About this file

This is a Past Performance Questionnaire template for solicitation SPRHA1-26-R-1003, designed to evaluate contractor performance on landing gear supply chain contracts. The questionnaire requests identifying information including contractor name, CAGE code, contract number, period of performance, and contract dollar amounts, along with a description of products or services performed.

The assessment evaluates contractors across three primary technical factors: Engineering (including expertise in landing gear items, quality deficiency resolution, and engineering oversight to manufacturers); Program Management (covering supply chain management, subcontractor performance, subcontract management, customer responsiveness, and risk management); and Order Fulfillment (addressing on-time delivery, logistics support, order processing, cost/price adherence, and financial management). A General Questions section also addresses organizational structure effectiveness and integration of teaming partners and subcontractors. Respondents rate performance using codes: A (Acceptable), U (Unacceptable), NA (Not Applicable), and UTA (Unable to Assess), with supporting narratives required for each response. Completed questionnaires should be submitted to Nathan Flinders at Nathan.flinders@dla.mil, with inquiries directed to Contracting Officer William Brown.
