enterprise) participation rates serve as a scoring criterion in the evaluation process.

1 A “conflict of interest” occurs when private interests or relationships interfere in any way with the interests of the
State. Simply being a current vendor or a past vendor of the State does not in itself cause a conflict requiring preclusion. Additionally, formal responses to a Request for Information (RFI) issued by the Division of Purchases does not constitute “participation” or “consultation” with respect to a future RFP and does not disqualify a vendor from future participation from a subsequent RFP.
