1 System Safety Group (SSG) Requirements
Department of Defense Directive (DoDD) 5000.1, The Defense Acquisition System, and Department of Defense Instruction (DoDI) 5000.02, Operation of the Defense Acquisition System, identifies the DoD Systems Engineering approach to eliminating hazards, where possible, and minimizing risks, where those hazards cannot be eliminated.
Air Force Instruction (AFI) 91-202 (Air Force Space Command (AFSPC) Supplement (Supp)), The United States Air Force (USAF) Mishap Prevention Program, requires a system safety program be established in accordance with Military Standard (MIL-STD) -882E, DoD Standard Practice System Safety. As part of the system safety program, AFI 91-202 (AFSPC Supp) establishes all space programs, regardless of acquisition category, are required to conduct SSGs to oversee the system safety program throughout the life of a system. The Space and Missile Systems Center Instruction (SMCI) 63-1205, The SMC System Safety Program, applies system safety throughout the program life cycle of all SMC acquisition programs and projects (existing and future SMC space systems) that involve design, development, modification, evaluation, demonstration, testing, operation and disposal. MIL-STD-882E, defines system safety as “the application of engineering and management principles, criteria, and techniques to achieve acceptable mishap risk, within the constraints of operational effectiveness and suitability, time, and cost, throughout all phases of the system life cycle.”
The LTRS SSG is structured based on definitions provided by AFI 91-202 (AFSPC Supp) and SMCI 63-1205.
1.1 SSG Purpose and Objective
The purpose of the LTRS SSG is to assure engineering and management principles, criteria, and techniques are applied to optimize all aspects of safety within the constraints of operational effectiveness, schedule and cost throughout all phases of the system lifecycle.
The objective of the LTRS SSG is to integrate sound Environmental, Safety and Occupational Health (ESOH) management practices into all systems engineering functions of the program. The ESOH process covers planning, designing, implementation, operations and disposal for all LTRS phases and segments. The LTRS SSG also conducts research into ESOH conditions, resources and legal obligations, and advises the Program Manager (PM) and the LTRS Chief Engineer in regard to methods, actions, and mitigations required for the best control of hazards.
1.2 SSG Scope
The LTRS SSG will collect and cross feed user inputs and insights into the LTRS System Safety efforts. As necessary, the SSG will modify its membership with optional members and/or advisors to properly address complex system safety activities
1.3 References
The following identifies requirements for LTRS SSG, and if established, the System Safety Working Group (SSWG) activities:

1.4 SSG Responsibilities
The RN Program Director or his/her alternate, is the central point-of-contact for all LTRS SSG matters and has appointed the LTRS Chief Engineer as the SSG Chairman. The LTRS SSG analyzes documented safety and health hazards associated with LTRS procurement, operation, maintenance and disposal.
Each SSG meeting will address the following topics at minimum:
- Program status.
- Action item summary including action agencies and suspense dates for old and new action items.
- Analyses of major safety design trade-offs and modifications
- Status of planned, pending, active and disapproved safety modifications.
- Safety investigation recommendations and High Accident Potential (HAP) reports.
- User/operator issues.
- Safety risk mitigation options
- Unmitigated hazards.
- System Safety program scope, including contractual requirements and deliverable System Safety data.
- Overall safety assessments and association with milestone reviews.
- Major modifications or engineering change proposals (ECPs).
- Establishment of SSWGs to address system safety issues.
- Safety recommendations during design, development, test, operations, sustainment and disposal.
- Assigning mishap risk indices to each SSG discussion and action item.
1.5 Meetings and Decision Making
SSGs meet at least annually as scheduled by the chair. Attendance of mandatory members or their designated representatives is required. In addition, any member of the SSG may request the chair call a meeting. Meetings may be waived with concurrence of all required attendees. All mandatory members must commit to supporting SSG meetings and must sign the charter.
The SSG chair will be the approval authority for all SSG recommended decisions. SSG minutes will be prepared, approved by the SSG chair and distributed electronically within 30 days.
SSG recommendations that require capability changes or materiel modifications by the program office must be validated and documented IAW the process and criteria outlined in AFI 10-601.
The SSG charter will be updated as needed to reflect the most current information, i.e., scope, requirements and membership. Membership updates do not require SSG charter coordination.
1.6 SSG Membership
The LTRS SSG membership is based on guidance provided in AFI 91-202 and SMCI 63-1205. The membership for the version of this SSG charter follows:
Mandatory members are:

Current functional organizational membership is listed in Appendix A.
2 System Safety Working Group (SSWG)
If established, the objective of the LTRS SSWG is to investigate and report on specific safety issues; to advise the SSG chairperson of the status of system safety program activities being accomplished; and to ensure the best methods, actions, and mitigation activities are being implemented to control identified hazards.
The SSG is responsible for identifying and establishing SSWGs as necessary to work detailed system safety issues. The SSG chairperson may establish short term SSWGs to address specific system safety issues as necessary. The SSG should assist the SSM to provide additional expertise in the establishment of necessary SSWGs. The SSWG is chaired by the SSM and does not generally require the attendance of the Systems Director or PM or Project Support Manager (PSM).
If established, the LTRS SSWG will meet at least once quarterly, and can be established at any time by the SSG Chairperson for the purpose of evaluating and providing recommendations to the SSG on specific safety issues. Membership of the SSWG will consist of selected SSG members plus selected technical advisors as deemed necessary by the SSG Chairperson. The findings of the SSWG will be reported to the SSG Chairperson and reviewed at the subsequent SSG.
Typical SSWG activities include:
Assessing the status of safety activities in the total system, various system segments, elements, subsystems and components. Hazards and their mitigations are reviewed and disposed as follows:
- Ill-defined hazards are returned to the originator for clarification
- Valid hazards that have been completely mitigated are recorded; a residual risk acceptance form is generated and presented to the appropriate risk acceptance authority for signature, and monitored
- Valid hazards that have been partially mitigated are documented, assigned to an action officer to be monitored
- Non-valid hazards are documented and archived
The SSM shall ensure minutes of meetings are prepared and distributed to members and attendees within 30 days after the date of the meeting. System safety reviews, SSG/SSWG meeting minutes and audit/inspection results will be written, distributed and stored in the system safety library.
3 Charter Approval and Revision
Approval of this Charter requires approval by all mandatory SSG members. The Charter will be developed and coordinated with the SSM and the SMC/SE System Safety Chief (also known as the CSSM). The approved Charter applies throughout the LTRS program life cycle.
