About this file
This is a Sensitive Security Information Acknowledgement Form issued by the City of Phoenix Aviation Department as Exhibit 6 to RFP 26-0007 for an Enterprise Asset Management software and implementation project. The form is a required compliance document that bidders and their authorized representatives must complete and sign to acknowledge receipt of documents containing Sensitive Security Information governed by 49 C.F.R. Parts 15 and 1520. The acknowledgement covers controlled aviation security documentation that may only be disclosed to individuals with a documented "need to know." Signatories agree to comply with federal regulations regarding the handling, use, and destruction of these sensitive materials, with destruction required upon the earlier of project completion or the contract end date. The form requires completion of specific fields including document titles, project name, contract end date, company name, authorized representative information, title, phone number, and complete address.
The acknowledgement form serves as a critical compliance mechanism for the five-year Enterprise Asset Management contract commencing May 1, 2026 (or July 1, 2026 per alternative language), with proposal responses due November 14, 2025. This document ensures that all bidding parties understand their legal obligations under federal security regulations and certifies their commitment to protecting sensitive aviation security information throughout the procurement and contract performance phases. Unauthorized disclosure of the controlled information may result in civil penalties or other enforcement actions under federal law. The form's inclusion as a required exhibit indicates that vendors will have access to sensitive security infrastructure details related to Phoenix's airport operations, making proper handling and safeguarding protocols essential components of the RFP compliance process.
