PWS Attachment 2

PREPARATION AND DECONTAMINATION OF THERAPY ROOMS

1.For release of room as unrestricted area: The Contractor shall monitor and decontaminate rooms used for inpatient and outpatient therapies as follows if the room is to be renovated or used for a non-therapy patient:
a.Monitor all areas of the therapy room and sanitary facility for contamination using an appropriate survey meter. Decontaminate the room to less than 200 dpm/100 cm2 beta/gamma and 20 dpm/100 cm2 alpha of removable contamination and less than 2 mrem/hour.
b.Place all potentially contaminated disposable items such as absorbent paper, vinyl cloth, plastic sheeting, and foodstuffs in plastic-lined MPW boxes labeled "Caution Radioactive Material." When each box is full, the Contractor shall seal it and affix a radioactive waste tag in accordance with procedures in the NIH Waste Guide (Attachment 10). The Contractor shall leave the MPW boxes in the therapy room and shall call the Radioactive Waste Service to arrange to have the boxes removed.
c.Collect used linens, place them in a cloth linen bag labeled "Caution Radioactive Material,” and leave the bag in the therapy room. Contact the Radioactive Waste Service to arrange for its removal.
d.Place non-disposable contaminated items such as the toilet seat, pillows, telephone, shower curtain in plastic bags labeled "Caution Radioactive Material," with the radionuclide and date listed.
e.Place contaminated items belonging to the patient in a plastic bag labeled "Caution Radioactive Material," with the patient's name, a description of the item(s) and the date.
f.Particularly for I-131 patient therapies, install an uncontaminated toilet seat in the therapy room and place uncontaminated pillows, telephone, and shower curtain in the therapy room. These items are either located in the designated storage room or can be obtained from the nurses.
g.Monitor all equipment remaining in the room, including IV bottles, unused bandages, syringes, and medications. The Contractor shall dispose of contaminated items of this type as radioactive MPW. For non-radioactive items, the Contractor shall inform the nurses that the items may be disposed as non-radioactive MPW.
h.Take a minimum of 30 swipes throughout the room to demonstrate that it has been decontaminated to less than 200 dpm/100 cm2 beta/gamma and 20 dpm/100 cm2 alpha of removable contamination. The Contractor shall prepare these swipes promptly and request expedited analysis by DRS. Notify the therapy HPs of the status of the therapy room as soon as the swipes have been submitted for analysis.
i.Secure the bagged, contaminated items in the designated storage room for decay-in-storage.
j.Monitor items in the storage room each month with a pancake GM, NaI or Zinc Sulfide detector and remove the labels from the plastic bags containing items that have decayed to background. For alpha therapy items, first monitor outside the bags with a NaI in a low background area and, if no counts, then open the bag to check with a ZnS meter. Notify the Area HP via e-mail when personal articles from patients have decayed to background levels.
k.Document the release for unrestricted use of the therapy room on the room clearance checklist (attachment 11), the room survey map with swipe results attached and the NIH 88-7 (attachment 12) form from the therapy room door. These forms shall be submitted to the NIH Contracting Officer’s Representative or designee within one workday of the swipe results being provided to the Contractor.
l.Lock the therapy room whenever it is unattended during the decontamination process.
m.Keep the therapy room posted with a "Caution Radioactive Materials" sign throughout the decontamination process. The Area HP shall be responsible for removing the "Caution Radioactive Material" sign after review and approval of the "Radiation Safety Procedures" (NIH 88-7) (attachment 12) submitted by the Contractor.
2.To prepare room for another therapy patient, the Contractor shall:
a.Place used linens in a cloth laundry bag labeled, “Caution Radioactive Material,” and leave the bag in the therapy room. Place absorbent pads on the bed, then make the bed, using fresh linens provided by the nurses on the therapy unit.
b.Place contaminated items belonging to the patient in a plastic bag labeled "Caution Radioactive Material," with the patient's name, a description of the item(s) and the date.
c.Place non-disposable contaminated items such as the toilet seat, pillows, telephone, and shower curtain in plastic bags labeled "Caution Radioactive Material," with a description of the item, the radionuclide, and the date.
d.Disinfect the sink, tub, and toilet.
e.Package radioactive waste (MPW) and contaminated linens in accordance with the NIH Waste Calendar, and contact the Radioactive Waste Service to schedule a waste pickup.
f.Decontaminate all areas (including sink and toilet) to less than 2,000 dpm/100 cm2 beta/gamma and 200 dpm/100 cm2 alpha of removable contamination.
g.Particularly for I-131 patient therapies, install an uncontaminated toilet seat in the therapy room and place uncontaminated pillows, telephone, and shower curtain in the therapy room.
h.If not already present, line the floor of the therapy room and bathroom with Herculite or other protective material. Place (or replace if already present) leak-proof absorbent paper on the floor and on the walls of the bathroom, anchoring it with duct tape.
i.Line the floor of the rest of the room with absorbent paper placed on top of the Herculite or other protective material. Anchor the absorbent paper with duct tape. If absorbent paper is already present, replace any portions that are stained, torn, or contaminated. Underlying Herculite or other protective material shall be left in place, but between patients, shall be decontaminated to less than 2000 dpm/100 cm2 beta/gamma and 200 dpm/100 cm2 alpha of removable contamination. Note that the extent of coverings may be reduced by the COR for certain radionuclide therapies.
j.Cover doorknobs, chairs, tables, TV control, and patient service panel with plastic. If these items are already covered with plastic, monitor the plastic and replace it if contaminated. Note that the extent of coverings may be reduced by the COR for certain radionuclide therapies.
k.Place a clean linen bag (available from nurses) in the room, and label it with "Caution Radioactive Material" tape.
l.Place several empty MPW boxes with plastic liners in them in the room; label them with "Caution Radioactive Material" tape.
m.Take a minimum of 15 swipes throughout the room to demonstrate that it has been decontaminated to less than 2000 dpm/100 cm2 beta/gamma and 200 dpm/100 cm2 alpha of removable contamination. The Contractor shall prepare these swipes immediately for expedited analysis by DRS and shall notify the therapy HP of the status of the therapy room.
n.Move the bagged, contaminated items to the designated storage room for decay-in-storage.
o.Monitor items in the storage room each month with a pancake GM, NaI or Zinc Sulfide detector and remove the labels from the plastic bags containing items that have decayed to background. For alpha therapy items, first monitor outside the bags with a NaI in a low background area and, if no counts, then open the bag to check with a ZnS meter. Notify the Area HP via e-mail when personal articles from patients have decayed to background levels.
p.Keep the therapy room posted "Caution Radioactive Materials" throughout the cleanup process.
q.Lock the therapy room whenever it is unattended during the decontamination/preparation process.
r.Document the decontamination of the therapy room on the therapy decontamination checklist (attachment 11), the therapy room survey map with swipe results attached, and the NIH 88-7 form (attachment 12) from the therapy room door. These forms shall be submitted to the NIH Contracting Officer’s Representative or designee within one workday of the swipe results being provided to the Contractor by DRS.
