About this file
This is a Past Performance Questionnaire template for the Point Mugu Range Air Transportation Services (PMRATS) contract solicitation HTC71126RE137 issued by the United States Transportation Command (USTRANSCOM). The form is designed to be completed by past customers or government agencies that have directly observed a contractor's performance on relevant contracts within the last five years. Respondents are required to submit completed questionnaires to USTRANSCOM by May 26, 2026, via email to marjorie.saurelcook.civ@mail.mil and sonya.m.barker4.civ@mail.mil with "Performance Survey" in the subject line.
The questionnaire requests detailed information about contract identification, including contractor name, role (prime or subcontractor), contract title and number, scope of work, performance locations, period of performance, and contract values. Section 3 requires respondents to rate the contractor's performance across 14 specific service criteria and three overall performance categories using a seven-point rating scale: Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory, Unknown, or Not Applicable. Rating criteria focus on air transportation service quality, including aircraft availability, crew management, FAA compliance, passenger and cargo transport reliability, crew certification, safety, communication during maintenance events, coordination on high-risk missions, mission adaptability, and contingency planning. Section 4 includes narrative questions addressing contract terminations for default or convenience, reservations about future award, security violations, and general performance comments. Marginal or Unsatisfactory ratings require detailed justification and explanation.
