About this file
This document is a template form for evaluating institutional capabilities and characteristics for participation in a United States Naval Community College (USNCC) consortium agreement.
The form is divided into two primary sections: Institutional Characteristics and Institutional Capabilities. Under Institutional Characteristics, institutions must provide basic information including degree levels offered, control/affiliation status, veteran/active-duty servicemember enrollment data, accreditation details, total student enrollment (full-time and part-time), online program enrollment numbers, and calendar system. For multi-institution submissions, institutions must indicate whether the submission is part of a larger agreement or system, identify centralized versus decentralized operational models, specify which institution awards degrees/certificates, and confirm coordinating institutions if applicable.
The Institutional Capabilities section requires institutions to demonstrate baseline and general capabilities across multiple dimensions. Baseline capabilities address modality (100% online and asynchronous instruction), admission policy (inclusive/non-selective), virtual labs capability, ability to award transfer credit for USNCC Naval Studies courses, four-year transfer pathways for both Associate of Science and Associate of Applied Science degrees, consortium agreement participation readiness, and technology infrastructure (SFTP file transfer, CSV export capability). General capabilities require documentation of program scaling ability, course section availability year-round, multiple course start dates, curriculum customization capability, certificate embedding and stand-alone options, professional accreditation status, industry certifications, military service credit via JST, ACE credit recommendations compliance, digital materials availability, material cost inclusion in tuition, dedicated USNCC student support staff, API file transfer capability, external system dependencies, identified technology integration staff, and executive-level attestation of technology requirement review.

INSTRUCTIONS: Respond to all questions and provide a brief explanation.
Questions Response
— Level of offering at the institution (Certificate, associate’s, bachelor's etc.)
— Institutional control or affiliation (as reported to IPEDS)
— Does the Institution serve veterans and active-duty service-members? If yes, include current campus and online enrollments for both.
— Is the institution accredited? If yes, include dates and accrediting body with contact information.
— How many students are currently enrolled at the institution overall (FT/PT)? How many students are currently enrolled in asynchronous, online programs?
— What is the institution’s calendar system (quarter, semester, etc.)
2 Multi-institution Characteristics
INSTRUCTIONS: Respond to the first question. If the answer is yes, respond to all questions.
QUESTIONS RESPONSE
Is the submission part of a larger, multi-institution agreement, program, district, or system?
IF THE ANSWER TO THE ABOVE IS NO – GO DIRECTLY TO PART 3
Will all program academic services and programmatic operations be centrally supported at the district/state level? If yes, then response should indicate the district/state and name which college will award degrees/certificates for each program area/degree/certificate identified.
Will one college be chosen to centrally coordinate all programmatic activities? If yes, then response should indicate coordinating institution and name which college will award degrees/certificates for each program area/degree/certificate identified.
Is the model decentralized such that shared academic services and operations are prohibited or not feasible? (Note: If yes, each college should submit a separate Form, under the auspices of the governing body, program, or consortium, outlining the programs areas that can be supported by that college.
INSTITUTIONAL CAPABILITIES
