About this file

Summary

This is a Subcontractor Summary Worksheet instruction document from the City of St. Petersburg, Florida, required for bidding on the Computer Room HVAC Upgrade project. The document establishes mandatory procedures for all prime contractors to disclose and manage subcontractors on City projects. Prime contractors must complete and submit the worksheet with their bid, listing all intended subcontractors with their legal names, business addresses, contact information, primary employees responsible for supervision, trade classifications, and subcontract amounts. The worksheet specifies that only first and second tier subcontractors are permitted unless otherwise noted in contract documents, and the percentage of work allowable for subcontracting is project-specific.

The instructions establish strict controls over subcontractor substitution, permitting changes only for specified reasons including failure to execute contracts, lack of appropriate licensure, bankruptcy, ineligibility, performance failures, unsatisfactory work quality, or substantial project delays. For projects with Small Business Enterprise (SBE) requirements, any replaced SBE subcontractors must be replaced with other SBE subcontractors unless the prime contractor demonstrates SBE goals are being met through alternative means. The Computer Room HVAC Upgrade is identified as a sheltered bid requiring at least 20 percent of the contracting effort to be self-performed by the SBE prime contractor. Requests for subcontractor changes must be submitted in writing to the Contracts Compliance Manager. The worksheet includes provisions for tracking additions and deletions with reason codes and city approver authorization.
