Quality Assurance Surveillance Plan (QASP)
Contract N00024-15-R-6401
22 April 2014
1.0 PURPOSE
This Quality Assurance Surveillance Plan (QASP) defines the Government’s approach to monitoring, managing, and executing appropriate action with regard to Contractor performance against contractual objectives. The QASP establishes the methods used to measure the effectiveness of the Contractor's Quality Program Plan (QPP) described in Contract Data
Requirements List (CDRL) A023, provides delivery criteria with thresholds, and reduces the risk of nonconforming material being delivered to the Fleet.
This document establishes surveillance and quality performance criteria which demonstrate that
MK48 Mod 7 Afterbody/Tailcone (AB/TC) Sections not only pass the individual Factory
Acceptance Test (FAT) process, but also are a product of a controlled quality process. Factory delivery or non-delivery status is established based on compliance with the criteria detailed in this QASP. MK48 Mod 7 AB/TC Sections will only be presented for sell-off while the factory is in delivery status. Acceptance of the MK48 Mod 7 AB/TC Sections shall be made by the
Procuring Contracting Officer (PCO) as detailed in Section E of the Contract.
2.0 SCOPE
The QASP provides Government actionable criteria to ensure that the Contractor's quality control efforts are timely, effective and are delivering the results specified in the contract. The
QASP is not intended to duplicate the Contractor's QPP, but instead is provided to support the
Contractor's efforts in developing a QPP which ensures the QASP criteria are met.
3.0 AUTHORITY
Authority for issuance of this QASP is provided under Federal Acquisition Regulations (FAR)
46.401 and Contract Section E - Inspection and Acceptance.
4.0 ROLES AND RESPONSIBILITIES
Procuring Contracting Officer (PCO) – The PCO ensures performance of all necessary actions for effective contracting; ensures compliance with the terms of the contract; and safeguards the interests of the United States in the contractual relationship. It is the PCO that ensures the
Contractor receives impartial, fair, and equitable treatment under the contract. The PCO is ultimately responsible for the final determination of the adequacy of the Contractor's performance.
Contracting Officer Representative (COR) – The COR is responsible for technical administration of the project and ensures proper Government surveillance of the Contractor's performance. The
COR is not empowered to make any contractual commitments or to authorize any contractual changes on the Government's behalf. Any changes that the Contractor deems may affect contract price, terms, or conditions shall be referred to the PCO for action.
5.0 METHODS OF SURVEILLANCE
The primary means of surveillance for the criteria outlined in this QASP is through CDRL deliverables. The Government, at its discretion, may independently verify the provided information using available Government resources in accordance with the contract and FAR requirements.
5.1 Performance of Surveillance
The Government representative(s) will monitor performance and review performance reports and
CDRLs furnished by the Contractor to determine how the Contractor is performing against communicated performance objectives. The contractor is responsible for making required changes in processes and practices to ensure performance is managed effectively against the QPP
(CDRL A023), Statement of Work, and the thresholds described in Table 1.
5.2 Surveillance Level Adjustments
The COR and PCO may periodically review the QASP to determine if changes in surveillance levels or methods are necessary and update the QASP accordingly. When a change in the
Contractor’s required work (e.g., a change of design) or a change in the Contractor’s performance results in a change of program risk, surveillance changes may be considered.
6.0 DEFINITIONS
Nonconformance (NC) – A defect; a unit of product that departs from a specified requirement with respect to one or more features, dimensions, properties, or parameters that may affect subsequent operations, form, fit, or function. A nonconformance may be detected by any process, inspection, or test.
Key Quality Indicator (KQI) –Process indicator or other quantitative measurement criteria with defined thresholds upon which delivery of supplies is based. KQIs provide indication of the expected quality level at subsequent tests or inspections.
Functional Item Replacement (FIR) – Items which are typically installed or replaced as subassemblies during torpedo turnaround at an Intermediate Maintenance Activity (IMA) and can be independently tested prior to installation.
Category I NCs – Category I NCs are CDRL A032 Nonconformances found during CDRL A030 testing at the FIR or subassembly level.
Category II NCs – Category II NCs are CDRL A032 Nonconformances found during CDRL
A030 testing at the section level.
Category III NCs – Those NCs found during Government assembly, disassembly, in-water test, environmental test, or any other Government test during the Conditional Acceptance period, as defined in Section E of the contract.
7.0 DELIVERY CRITERIA, KQI USAGE and THRESHOLDS
All KQIs established in this QASP are based on nonconformances as documented by CDRL
A032 (Failure Reporting, Analysis, and Corrective Action (FRACA) Reports). The KQIs are categorized by the test phase where the nonconformance is found. For KQI thresholds, the tested quantity and the allowable number of open nonconformances are assessed against each production year’s material. For example, tested quantity and Category I, II, or III NCs generated while producing and testing CLIN 0102 are not applicable to CLIN 0202 in terms of KQI thresholds. For a graphical representation of the KQIs, see Figure 1.
Figure 1: KQI Graphical Representation
7.1 Category I Nonconformances; FIR/Subassembly Factory Acceptance Test KQIs
Allowable quantities of open Category I NCs are dependent on the number of FIR and subassembly level factory acceptance tests completed. Category I NCs are CDRL A032
Nonconformances attributed to the below bulleted items found during CDRL A030 testing at the
FIR or subassembly level. Table 1 shows the correlation between the allowable number of open
NCs and tested quantities.
Tailcone Assembly (8243661)
Regulator Assembly (5894200)
Coolant Pump and Head Assembly (6935010)
Hydraulic System (VARIOUS)
Fuel Delivery System (6935014)
Alternator (6935009)
Engine Assembly (7052739)
Lube Oil Pump (2074259)
Steering Assembly (7052749)
Propulsor (VARIOUS)
Type II Cables (VARIOUS)
Slide Valve Assembly (2507318)
7.2 Category II Nonconformances; Section Level Factory Acceptance Test KQIs
Allowable quantities of open Category II NCs are dependent upon the total number of AB/TC section level Factory Acceptance Tests completed. Category II NCs are CDRL A032
Nonconformances found during CDRL A030 testing at the section level. Table 1 shows the correlation between the allowable number of open NCs and tested quantities.
7.3 Category III Nonconformances; Post-delivery
Allowable quantities of open Category III NCs are dependent upon the total number of AB/TC
Sections that have been tested by the Government, delivered under CLINs 0002, 0102, 0202, 0302, and/or 0402. Table 1 shows the correlation between the allowable number of open NCs and tested quantities. Tested quantity for Category III is defined by number of deliveries which have completed an in-water test series (pre-range CST, In-water, post range CST) or Periodic test sequence.
7.4 KQI Delivery Suspension Thresholds
Exceeding the KQI thresholds defined in this QASP may be used as cause for suspension of production deliveries, as detailed in Section 3.3.13.2 of the Contract’s Section C, Statement of
Work (SOW). Table 1 shows the correlation between allowable open NCs and tested/delivered quantities.
