SURVEY SCOPE
PBS endeavored to locate all the suspect asbestos-containing materials in the building; however, suspect asbestos-containing materials may be present concealed within wall, ceiling, or floor spaces. If suspect materials are uncovered during demolition activities that are not identified in this report, testing should be performed prior to impact.
PBS has conducted a physical inspection of the building, compiled this report consistent with the survey scope, and certifies that the information is correct and accurate within the standards of professional quality and contractual obligations.
PBS Engineering and Environmental has performed a general asbestos survey of accessible building areas in accordance with OSHA in 29 CFR 1910.1001 and compiled a report with the following information:

The following materials tested negative based on ASHARA sampling minimums and testing by NVLAP participating laboratories.
Although no asbestos was detected, it is possible that further sampling could indicate asbestos content. It may be prudent to test prior to impact through demolition, renovation, etc.

PBS Engineering and Environmental has investigated accessible areas inside of the building(s) to locate suspect asbestos-containing building materials (ACBM). Suspect materials may be present in concealed areas (e.g. behind walls and under carpet). The findings are listed below.
ASBESTOS MATERIALS
The following materials either tested positive, or, based on the experience of PBS field personnel, were not tested and should be considered asbestos-containing. Materials that had mixed results are considered positive. Materials not sampled may not contain asbestos and should be tested to verify asbestos content prior to impact through demolition, renovation, etc.

Asbestos bulk samples obtained from these buildings were collected by an EPA AHERA accredited inspector and analyzed using Polarized Light Microscopy (PLM) with dispersion staining. The results of the analysis are presented in a separate section of this report. See the inspection summary for additional information.

FINDINGS
ASBESTOS SUMMARY
During the period from April 26, 2001 through May 15, 2001, PBS Environmental performed an asbestos-containing materials investigation of the 911 Building located at 911 NE 11th Avenue in Portland, Oregon.
Bulk samples of all accessible suspect asbestos-containing materials were collected by an EPA- AHERA accredited inspector and analyzed by an NVLAP certified laboratory using Polarized Light Microscopy (PLM). No destructive investigation was performed during this inspection.
PBS endeavored to locate and sample all accessible suspect asbestos materials. However, hidden or concealed suspect materials may exist. Materials may exist in areas of the building that were not investigated in the scope of this survey. If concealed suspect asbestos materials are discovered during renovation activities that are not identified in this report, destructive sampling should be performed prior to impacting.
The building reportedly underwent a major renovation during the mid-eighties. During the renovation, the majority of asbestos-containing materials were removed. PBS reviewed prior asbestos survey reports and asbestos abatement drawings to aid in completion of this survey.
Sample data from existing surveys that was used to complete this survey is not included in this report.
Floor coating located in the penthouse mechanical rooms and in the east and west stairwell landings tested positive for asbestos. The floor coating is in poor condition in the stairs. It is chipping and flaking from wear. The material is covered with a heavy coat of paint in the mechanical room areas.
Floor tile in various locations in the building tested positive for asbestos. Asbestos-containing floor tile is located in the penthouse microwave equipment room. The mastic in this room tested negative for asbestos; the T/E rooms on each floor under elevated cable floor; and in the break room on the sub-basement Mezzanine under carpet. Remnant, Red 9 inch floor tile presumed to be asbestos-containing was also identified inside the east and west electrical closets on each floor. All floor tile was observed non-friable and in good condition. It is possible that floor tile exists under the elevated cable floors in the office spaces of the building. The interstitial space under the cable floors was not inspected except where the floors were found exposed.
Hard fitting pipe insulation was identified in two remote locations in the building. The hard fittings are located in the freight elevator shaft at the eighth floor level and in a fan room in the basement. The pipe insulation was not sampled, but is presumed to be asbestos-containing.
Black electrical equipment mounting boards located in the elevator equipment room are asbestos-containing. The boards are inside the electrical cabinets, are ¼ to ¾ inch thick and are non-friable.
An asbestos-containing coating was identified on the exterior of the elevator cars. The coating
911 Building1. 4 material is a non-friable, tar-like material applied to the sheet metal as a sound dampener.
Brake pads on the elevator hoists in the penthouse elevator mechanical room tested positive for asbestos. The brake pads are non-friable and brake dust is present on the hoists.
The elevator doors are lined with an asbestos-containing insulation. The door linings are friable and are enclosed inside the steel doors.
