About this file

This document is an Infection Control Risk Assessment (ICRA) permit template used for construction and maintenance activities in healthcare facilities. The form outlines four types of work classifications (A through D) and four risk levels (Low, Medium, High, and Highest) with corresponding control measures for each class.

The document provides detailed requirements for different classes of work, from Class I (minimal dust generation) to Class IV (major construction), including specific protocols for dust control, HVAC isolation, barrier installation, and cleaning procedures. It includes a comprehensive matrix showing how project types and risk groups determine the required class of infection control measures. The risk groups are clearly defined, ranging from low-risk areas like offices and storage spaces to highest-risk areas such as operating rooms and sterile areas. Project types are categorized from Type A (non-invasive activities) to Type D (major demolition and construction projects), with specific examples and requirements for each category. This ICRA permit is specifically referenced in the related federal contract opportunity for HVAC and isolation room work at the Phoenix Indian Medical Center, where infection control mitigation will be required.

CLASS IV 1 Obtain Infection Control permit prior to start of construction.

2 Isolate HVAC system in work area to prevent contamination of duct system.

3 Complete all critical barriers or use control cube method prior to start of construction.

4 Maintain negative air pressure within work site – utilizing HEPA-equipped air filtration units.

5 Seal holes, pipes, conduits, and punctures.

6 Construct anteroom and require all personnel to pass through this room so they can be vacuumed off using a HEPA vacuum cleaner before leaving work site, or wear protective paper coveralls (bunny suit) that is removed each time leaving the work site.

7 Require all personnel entering work site to wear shoe covers.

1 Do not remove barriers from work site until project is checked by Infection Control; and is thoroughly cleaned by environmental services.

2 Vacuum work area with HEPA-filtered vacuums.

3 Wet mop with hospital–approved disinfectant.
