About this file

This document contains a Past Performance Questionnaire (PPQ) template for a federal contractor to submit with its proposal. The PPQ requests information about a prior contract for a Women's Health Clinic to be built at the Manchester VA Medical Center. Key details include the solicitation number, the agency issuing the solicitation as the Department of Veterans Affairs Technology Acquisition Center Austin, and a description of the opportunity as construction of the Women's Health Clinic. The PPQ template requires the contractor to provide details of the prior contract such as award dates, pricing, and performance ratings in areas like quality, schedule, communication, management, cost, and subcontracting. The federal agency evaluator would then assess the contractor's past performance by completing sections in the PPQ on the evaluator's information, ratings for the contractor's performance, and an overall assessment of whether they would hire or work with the contractor again.
