About this file
This is a Subcontractor Summary Spreadsheet form issued by the City of St. Petersburg, Florida, for use in competitive bidding on city projects. The form serves as a mandatory submission requirement with all bids and documents the subcontractors that prime contractors intend to employ on awarded projects. Prime contractors must complete the worksheet by entering their company name, the names and contact information of all intended subcontractors, the trades or services each subcontractor will provide, and the subcontract amounts. The form specifies that subcontractors may only be changed or substituted after contract award for specific documented reasons, including failure to execute a contract, lack of proper licensing, insolvency, unsatisfactory work performance, or project delays. The worksheet also requires identification of whether subcontractors are Small Business Enterprise (SBE) or Minority/Women Business Enterprise (MWBE) contractors, as SBE subcontractors used to meet percentage requirements must be replaced with other SBE contractors if substitution becomes necessary.
The spreadsheet includes fields for tracking whether subcontractors are first-tier or second-tier, with a note that only these two tiers are permitted unless otherwise specified in contract documents. Any requested changes to the subcontractor list after contract award must be submitted in writing to the City's Procurement & Supply Management office, specifically to the Contracts Compliance Manager. The form includes a legend documenting seven permissible reasons for subcontractor substitution and provides space to record the basis for any additions or deletions. Notably, subcontracting restrictions and multi-tier limitations do not apply to Florida Department of Transportation (FDOT) Local Agency Program (LAP) projects or federally funded initiatives.
