About this file
This is a Past Performance Questionnaire issued by the Marine Corps Systems Command (MCSC), Product Manager for Maintenance Supports Systems (PdM MSS), to be completed by government evaluators assessing contractor performance on related contracts.
The questionnaire evaluates contractor performance on efforts to manufacture, integrate, test, produce, provide, and deliver the Tactical Radio Intermediate Test System (TRITS) and associated logistics and technical information. The capability includes necessary hardware, computer software, display, interfaces, buses, connectors, cabling, and transport casing, with the contractor responsible for program management, engineering, test and evaluation, and logistics support to ensure delivery schedules, performance requirements, and supportability are met. The form requires completion of required contract information fields (Contractor Name & Address, Contract Number, Award Date, Completion Date, Contract Value, and Type of Contract) and documentation of whether cure notices or show cause notices were issued. Evaluators must rate contractor performance across six dimensions—Quality of Support Services Provided, Responsiveness to Government Requests/Concerns, Schedule Compliance for Services and Products, Technical Capabilities and Qualifications of Contractor Workforce, Customer Satisfaction/Business Management Relations, and Overall Rating for Vendor Performance—using standardized definitions (Outstanding, Exceeds Expectations, Meets Expectations, or Unsatisfactory). The form also includes a recommendation section for future work (Highly Recommended, Recommended, Neutral, Recommended with Qualifications, or Not Recommended) and requires indication of whether findings have been previously shared with the contractor. Completed questionnaires should be submitted to Mr. Mark Denhup at mark.denhup2@usmc.mil or mailed to Marine Corps Systems Command/Code CT2.5, Quantico, VA 22134.
