About this file

This is a Subcontractor Summary Worksheet for the City of St. Petersburg related to a shelter bid project. The document serves as an instruction template and form that prime contractors must complete and submit with their bids for projects involving the City of St. Petersburg. The worksheet requires bidders to identify all intended subcontractors, provide their complete legal names and business addresses, list the specific trade classifications being subcontracted, and indicate subcontract amounts. For the shelter bid project referenced, up to 80 percent of the contract work may be subcontracted. The form mandates disclosure of subcontractor contact information including the name, phone number, and email of the employee responsible for project supervision, as well as designation of whether subcontractors are first or second tier.

Subcontractors cannot be changed or substituted after a prime contractor is awarded the project and the pre-construction meeting is held, except under specific circumstances including failure to execute a contract, lack of appropriate licensing, bankruptcy or insolvency, ineligibility, failure to perform, unsatisfactory work quality, or substantial delays. Any requests for subcontractor substitutions must be submitted in writing to the Procurement & Supply Management office. Small Business Enterprise (SBE) subcontractors used to meet SBE percentage requirements must be replaced with another SBE subcontractor if substitution becomes necessary, unless the prime contractor demonstrates SBE requirements are being met through alternative means. Restrictions on subcontracting percentages and multiple-tier subcontractors do not apply to FDOT LAP or federally funded projects.
