About this file

This is an inspection report document prepared for the Kenton County Airport Board (KCAB) concerning two 125,000-gallon underground glycol holding tanks located at South Glycol Pump Station II in Hebron, Kentucky. The file contains three separate inspection reports spanning from July 2018 to August 2023, documenting the structural integrity and condition of the East and West tanks. The initial inspection conducted by Genesis Environmental Solutions on July 24, 2018, determined that both tanks were in good structural condition with no major integrity issues. Tank 2B (West) required patching in the southwest corner where concrete had fallen away, while Tank 2A (East) contained standing water approximately 3-4 inches deep covering 50 percent of the floor surface, preventing complete seal inspection at floor-to-wall connections. A subsequent inspection by Corrosion Protection Resources on August 2-7, 2023, identified two repair areas in the West tank: a 2-inch repair where the boot bridged two precast joints on the south side at the east end, and a 12-inch repair at the third seam from the east end. The East tank was determined to require no repairs.

The August 2023 repair work on the West tank involved surface preparation using a braided wire wheel, cleaning with denatured alcohol, application of CIM surface conditioner, and patching with CIM trowel-grade material followed by re-sealing of joints. Both repair locations in the West tank were completed and the tank was deemed ready to return to service. These inspection reports serve as supporting documentation for the KCAB's procurement of Tank Inspections & Repairs & On-Call Services, with bids due by May 21, 2026, at 2:00 pm.

Re: Structural Integrity Inspection - (2) 125,000 Gallon Glycol Holding Tanks

On 24 July 2018, Genesis Environmental Solutions, Inc., (GES) performed an internal visual inspection on (2) 125,000-gallon underground storage tanks. This assessment / inspection was conducted Mr. Robert McChan American Petroleum Institute (API) Inspector Certification #37259, Steel Tank Institute (STI) SP001 Inspector Certification #AC11006.

West Tank (Tank 2B) Upon arrival, the tank hatch was opened, and ventilation was set up. The tank was ventilated and monitored to ensure a safe environment for entry following 29 CFR 1910.146 “Confined Space” entry procedures.

The inspector entered Tank 2B and conducted a visual inspection of the tank structure.

. Tank interior looking West Tank interior looking East

During this inspection, the inspector visually inspected all corner, base and section sealed seams.

. Floor to Side Section Seal Seam Sectional Seal Seam Corner Seal Seam

Once the seams and seam seals were inspected, the inspector closely examined the structural integrity of the concrete structure looking for indications of crack, spalling and any other imperfection that could potentially affect the tanks structural integrity. The walls and ceiling were closely examined with no indications of any deficiencies.

The floor was examined closely. There was some slight cracking throughout the floor surface. Upon close examination, this cracking appeared to be “crazing cracks”. This occurs if the top of the concrete slab dries too quickly, crazing cracks can form. These are very fine surface cracks that resemble spider webs or shattered glass.

Crazing cracks can be unsightly but are not a structural problem.

There is an area located in the Southwest corner of the tank that looks as though it has cracked and some of the concrete has fallen away. It has a crack running through it. Would recommend patching this are prior to use.

Area of concrete missing – slight crack running lengthwise

Other areas of the tank were visually inspected as well. The tank sump pit and piping, ladder rungs and access.

Overall Evaluation and Recommendations Tank 2B:

- The tank is in good condiction structually;

- There were no major integrity issues discovered;

- The single area in the Southwest corner needs patching;

- All rubber seam seals were intact with no visual defects noted;

East Tank (Tank 2A) After completion of the inspection of Tank 2B, personnel relocated to tank 2A and opened hatch access and ventilation was set up. The tank was ventilated and monitored to ensure a safe environment for entry following 29 CFR 1910.146 “Confined Space” entry procedures.

The inspector entered Tank 2A and immediately discovered that there was still significant water remaining. The water was approximately 3-4” deep in some areas and covered approximately 50% of the floor surface. The tank floor was somewhat elevated in the center. The inspector was able to continue the inspection but was unable to conduct the seal inspection at the tank floor to wall connection and corner connections.

During this inspection, the inspector visually inspected all corner, base and section sealed seams that were assessible and not covered in water.

Kenton County Airport Board South Glycol Pump Station II East & West 125 Tank Interior Inspection Report 8-2-2023.002

General:

The plan for the day is to inspect the General condition of the South Glycol Pump Station II East & West 125 tank Interiors.

With JSA and a meaningful discussion on safety complete, we are using a 5x gas air monitor to find the tank safe for entry.

In the WEST tank there were two repairs identified. The First was about 2” long where the boot bridges the two precast joints. (East end, South side last joint.)

Summary:

The EAST 125 tank is in good shape and no repairs are needed.

The WEST 125 tank has two spots that need repairs. The method would be to clean the location and abrade the mating surfaces. Then apply CIM surface conditioner and patch with CIM Trowel grade the re-seal the joint.

Please feel free to reach out with any questions.

Respectfully submitted.
