About this file

This document is an Application Cover Sheet for Request for Application (RFA) #S-73000-00009423, appearing to be associated with the Oregon Department of Transportation (ODOT). The cover sheet is a comprehensive application submission document that requires potential applicants to provide detailed information and certify compliance with various state and federal requirements. While specific project details are not fully elaborated, the document indicates the application is related to an EVR (Employer Volunteer Recognition) Training Program, with requirements including submission of a training plan, marketing plan, inventory plan, references, and subcontractor list.

The document emphasizes strict compliance requirements, including tax compliance, conflict of interest disclosures, and non-discrimination policies. Applicants must certify they are not debarred, have not been convicted of fraud, and meet specific ethical standards. The application process requires submission through OregonBuys, with a 180-day validity period for submitted information. Key submission requirements include providing references, demonstrating minimum qualifications, and submitting various supporting documents such as class materials, marketing plans, and an EVR Account Holder Agreement. The document also highlights that all submitted applications are considered public records and subject to public inspection after the Notice of Intent to Award.
