About this file

This is a comprehensive safety specifications document containing detailed requirements for construction safety, including accident prevention plans, activity hazard analyses, and specific safety protocols. The document outlines mandatory requirements for the Site Safety and Health Officer (SSHO), safety training, personal protective equipment (PPE), inspections, mishap reporting, and specialized safety procedures for activities like electrical work, fire safety, excavation, and fall protection. Key requirements include having a full-time SSHO with professional safety certification and 3 years of construction safety experience, OSHA 30-hour Construction Safety training for competent persons, OSHA 10-hour training for all workers, daily safety inspections, and mandatory PPE including hard hats, safety glasses, appropriate safety shoes, hearing protection, and high-visibility apparel.

The document details specific protocols for medical facilities, including tuberculosis screening requirements, infection control measures, dust control procedures, and fire safety requirements. It mandates submission of an Accident Prevention Plan (APP) 15 days before the preconstruction safety conference, requires detailed Activity Hazard Analyses (AHAs) for all work activities, and establishes procedures for mishap reporting and investigation. The specifications emphasize coordination with VA facility safety managers, particularly regarding infection control risk assessments and interim life safety measures in occupied medical buildings.

3. Documentation shall combine indexing and navigation system with inspection-grade digital photography designed to capture actual conditions throughout construction and at critical milestones. Documentation shall be accessible on-line through use of an internet connection. Documentation shall allow for secure multiple-user access, simultaneously, on-line.

4. Before construction, the building pad, adjacent streets, roadways, parkways, driveways, curbs, sidewalks, landscaping, adjacent utilities and adjacent structures surrounding the building pad and site shall be documented. Overlapping photographic techniques shall be used to ensure maximum coverage. Indexing and navigation accomplished through interactive architectural drawings. If site work or pad preparation is extensive, this documentation may be required immediately before construction and at several pre-determined intervals before building work commences.
