AMENDMENT 0005

Pre-Proposal Inquiries & Responses

54. Does the government expect us to submit the last 5 years' worth of OSHA 300A forms for all the sites where we have provided services?

Response: OSHA 300A Forms shall be completed in accordance with the Department of Labor requirements and provided as required by the RFP.

55. Should we consolidate all the site data into one comprehensive OSHA 300A form for submission?

56. Alternatively, is it sufficient to submit only the OSHA 300A forms associated with the contracts referenced in our technical?

57. Define the timeline for "routine" in regards to pre-emergent care and post emergent herbicide application as mentioned under Grounds Maintenance for COL 1.

Response: The timeline is determined by the Contractor's schedule that provides the level of care IAW the associated COLS/instructions.

58. Can we be provided a detailed drawing/ sketch or aerial photo of the base areas outlining the building/ facilities and all other areas defined as Primary, Secondary, and Tertiary that the contract covers. Not only for the base but for Cranny Island Fueling Station as well.

Response: Maps will only be provided to the awardee.

59. In looking at attachment J-0200000-07 A012 CRNMA (UIC N6268) Chambers Field grounds A012AA - Herbicide Glyphosate Treatment Spraying services and A012AB - Wetland Seeding Mix Services, it appears that this area may be wetlands with Phragmites that the government is working to eliminate and establish Wetland Grass in its place. Do these wetlands require any mowing? If these areas do require mowing, does the mowing equipment need to meet the low PSI requirement of equipment working in wetlands?

Response: Refer to Section C, Annex 1503050 - Grounds Maintenance and Landscaping, Spec Item 3.1.13 Phagmite Control (Chambers Field).

Can this historical data be provided?

Response: The Government does not maintain records on the frequency that services were provided. All areas are maintained IAW its associated COLS/instructions.

63. Site Maps 150305-04 - Not having site maps pre- proposal severally limits the ability of a contractor to competitively price this opportunity and makes an unfair advantage to the incumbent. These maps help the contractor to determine the size and types of equipment needed to perform the work, having the just acreage is not sufficient and not having the site maps did allow us to evaluate the areas during the site visits. Can site maps be posted pre-proposal showing the areas where the different COLS, semi-improved areas and un-improved areas are to be performed?

64. Are there any storage spaces provided by the government or will the contractor need to provide containers for storage of their equipment and supplies?
