Monthly service CLINS will include all costs (labor and material) associated with providing the service. In addition, the Government has inserted a fixed-price plug-in ceiling for travel costs and will be calculated in the total http://www.sam.gov/ price for Outreach and Communications CLINS (0002, 1002, 2002, 3002, 4002, 5002). Level of Effort (LOE): The Government estimates that this acquisition will require an approximate LOE, as outlined in Part 7 of the PWS. Please note that the Government estimate is only put in place for offerors to ascertain the approximate LOE for this requirement. However, this is not to be construed as either mandatory or necessarily the best technical approach. It is primarily a reference to allow offerors to better understand the general scope of this effort. Offerors shall propose the best LOE, and labor mix required to accomplish the work being requested. Refer to Addendum to FAR 52.212-2 for specific pricing evaluation criteria.

Therefore, the offeror’s initial offer should contain the offeror’s best terms from a price and technical standpoint. However, the Government reserves the right to conduct discussions if later determined by the Contracting Officer to be necessary. The Government may reject any or all offers if such action is in the public interest; accept other than the lowest offer; and waive informalities and minor irregularities in offers received.

(End of provision)

ADDENDUM TO FAR 52.212-2 EVALUATION-COMERCIAL PRODUCTS AND COMMERCIAL SERVICES

a) The government will award a Firm-Fixed Price (FFP) contract, because of this solicitation, to the offeror who represents the best value to the Government. Award of this contract will be made on a competitive best value basis, using a Performance-Price Tradeoff methodology. Three factors (Factor 1-Technical Capability, Factor 2- Past Performance and Factor 3-Price) will be used to evaluate offers. All factors, to include any subfactors, will affect award decision. Factor 2 (Past Performance) is more important than Factor 3 (Price); however, Factor 3 is relevant. The Government will evaluate Factor 1 (Technical Capability) and assign an overall adjectival rating of “Acceptable” or “Unacceptable”. Proposals with an “Unacceptable” rating in Factor 1 will be eliminated from consideration for award. The Government will also evaluate Factor 2 (Past Performance) and assign a past performance confidence rating of “Substantial Confidence”, “Satisfactory Confidence”, “Neutral Confidence”, “Limited Confidence” or “No Confidence”. Lastly, the Government will evaluate Factor 3 (Price) for the 12-month base period, plus four 12-month option periods, to include the six-month extension period. A FFP contract will be awarded based on a best value determination utilizing a Past Performance (Factor 2)/Price (Factor 3) tradeoff methodology for all technically acceptable (Factor 1) offerors. For purposes of the Past Performance/Price tradeoff, Past Performance is more important than Price.

The following factors shall be used to evaluate offers:

FACTOR 1: Technical Capability Sub-factor 1: Staffing/Key Personnel Sub-factor 2: Operations Sub-factor 3: IT/Security Sub-factor 4: Transition-In and Out Plan Sub-factor 5: Deliverables FACTOR 2: Past Performance FACTOR 3: Price

FACTOR 1: Technical Capability- The Government will review the offeror’s technical approach, as well as the proposed staffing plan/matrix, to assess the adequacy to perform the tasks outlined in Part 5 of the PWS. The Government will assign an adjectival rating (Table A-1) to reflect the degree to which the offeror’s proposed approach meets or does not meet the capability requirements. Technical Capability must receive an overall adjectival rating of Acceptable to be considered for award; a rating of Unacceptable renders the entire proposal ineligible for award. All sub-factors are equal in importance to each other.

Table A-1 Technical Ratings

Adjectival Ratings Descriptions

Acceptable The proposal demonstrates an adequate technical approach and understanding of the requirements; therefore, meets the requirements of the solicitation.

Unacceptable The proposal failed to demonstrate an adequate technical approach and understanding of the requirements; therefore, does not meet the requirements of the solicitation.

The Government will use the following terms (definitions) to assess the offeror’s technical approach with consideration of its assigned adjectival rating:

• Meets: aspects of the offeror’s technical approach indicates that the offeror will meet or exceed specified performance or capability requirements, and risk of unsuccessful performance is low.

• Does Not Meet: aspects of the offeror’s technical approach indicates that the offeror will not meet specified performance or capability requirements, and the risk of unsuccessful performance is high.

The technical subfactors that establish the requirements of overall acceptability under Factor 1 are hereby set forth in this solicitation as follows:

a. Sub-factor 1- Staffing/Key Personnel: The proposal must illustrate an adequate staffing approach to effectively meet and execute the scope of work required in support of the inTransition program. Deviations from the Government’s estimated LOE are permitted with the expectations of meeting the tasks in Part 5 of the PWS. While deviations are permitted, if the Government determines the deviation substantially increases the risk of unsuccessful performance, the deviation will be marked as a deficiency, and the proposal will be ineligible for award. The proposal should include a staffing plan/matrix identifying key personnel and all other personnel that will delineate responsibility for the task objectives outlined in Part 5 of the PWS. The proposal shall include a recruiting and retention plan that demonstrates maintaining staffing in support of the tasks in PWS. The proposal shall identify the following key personnel assigned to work under the resulting contract:

• Program Manager (PM)

• Data Analytics SME

• IT SME

• Contact Center Coaches (2)

• Three contact center program staff

Key personnel should meet the minimal requirements listed in PWS Attachment 7.1.4. A one-page resume shall be submitted identifying the Key Personnel’s name, title, years of experience and description of roles and responsibilities to facility the Government’s evaluation of whether the key persons meets the minimum requirements outlined in Attachment 7.1.4. Qualifications of personnel that demonstrate they have the requisite, knowledge, skills, and abilities deemed by the Government to be necessary to minimize risk and successfully complete the requirements in the PWS shall also be included. Resumes should be no longer than 1 page.

The staffing plan shall include a recruiting plan that delineates how staffing will be executed for the remaining contracted personnel in support of Outreach and the Contact Center. The Offeror’s staffing plan must also describe their capacity and strategy to recruit and maintain sufficient licensed mental health professionals. The Offeror must also identify specific plans for how the Contractor will provide supervision, support, and monitoring for contract personnel in accordance with PWS Part 5.1.1.

b. Sub-factor 2- Operations: The proposal must demonstrate an adequate approach to achieving the objectives and scope of work required in accordance with PWS. The proposal must include a Project Management Plan in accordance with Section 1.19. demonstrating its approach to establishing and implementing a fully staffed contact center with licensed mental health professionals and trained contact center personnel to execute the tasks in Part 5. Additionally, the proposal must include a Quality Control Plan in accordance with Section 1.10.1 of the PWS, to demonstrate the offerors’ ability to meet and maintain quality standards. Separate from the Project Management and Quality Control Plan, the offeror’s proposal must demonstrate the following under Sub-factor 2:

• Contact Center Operating: The proposal must illustrate a detailed plan for operation of the contact center and how services will be met during and outside of operating hours. The proposal should also illustrate how outreach/follow-up attempts for program referrals will be implemented.

• Contact Center Volume and Case Tracking: The proposal must include details on its approach to case tracking of all referred names and program enrollees from receipt of name to case closure, as well as contact center volume of various activities.

Similarly, the plan must detail how all metrics within the performance requirements summary in Section 7.1.1 of the PWS can be assessed documented to show program success. The plan should include detailed evaluation strategies that will be implemented in accordance with Section 5.2.2 of the PWS. The proposal must demonstrate the offeror’s capability to document metrics in a “dashboard” type report that can display all metrics.

• Coaching Services: The proposal must demonstrate the offeror’s capability to execute tasks in Section

5.2.7.1. of the PWS.

• Optional inTransition Support: The proposal shall include a detailed plan for staffing and executing Optional inTransition Support in accordance with Section 5.3 of the PWS

c. Sub-factor 3- IT/Security: The proposal must demonstrate an adequate approach and understanding of the information technology and security requirements in accordance with PWS Part 6. All personnel expected to perform under the resulting contract are subject to a background investigation in accordance with PWS 1.16.1 and Key Personnel must initiate a DD Form 2875 to obtain access to the DoD network and applications. In addition, the offeror’s proposal must illustrate having knowledge or familiarity with all DoD policies, manuals, instructions, and procedures as they relate to information systems, software, and security. The proposal shall also identify company protocols in place to maintain and protect sensitive Government information, to specifically include personally identifiable information and protected health information as it is transmitted across platforms and to the Government, as well as program partners, such as VA personnel.

d. Sub-factor 4- Transition In and Out Plan: The proposal must contain a plan that demonstrates the Offeror’s ability to meet all transition-in and transition-out requirements as specified in Section 1.6.1 of the PWS.

e. Subfactor 5- Deliverables: The proposal must demonstrate the Offeror’s ability to fulfill all requirements of the deliverables listed in Technical Exhibit 1 of the PWS.

FACTOR 2: Past Performance- The offeror shall submit a no more than of six (6) past performance references, which may relate to Government and/or Private Sector contracts. Past performance references may be those of the offeror, combination of references of the offeror and/or the offeror’s proposed subcontractors and/or teaming partners. The Government will assess the offeror's present and past performance that meets the recency and relevancy criteria. Recent is defined as services ongoing for a least 6 months and performed within the past four (4) years whether as the prime or subcontractor. Relevant is defined as work similar in size, scope and complexity of the work being requested. The past performance reference shall include a contract#, dollar value, period of performance, name of prime/sub-contractor/or teaming partner, technical POC, relevant description of the services provided and performance rating, if applicable. Subcontractor past performance consent forms are required to be included in the past performance volume (Attachment 6). Past performance concerning subcontractors/teaming partners cannot be disclosed to a private party without the sub/teaming partner’s consent.

Because the prime is considered private party, the Government will need that consent before exchanges with the Government and prime.

The recency and relevancy of the information, source of the information, context of the data, and general trends in the contractor’s performance will be considered. Using the rating and descriptions outlined in the DoD Source Selection Procedures dated August 20, 2022, the government will assign an adjectival performance confidence rating as identified in Table A-2.

Table A-2 Past Performance Rating

Adjectival Ratings Descriptions

Substantial Confidence

Based on the offeror’s recent/relevant performance record, the Government has a high expectation that the offeror will successfully perform the required effort.

Satisfactory Confidence

Based on the offeror’s recent/relevant performance record, the Government has a reasonable expectation that the offeror will successfully perform the required effort.

Neutral Confidence

No recent/relevant performance record is available, or the offeror’s performance record is so sparse that no meaningful confidence assessment rating can be reasonably assigned. The offeror may not be evaluated favorably or unfavorably on the factor of past performance.

Limited Confidence Based on the offeror’s recent/relevant performance record, the Government has a low expectation that the offeror will successfully perform the required effort.

No Confidence Based on the offeror’s recent/relevant performance record, the Government has no expectation that the offeror will be able to successfully perform the required effort.

The Government will use the following terms (definitions) to assess the offeror’s ability to perform with consideration of its assigned adjectival rating:

• Significant strength: an aspect of an offeror’s performance history with appreciable merit or indicates that the offeror will exceed specified performance or capability requirements to the considerable advantage of the Government during contract performance.

• Strength: an aspect of an offeror's performance history that indicates the offeror will exceed specified performance or capability requirements to the advantage of the Government during contract performance and risk of unsuccessful performance is low.

• Significant Weakness: an aspect of an offeror’s performance history that appreciably increases the risk of unsuccessful contract performance.

• Weakness: an aspect of the offeror’s performance history that increases the risk of unsuccessful contract performance.

• Deficiency: An aspect of the offeror’s performance history that indicates an offeror’s inability meet a Government requirement or a combination of significant weaknesses in the offeror’s performance history that increases the risk of unsuccessful contract performance to an unacceptable level.

In the case of an offeror without a record of relevant past performance or for whom information on past performance is not available or so sparse that no meaningful past performance rating can be reasonably assigned, the offeror may not be evaluated favorably or unfavorably on past performance (see FAR 15.305 (a)(2)(iv)). Therefore, the offeror will be determined to have an unknown (or neutral) past performance and considered acceptable. In the context of confidence rating, a neutral rating shall be considered “Neutral Confidence”. For the purpose of evaluation of Factor 2, a “Substantial Confidence” or “Satisfactory Confidence” past performance rating is worth more than a “Neutral Confidence” rating with regards to the Performance-Price Tradeoff.

Past performance information may be obtained from other sources available to the Government, to include, but not limited to, Contractor Performance Assessment Reporting System (CPARS), Federal Awardee Performance and Integrity Information System (FAPIIS), Electronic Subcontract Reporting System (eSRS), or other databases, if necessary.

FACTOR 3: Price- In accordance with FAR 15.305(a)(1), price will be evaluated for the 12-month base, plus four, 12-month option periods to include the six-month extension period. All CLINS are Firm-fixed Price. Proposed pricing in the Excel pricing sheet (Attachment 2) shall include a completed Tab 1 and labor breakdown (Tab 2), inclusive of a fully burden labor rate for each labor category being offered. Offerors shall propose on all contract line items. A line-item price left blank will be considered nonresponsive to this request and the offer may not be considered for award. Offers shall remain valid for a minimum of 180 days from the date of submission.

(b) Option (Extension Provision) price: In accordance with FAR 52.217-8, OPTION TO EXTEND SERVICES (NOV 1999), the Government has the unilateral right to extend any resulting contract under the same terms and conditions for a total of not more than six months. To account for the option period(s) possible under 52.217-8, the Government will evaluate the option to extend by adding six months of the Offeror’s performance period to the Offeror’s total price, if not included in price proposal. The method for evaluating the extension is to multiply an additional six months of requirements by the offered monthly (unit) price of the last option period (Option Year 4). Evaluation of options shall not obligate the Government to exercise the option(s).

(c) The Government intends to award without discussions. A written notice of award or acceptance of an offer, emailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer’s specified expiration time, the Government may accept an offer, whether there are negotiations after its receipt, unless a written notice of withdrawal is received before award.
