About this file

This is a Subcontractor Summary Worksheet template issued by the City of St. Petersburg, Florida for use with state and local contract opportunities. The worksheet must be completed and submitted with bid submissions and serves to document all intended subcontractors for a project. Bidders are required to enter their company name as the prime contractor, identify the completing employee with date, and list all subcontractors with their complete legal names, primary business addresses, and project supervisors' contact information. The form requests the trade classifications being subcontracted, subcontract amounts, tier level (first or second tier only unless otherwise noted), and whether subcontractors are Small Business Enterprises (SBEs) or Minority/Women Business Enterprises (MWBEs). If no subcontractors will be used, bidders must provide a written statement to that effect.

Subcontractors may not be changed or substituted after award and the pre-construction meeting except under specific circumstances including failure to execute a contract, lack of appropriate licensing, bankruptcy or insolvency, ineligibility, failure to perform, substantially unsatisfactory work quality, or substantial delays to project progress. Any requested changes must be submitted in writing to the Procurement & Supply Management department with the Contracts Compliance Manager. SBE subcontractors used to meet SBE percentage requirements must be replaced with other SBE subcontractors unless the prime contractor demonstrates compliance through alternative means. The worksheet includes fields for tracking additions and deletions with corresponding reason codes and approvals by city personnel. Restrictions on subcontracting percentage and multiple-tier subcontractors do not apply to FDOT LAP or federally funded projects.
