About this file

This is a Reference Questionnaire form utilized by Collier County, Florida's Procurement Services Division to evaluate vendor performance on past projects. The form is designed to be completed by previous clients of firms bidding on Collier County contracts, specifically for the Uninterrupted Power Systems (UPS) Maintenance, Repairs, Replacement & Installation solicitation (No. 26-8619). Vendors must submit one completed form for each required reference along with their bid or proposal submittal by the December 22, 2025 deadline at 3:00 p.m. The County intends to award Primary, Secondary, and Tertiary vendors to establish a vendor pool, with an initial three-year contract term and an optional two-year renewal period.

The questionnaire evaluates ten specific criteria on a scale of 1 to 10, including project cost management, schedule adherence, work quality, consulting capability, personnel professionalism, project administration, communication clarity, risk management, compliance with contract documents and regulations, and overall customer satisfaction. Evaluators must provide the project description, completion date, budget, and duration for context. Any criteria left blank receives a score of zero. Collier County historically allocates approximately $150,000 annually for UPS services, though actual usage varies. The County requires all service personnel to pass background checks valid for five years, maintain E-Verify enrollment, and comply with OSHA standards. Contractors must provide general liability and workers' compensation insurance, warrant all replacement parts for one year or the manufacturer's maximum period, and adhere to strict markup limitations of 15% for materials and subcontractors and 10% for rental equipment.
