About this file
This is a Request for Quotation (RFQ) specifications document issued by the West Virginia Department of Health and Human Resources, Bureau of Children and Families (WVDHHR BCF) for the Jackson County Office. The solicitation seeks bids for the rental of a digital copier (Band 6) and related accessories to support the agency's operational needs. The primary equipment required is a Komax Band 6 Bizhub 654e (65 PPM) or equivalent digital copier with base configuration capabilities including copy, print, scan, and fax functions connected to the network. Required accessories include a punch kit (2/3 holes), working table, 20 AMP surge protector, and toner supplies. The solicitation does not specify response deadlines, site visits, or bidder conferences, though vendors are instructed to complete pricing pages through the wvOASIS system or submit electronic documents. The contract term is based on a 48-month pricing calculation methodology.
The pricing evaluation requires vendors to list monthly rental rates for each item and calculate a 48-month total cost. Items included in the copier lease cost should be marked as "included" with zero cost in the pricing calculation. Maintenance and supplies up to 80,000 copies per month are included in the lease, with per-copy overages charged at $0.01 for copies exceeding 80,001 monthly. The contract will be awarded to the vendor offering the lowest overall grand total cost meeting all mandatory specifications. This procurement is partially or wholly federally funded, requiring all bidders to acknowledge compliance with federal procurement provisions. Vendor responsibilities include designating a primary contract manager available during normal business hours, managing facility access procedures if required, and absorbing all travel and mileage costs associated with contract performance.
