Past Performance Questionnaire Template
To: Contractor References (Assessor):
The United States Marine Corps is in the process of selecting contractors for award under the Marine Corps Prepositioning Program (MCPP) Services Contract. In general terms, this contract shall provide for long-term logistics support services for the United States Marine Corps’ three strategic crisis response combat capability sets (Maritime Prepositioning Ships (MPS) Program, the Marine Corps Prepositioning Program Norway (MCPP-N), and the Marine Expeditionary Unit Augmentation Program – Kuwait (MAP-K)). The requirement calls for the full spectrum of logistics services on over 36 million pieces of prepositioned equipment and supplies that consists of everything that a Marine combat unit requires to fight and sustain itself for 30 days of expeditionary operations. The dynamic complexity of the effort is compounded by the sheer volume of the materiel and exceedingly tight turnaround time lines. The length of this contract will be eight years. The Government wants to be confident that the contractors selected will be able to successfully accomplish our requirements.
One of the ways the Government intends to evaluate each offeror is through the verification of the offeror's past and present performance on other contracts. For this, we must depend on information received from agencies such as yours, which have had firsthand experience with the offeror.
We have tried to make our questionnaire as quick and simple to complete as possible. General contractor information was provided by the offeror in Part I. Please answer the questions that follow in Part II. We would greatly appreciate your comments describing why you rated a particular question the way you did. We ask that you complete and return by scanning and emailing to nicholas.vonbargen@usmc.mil no later than 25 June 2018.
Please do not return or e-mail the questionnaires back to the offeror or contractor. Completed questionnaires are source selection information and must be protected. They will not be released to the offeror.

Consent Letter (Example Format for Past Performance Consent Letter)
Past performance information concerning subcontractors and teaming partners cannot be disclosed to an Offeror without the subcontractor's or teaming partner's consent. In an effort to assist the Government's Performance Evaluation Team (PET) in assessing your past performance relevancy and confidence, we request that the following consent form be completed by the subcontractors identified in your proposal. The completed consent forms shall be submitted as part of your Past Performance Volume.
