About this file
This is a Past Performance Questionnaire template issued by the Marine Corps Systems Command (MCSC), Product Manager for Maintenance Supports Systems (PdM MSS), designed to collect performance history information on contractors for source selection purposes.
The questionnaire evaluates contractor performance on the Tactical Radio Intermediate Test System (TRITS) contract, which requires the manufacture, integration, testing, production, and delivery of TRITS hardware, computer software, displays, interfaces, buses, connectors, cabling, and transport casing, along with associated logistics and technical documentation. The contractor must provide program management, engineering, test and evaluation, and logistics support to meet delivery schedules and performance requirements. The form collects basic contract information including contractor name and address, contract number, award date, completion date, contract value, and contract type, along with evaluator identification. Evaluators assess contractor performance across six categories: Quality of Support Services Provided, Responsiveness to Government Requests/Concerns, Schedule Compliance for Services and Products, Technical Capabilities and Qualifications of Contractor Workforce, Customer Satisfaction/Business Relations, and Overall Vendor Performance Rating. Each category uses standardized performance ratings: Outstanding, Exceeds Expectations, Meets Expectations, Unsatisfactory, or Not Applicable. The form also includes a recommendation section for future work with options ranging from Highly Recommended to Not Recommended. Completed questionnaires should be sent to Mr. Mark Denhup at Marine Corps Systems Command/Code CT2.5, 2200 Lester Street, Quantico, VA 22134, or via email at mark.denhup2@usmc.mil. The information is classified as Source Selection Sensitive and restricted to DoD Community distribution.
