About this file
This document is an affidavit required by the City of Milton, Florida, for the Directional Boring Services contract (ITB 2026.05, Contract No. PW-DB-2026-01). The affidavit serves as Exhibit F to the bidding documents and requires the bidder's authorized representative to swear under oath that neither the bidder nor any of its affiliates appear on the State of Florida's Convicted Vendor List following conviction for a public entity crime as defined in Florida Statutes Section 287.133. The affidavit must be completed and notarized prior to bid submission, with the notary certifying the signatory's identity through either physical presence or online notarization. The document includes blanks for the bidder's name, authorized signatory information, and notary credentials.
Submission of a false sworn statement in this affidavit may result in bid rejection, contract termination, and any other remedies available under applicable law. The affidavit constitutes a material representation of fact upon which the City will rely during bid evaluation. This document is a mandatory compliance requirement for all bidders responding to the Directional Boring Services solicitation, which seeks contractors to provide horizontal directional drilling services for the installation, extension, maintenance, and repair of underground utility infrastructure (water, sewer, and natural gas systems) within the City of Milton and Santa Rosa County on an as-needed, work-order basis.
