About this file
This document is a detailed guide to acceptable forms of identification for Personal Identity Verification (PIV) applicants, issued by the Office of Security and Hazardous Materials Safety (ASH). The document outlines specific requirements for identity proofing and registration, mandating that applicants must provide two original, non-expired documents, with one being a Federal or State-issued photo identification.
The identification requirements are categorized into primary and secondary forms, with specific documentation options for each category. Notably, effective May 7th, 2025, REAL-ID compliant identification will be required, which impacts driver's licenses and state-issued ID cards. The document provides comprehensive lists of acceptable primary and secondary identification documents, including passports, government-issued IDs, citizenship certificates, and other official documents. For individuals under 18 who cannot present standard documents, alternative options like school records, medical records, and day-care documents are permitted. This guidance is specifically linked to the FAA's William J. Hughes Technical Center's Center Operations and Maintenance Services (COMS) solicitation, ensuring contractors can meet identity verification standards for facility access.
