3. REQUIREMENTS:

3.1 The following requirements shall be observed, in addition to the specific requirements of the Job Order, for work accomplished on naval ships and craft located at NNSY.

3.1.1 No regulatory agencies shall be allowed into the Shipyard without notification to the SUPERVISOR for submittal to NNSY Code 106 and approval by Code 106. No information concerning Navy property or operations, or operations performed under contract to the Navy, shall be released to regulatory agencies without the approval of the SUPERVISOR and NNSY Code 106.

3.1.2 All work will require completed Work Authorization Forms Prior to commencing work.

3.1.3 CRANE SAFETY REQUIREMENTS. Cranes rented or owned by the contractor shall meet the requirements of 2.2, 2.3 and all state and local regulations, and predetermined ground loading capacities. The contracting official will schedule an initial crane inspection with Code-720c to meet in a parking lot off of Effingham and Portsmouth Blvd. adjacent to the NNSY Pass office or Bldg. 1502 for crane validation prior to entering NNSY. Code-720c is a crane inspector from Lifting and Handling Department of the Crane Division Code-700.

Schedule an initial inspection with NNSY Code 742, via the SUPERVISOR, in the parking lot of building 1502, Pass Office, for crane validation prior to entering NNSY.

3.1.3.1 Submit one legible copy, in hard copy or approved

transferrable media, of a report during the initial inspection listing documentation supporting certification requirements, the names of each crane operator including qualifications on specific crane being utilized, and crane ground loading requirements to the SUPERVISOR.

3.1.3.2 Submit one legible copy, in hard copy or approved

transferrable media, of a report listing crane ground loading data calculated using crane weight, load (s) weight to be lifted, boom extension and angle, lift radius, and sufficient strength and size of matting materials to determine maximum load on outriggers to the SUPERVISOR, 72 hours prior to the initial inspection.

3.1.3.3 Maximum ground loading is obtained for the crane working area at NNSY from PWC Engineers Code-452 via the SUPERVISOR.

3.1.3.4 Submit one legible copy, in hard copy or approved

transferrable media, of a report required in 3.1.3.2 to the Code- 452 PWC Engineers via the SUPERVISOR not later than 24 hours before end of last shift, when initial crane inspections are scheduled after normal day shift working hours, on a weekend, or on a federal holiday.

3.1.3.5 Blockage of crane rails (within yellow travel zones)

must be coordinated with NNSY, Code 742, via the SUPERVISOR.

3.1.3.6 No weight handling equipment will travel, set-up, or

operate within 12 feet of the drydock walls at Numbers 3 and 4 drydocks. This includes mobile cranes, truck cranes, forklifts, JLGs, etc. No weight handling equipment will operate between the drydock wall and the yellow stripping for the inboard dock crane rail.

3.1.3.7 Contractors using subcontractors to perform any work

involving weight handling equipment will ensure their subcontractors' work practices follow and adhere to these guidelines.

3.1.3.8 Crane Accidents: Follow the guidance of 2.3

3.1.4 AERIAL PLATFORMS.

3.1.4.1 Accomplish a verification inspection of each aerial

platform rented or owned by the contractor prior to utilization on production work. Inspection can be performed at the job site.

Record inspection results for each aerial platform on NNSY form (Pre-entry Aerial Inspection Form). Submit a completed and signed copy of each form to NNSY Code 970.2, via the SUPERVISOR, by fax or delivery of hard copy. Upon submission, production work may proceed. The form will serve as notification to NNSY of the presence and location of the equipment and afford NNSY an opportunity to visually inspect and confirm the information received to ensure and maintain the safety and protection for its employees and Naval facilities. The contractor/vendor remains responsible for the safety, reliability, certifications and operation of the equipment, all operators, and other personnel affected.

3.1.4.2 Copies of a form for providing the above information

will be provided upon request by the SUPERVISOR. An asterisk on the form denotes items operator(s) shall inspect at job location after set-up of unit. Maintain a copy of the document with the unit. For aerial platforms delivered and/or requiring replacing after 1600, perform the verification inspection and submit the completed and signed form no later than 0800 hrs. the following day. Any aerial platforms requiring lifting at the job site by NNSY will require prior approval by NNSY Code 700. Aerial platforms equipped with lifting pads and an approved manufacturer's procedure/sketch for lifting in accordance with the requirements of ATTACHMENT A will be approved for lifting without any production delay. Aerial platforms not equipped with lifting pads with an approved manufacturer's procedure/sketch for lifting will require a minimum of one working day for Code 700 review/approval.

3.1.4.3 No aerial platforms will travel, set-up, or operate

within 12 feet of the drydock walls at Numbers 3 and 4 dry docks.

Do not operate between the drydock wall and the yellow stripping for the inboard dock crane rail.

3.1.4.4 Operators shall have available at the job site a copy of their certification of training for the specific unit being utilized. Provide for review upon request.

3.1.4.5 Perform daily pre-use inspections by the operator(s)

prior to assuming operating responsibility.

3.1.4.6 Report verbally each accident (i.e., personnel injury, material or equipment damage, collision or unplanned contact with objects) to the SUPERVISOR as soon as management becomes aware;

provide a formal written report of the event to NNSY via the SUPERVISOR within 24 hours of each accident, followed with a summary of circumstances, explanation of cause, and corrective actions taken. Secure the accident site, protect evidence, and withhold further aerial platform operations until the cause is determined and corrective actions are implemented and approved by the SUPERVISOR.

3.1.4.7 Ensure subcontractors follow and adhere to these

guidelines.

3.1.4.8 NNSY inspection team will be performing random

monitoring of aerial platforms while in use. Any unsafe operations/conditions observed/found will be questioned and may result in stoppage of work operations.

3.1.5 DIVING OPERATIONS.

3.1.5.1 Notify Port Operations within thirty minutes of divers entering the water. Submit one legible copy of the Port Operation notification to NNSY via the SUPERVISOR.

3.1.5.2 Ensure that the ship's "Code ALPHA" flag is up each day prior to and during all diving operations.

3.1.6 COORDINATION OF INCOMPATIBLE OPERATIONS.

3.1.6.1 Prior to start of work each day, coordinate all

potentially incompatible work operations with the NNSY Project Work Integration Leader and Ships Force via the SUPERVISOR. DO

NOT PROCEED WITH ANY WORK UNTIL INCOMPATIBILITIES ARE RESOLVED.

Operations that may be incompatible and may require coordination of scheduling prior to start up are: ventilation of toxic or flammable/combustibles; spray painting and paint application; use of solvents; hot work; tank work; fueling/defueling operations, C02 Fire Extinguisher System Work; asbestos work; mercury work;

energizing circuits and starting machine; working aloft; removing floor plates or gratings; diving operations; abrasive blasting;

compressed air (blow down); and, CHT (Sewage) work. Note: The above list is presented as a guide and is not all inclusive. The contractor is expected to be alert to the possibility of potentially incompatible operations, and resolve incompatibilities to ensure hazards are adequately controlled.

3.1.7 CONTRACTOR TRAILERS FOR OFFICE SPACE USE.

3.1.7.1 Contractor trailer temporary services (potable water, sewage disposal, telephone and electricity) shall be requested through the SUPERVISOR. NNSY will provide temporary services via pierside risers only. The contractor will provide, install, connect and maintain all temporary services between the NNSY pierside riser/location source to its trailer and /or conex box.

Remove services when directed by the SUPERVISOR.

3.1.7.2 Contractor trailers location other than on-board ship must have the NNSY Fire Department and NNSY Code 300.2 approval.

3.1.7.3 Trailers shall meet Federal, State, and Local building and environmental requirements, (i.e., lighting, ventilation, heating/cooling, safety, etc.).

3.1.7.4 All manned office trailers and conex boxes located on the ship will be equipped with an automatic or manual internal sprinkler system designed to provide 0.1 GPM per square foot of floor area and an audible alarm that will sound when the sprinkler system is activated. In addition, two fully charged 15 pound CO2 wall-mounted fire extinguishers shall be provided in a convenient location in each manned office trailer and conex box.

3.1.7.5 Smoke alarms, approved by Underwriter's Laboratory, shall be installed in enclosures and shall be audible outside the enclosures.

3.1.8 NO SMOKING AREAS.

3.1.8.1 All Naval vessels shall be designated NO SMOKING AREAS unless otherwise designated by the vessel's Commanding Officer.

3.1.9 TEMPORARY COVERS.

3.1.9.1 Template, fabricate and install temporary top hats on each tank top manhole for tanks and voids that open into bilges of machinery spaces.

3.1.9.2 Fabricate top hats from 1/4 inch thick aluminum.

3.1.9.3 Top hats shall not exceed 12 inches in height.

3.1.9.4 Install each top hat using new 3/16 inch thick gaskets

conforming to MIL-C-6183, Class I, grade firm, synthetic rubber and cork composition. Install top hats using the existing fasteners; replace 25 per cent of fasteners found missing or unserviceable.

3.2 Prior to loading materials, tools and equipment aboard ship, the contractor shall install identification tags consisting of contractors name, description and destination for each item. Tags must endure repair process.

3.3 For temporary services requested by the contractor, the contractor is responsible for possessing the necessary materials to connect hookups at locations pierside, provided by NNSY. The contractor is responsible to run services from the NNSY pier side location to their worksite, shipboard or pierside. The connections used by the contractor must be capable of adapting to the following shipyard temporary service connections:

3.3.1 Temporary services will be provided from pierside riser connections;

3.3.1.1 NNSY will only supply steam to the 2 inch diameter

line/connection.

3.3.1.2 NNSY will only supply Electric 440V 3 Phase and/or 110V single phase.

3.3.1.3 NNSY will only supply potable water 2 1/2 inch diameter.

3.3.1.4 NNSY will only supply L.P. Air 2 inch diameter.

3.3.2 The contractor will not be allowed to use electric air

compressors in NNSY. Only diesel air compressors are allowed.

3.3.3 All temporary services shall be tagged at the source and destination stating; use, source, destination and contractors name.

Identification tags should also be used at bulkhead penetrations.

Quick disconnects shall be used when necessary within 3 feet of a safety bulkhead if the services penetrate the bulkhead. All waivers shall be requested through the SUPERVISOR and Project Temporary Service Manager (PTSM).

3.4 All scaffolding being installed for NNSY's use shall be inspected and approved for use in accordance with 2.4. This requirement is for scaffolding installed by the contractor for NNSY's utilization.

3.5 The contractor will provide a completed copy of ATTACHMENT B to the SUPERVISOR after all repair, testing and inspection are completed and accepted by Ships Force, MARMC and NNSY for each Work Item in the Job Order.
