About this file

This document is a Performance Assessment Questionnaire designed to evaluate a contractor's past performance for potential federal contract opportunities. The questionnaire provides a comprehensive evaluation framework with four confidence rating levels: Substantial Confidence, Satisfactory Confidence, Limited Confidence, and No Confidence, which assess various aspects of contractor performance including compliance with specifications, project management effectiveness, timeliness of deliverables, cost control, and customer satisfaction.

The questionnaire requires detailed input from both the contractor and a point of contact, capturing critical information such as contract identification details, contract type, total contract value, and specific performance ratings with mandatory rationale for each rating. The document is structured in three parts: contractor self-identification, performance evaluation by a reference/point of contact, and return instructions for submission to the Contracting Officer. While this specific questionnaire is not directly linked to a specific contract solicitation, it serves as a standardized tool for performance assessment that can be used in federal contract award decisions.

Work was accomplished with few, if any, minor problems for which corrective actions taken by the contractor were highly effective. The offeror has been highly successfully in performing the required effort.

Satisfactory Confidence: Performance meets contractual requirements and exceeds some requirements that benefit the end user. Work was accomplished with some minor problems for which corrective actions taken by the contractor were effective. The offeror has successfully performed the required effort.

Limited Confidence: Performance does not meet some contractual requirements. Serious problems with contractor performance were experienced for which the contractor has either not yet identified corrective actions or the corrective actions taken appear only marginally effective. The offeror has had little success performing the required effort.
