About this file

This is an Insurance Requirements document established by the State of Oregon for contractors providing instructional services under the NFA Leadership Series contract. The document outlines comprehensive insurance obligations that contractors must maintain throughout the contract duration and any extended warranty periods. Contractors are required to obtain Workers' Compensation Insurance with Employers' Liability coverage limits of not less than $500,000 per accident, Commercial General Liability Insurance with minimum limits of $1,000,000 per occurrence and $2,000,000 annual aggregate, and Professional Liability Insurance with minimum limits of $1,000,000 per claim and $2,000,000 annual aggregate. Additional insurance requirements include Automobile Liability Insurance with combined single limits of not less than $1,000,000 for bodily injury and property damage. All insurance must be obtained from entities authorized to transact business in Oregon and acceptable to the Agency, with coverage maintained on a primary and non-contributory basis except for Professional Liability and Workers' Compensation.

The contract requires contractors to provide Certificates of Insurance listing the State of Oregon as a Certificate holder and Additional Insured before performing any services, with all liability insurance except Workers' Compensation, Professional Liability, and certain other coverages including Additional Insured endorsements on, or at least as broad as, ISO Forms CG 20 10 and CG 20 37. Contractors must waive rights of subrogation against the Agency and State of Oregon and provide at least 30 days' written notice before cancellation or material changes to required coverage. If Professional Liability Insurance is provided on a claims-made basis without an extended reporting period of at least 24 months, contractors must maintain continuous claims-made coverage for a minimum of 24 months following completion of services or contract termination. The Agency reserves the right to request complete copies of insurance policies, endorsements, and related documents, and to conduct periodic reviews of insurance requirements with updated provisions as mutually agreed upon.
