About this file
This is a Reference Questionnaire form developed by Collier County's Procurement Services Division in Florida for evaluating vendor performance on the Uninterrupted Power Systems (UPS) Maintenance, Repairs, Replacement & Installation contract (Solicitation No. 26-8619). The form requires each bidder to submit one completed questionnaire for each reference, with references to be returned as part of the bid or proposal submittal. The questionnaire collects evaluator information (name, company, contact details) and project specifics (description, completion date, budget, and duration) to assess vendor and key personnel performance across ten criteria on a scale of 1 to 10, where 10 indicates strong satisfaction and likelihood of future engagement and 1 indicates dissatisfaction and unwillingness to rehire. Bids are due December 22, 2025, at 3:00 p.m. through the County's online bidding system, with the contract structured for an initial three-year term plus an optional two-year renewal.
The evaluation criteria encompass cost management and change order minimization, schedule adherence, work quality, consultative advice, personnel professionalism, project administration documentation and deliverables, communication clarity, risk management capabilities, contract compliance, and overall customer satisfaction. The County intends to select Primary, Secondary, and Tertiary awardees to ensure service continuity, with pricing structured as unit rates for preventative maintenance and hourly billing for corrective maintenance, subject to strict markup limitations (15% for materials and subcontractors, 10% for rental equipment). Historical annual expenditure for these services is approximately $150,000, though actual usage may vary. All contractors must be enrolled in E-Verify, comply with immigration laws, provide comprehensive insurance coverage, and ensure personnel undergo background checks valid for five years. The County offers a Local Vendor Preference Certification for businesses with physical addresses in Collier or Lee County.
