About this file

This is a Past Performance Questionnaire template designed to evaluate contractor performance on federal contracts. The form is completed jointly by the contractor and the contracting client, with the contractor providing firm information (name, CAGE code, DUNS number, contact details) and contract details (contract number, type, award date, completion date, original and final prices), while the client completes evaluation sections and signs off on the assessment. The questionnaire requires contractors to identify their role (prime, subcontractor, joint venture) and explain the relevance of the past project to the current solicitation.

The evaluation framework assesses performance across seven categories using a six-point scale (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory, Not Available): Quality (technical data preparation, quality standards, problem resolution, quality assurance); Schedule/Timeliness (delivery compliance, resource utilization); Customer Satisfaction (end-user satisfaction, staff cooperation, customer interest); Management/Personnel/Labor (on-site management, workforce qualification, government property control, personnel expertise, small business utilization, multi-project management, change management, overall leadership); Cost/Financial Management (price compliance, cost reduction innovations, invoice timeliness for cost-type contracts, accounting system adequacy, terminations for default, financial stability); Safety/Security (safety environment maintenance, security compliance); and General (emergency response, contractual compliance, willingness to work with firm again, overall performance rating). Narrative explanations are required for any unsatisfactory or marginal ratings, with sections for identifying strengths, weaknesses, deficiencies, and additional comments to inform performance risk evaluation.
