EXHIBIT C - APPLICATION FOR PAYMENT AND INVOICE REQUIREMENTS
This document is an exhibit to the Directional Boring Services Contract (PW-DB-2026-01) issued by the City of Milton, Florida, establishing the requirements and procedures for contractor invoicing and payment. The City of Milton seeks horizontal directional drilling (HDD) services for the installation, extension, maintenance, and repair of underground utility infrastructure including water, sewer, and natural gas systems on an as-needed, work-order basis within Milton and Santa Rosa County. The contractor must submit invoices only after work is fully completed, inspected, and accepted in writing by the City, with all required documentation submitted. Invoices must include minimum information such as contractor name and remittance address, invoice and contract numbers, applicable work order numbers, work location, work description, and total amount requested. All charges must be itemized using the pay item structure set forth in Exhibit B, with individual line items showing pay item description, quantity performed in linear feet, unit price, and extended amount; lump sum, blended, estimated, or re-labeled pricing is prohibited.
Each invoice must be accompanied by supporting documentation including a copy of the applicable work order, written confirmation of acceptance by the City, and documentation sufficient to verify work completion such as field measurement records, daily logs, as-built or bore documentation, and photographic documentation as required. The City may reject any invoice that is incomplete, lacks required documentation, includes unauthorized or partially completed work, does not conform to Exhibit B, or fails to meet proper invoice requirements. Rejected invoices must be corrected and resubmitted by the contractor and do not qualify as proper invoices for payment timing purposes under the Florida Prompt Payment Act. Each invoice must include a certification statement from the contractor confirming that the work has been fully completed per contract documents, inspected and accepted by the City, and that quantities and amounts are accurate and in compliance with the contract. Payment shall not constitute acceptance of defective or non-conforming work nor waive any City rights under the contract.
