This checklist supplement contains the required information that is needed in order for NCO 1 to build the procurement package for Construction Projects (Excluding Design Build). Please complete this checklist and then attach all of the requested items as noted in the list.

Upon receipt of this checklist and the required documents, the assigned CO/CS will contact the COR via email to confirm receipt and to provide review comments. If necessary, a teleconference will be scheduled to discuss the procurement timeline. Please fill out all blocks listed below completely, verbage that is currently in the answer blocks does not need to stay upon completion of the information. If any of the answers are unknown or subject to change, please note this in the comments.

Contractor shall finish all work assoicated with 1 hydrant before moving onto another. Unforseen conditions are possible since this project requires excavation. Common issues encountered here during excavation include: Untracable utilities, Utilities in conflict with work to be performed which may or may not contain asbestos, contaminted or unsuitable soils.

Will there be any Government Furnished Equipment provided as part of this project? No If so, please identify what items and where the items can be found in the specs and/or drawings.

Was there a Life Cycle Cost Analysis done for this project? No If so, please provide a copy to PCAC.

Procurement Preference Is there a preference as to the Procurement Method used for this project? (i.e. IFB, RFP-Trade-Offs, RFP-LPTA) What is the justification for preferred method? Prior experience with Trade-Offs has shown we end up getting a better quality product and service without costing above the IGCE. These contractors will be visable to all on campus and are a easy target for scrutanty from a safety and traffic control prespective so we need someone who will work well with the campus and adhere to set schedules, OSHA standards, and professionalism.

Will the NTP be able to be issued after award of the Construction project or are there potential issues that will impact the start of the project?

Excavation in Augusta, ME isnt feasible until mid march - april due to frozen ground and snowbanks.

Will the site be able to handle the security/badging process or should PCAC work through the VSC for security/badging? Yes Are there any unique security/badging requirements mandated by the site? No
