About this file
This is a Past Performance Questionnaire template for evaluating contractor performance on a previous project relevant to the Senate Food and Related Services solicitation (AOCSSB26R0002) issued by the Architect of the Capitol. The questionnaire is to be completed by previous customers or clients of a contractor bidding on the solicitation and submitted directly to the AOC Contracting Officer, Moyah Wilson, and Andrew Eichhorn by the proposal submission deadline of Wednesday, May 20, 2026.
The questionnaire requests evaluations across 16 performance categories using a four-point rating scale (Outstanding, Satisfactory, Marginal, Unsatisfactory), covering delivery schedules, quality control, work quality, pricing reasonableness, labor and safety compliance, corrective actions, contract modifications, overall corporate management, and customer satisfaction. Respondents are instructed to provide supporting details for ratings of Outstanding or Unsatisfactory performance and to explain any negative ratings. The responses are designated as Source Selection Sensitive information and are protected under FAR 42.1503, restricting distribution to government personnel involved in source selection evaluation only. This questionnaire is a standard evaluation document used by AOC to assess the past performance of companies submitting proposals for federal contracting opportunities.
