About this file
This is a Past Performance Questionnaire template for solicitation AOCSSB26R0048, an Indefinite Delivery Indefinite Quantity (IDIQ) contract for Elevator, Escalator, and Accessible Lifts Maintenance, Repair, Inspection, and Modernization Services with the Architect of the Capitol (AOC). The questionnaire is issued to previous customers of prospective contractors to evaluate their past performance as part of the source selection process. Responses must be submitted directly to the Contracting Officer, Roman Davydov, at roman.davydov@aoc.gov by the SF33 Due Date specified in the solicitation.
The questionnaire requests ratings and supporting comments across 16 evaluation areas, including contractor performance on delivery schedules, quality control, quality of work, pricing reasonableness, labor and safety compliance, cure notices or terminations, customer satisfaction, team relationships, on-site management, corporate integrity, contract modifications, and an overall rating. Evaluators must assign one of five confidence ratings to each question: Substantial Confidence, Satisfactory Confidence, Limited Confidence, No Confidence, or Unknown Confidence (Neutral). For Limited Confidence or No Confidence ratings, the questionnaire requires specific explanations of performance deficiencies. The template serves as a standardized tool to gather objective information regarding a contractor's performance history for use in the competitive evaluation process.
