About this file
This is a Subcontractor Summary Spreadsheet instruction document from the City of St. Petersburg used to manage subcontractor information for the Citywide Custom Uniform & Apparel Services procurement (IFB 26-208). The spreadsheet must be completed and submitted with all bids and requires contractors to identify all intended subcontractors, including their business addresses, contact information, trade classifications, and subcontract amounts. The document specifies that subcontractors may only be changed or substituted after award for specific documented reasons, including failure to execute contracts at specified prices, lack of appropriate licensing, bankruptcy or insolvency, ineligibility to perform work, failure to perform, substantially unsatisfactory work performance, or substantial project delays or disruption. For projects with Small Business Enterprise (SBE) participation requirements, any replacement of an SBE subcontractor must be with another SBE subcontractor unless the prime contractor demonstrates that SBE requirements are being met through alternative means. Restrictions on subcontracting percentages and multiple-tier subcontractors do not apply to FDOT LAP or federally funded projects.
The spreadsheet serves as a compliance and tracking tool requiring prime contractors to document the name of their company, the name and date of the employee completing the form, and detailed information for each subcontractor including whether they are first or second tier, their SBE or MWBE status, and the associated subcontract amounts. Changes to the subcontractor list after award must be submitted in writing to the City's Procurement & Supply Management office, specifically to the Contracts Compliance Manager at Stephanie.Swinson@stpete.org, with written justification supported by one of the seven specified reason codes. The document includes a legend detailing the allowable reasons for subcontractor additions or deletions, providing transparent criteria for post-award subcontractor modifications.
