About this file

This is an instruction sheet and template for submitting past performance information as part of a proposal response to solicitation SPRHA1-26-R-1003, a Draft RFP for Landing Gear Collaborative Supply Chain Integration II issued by the Defense Logistics Agency Aviation and the Air Force. The template does not address specific products or services required; rather, it provides guidance on documenting prior contract performance relevant to the instant acquisition.

Offerors must complete the Past Performance Information Sheet for each contract or program being submitted, providing details including company name and CAGE code, program title, contracting agency, contract number, contractor role (prime or subcontractor), contract type and pricing arrangement, original and current contract values with explanations for any changes, period of performance with original and current completion dates, modification history, primary customer points of contact (Program Manager/Technical POC, Contracting Officer, and Administrative POC), unique technical considerations, key personnel names and roles, a brief program description, and detailed relevancy statements demonstrating how performance on each submitted contract applies to the technical subfactors (Engineering, Program Management, and Order Fulfillment) outlined in the RFP's evaluation criteria. Submissions should be limited in number and length per Section L, Instruction to Offerors paragraph 4.4, and relevancy will be evaluated based on how closely the prior work aligns with the requirements and the specific role the submitting team member will perform on the proposed effort.
